[Laboratory diagnosis in clinical renal transplants. II. Exfoliative urinary cytology in non-immunologic complications].

5,800 exfoliative cytology smears have been studied during the period 1979-1982. The study was performed as part of the monitoring in a group of patients who underwent kidney transplant at the II Patologia Speciale Chirurgica, University of Rome (Italy). Cytological findings related to non-immunological complications in the first postoperative period are described and discussed in detail. The urinary exfoliative cytology method in the differential diagnosis among ATN, ischemic tubular necrosis and viral infections, tested by sequentially repeated examinations, could allow a prompt diagnosis before the evidence of clinical and hematochemical signs of renal damage.

[Laboratory diagnosis in clinical renal transplants. III. Exfoliative urinary cytology in immunologic complications].

The authors summarize their experiences on cytological monitoring in kidney allografts performed at II Patologia Speciale Chirurgica, University of Rome (Italy), during the period 1979-1982. The different aspects of the urinary exfoliative cytology dealing with the post transplant immunological complications, and the differential cytomorphological criteria are described in detail. A good correlation among cytological, immunological and clinical parameters has been observed in 78% of the examined patients. Moreover the cytological findings allowed in 90% of cases an early diagnosis of rejection, increasing the possibility to successfully prevent an irreversible damage of the graft.

[Laboratory diagnosis in kidney transplants. I. Exfoliative urinary cytology].

Clinical microscopy is a worthwhile method to monitor patients with kidney transplants. The authors describe their experience in 5,800 urinary cytological specimens. The study includes several methods of collecting, processing and staining of the specimens, showing the different cytological pictures: background, hematopoietic-inflammatory cells, renal epithelial cells and casts.

Randomised trial of pentoxifylline versus acetylsalicylic acid plus dipyridamole in preventing transient ischaemic attacks.

In a multicentre trial to compare the ability of a combination of acetylsalicylic acid and dipyridamole (1050 mg + 150 mg/day, group A) to prevent recurrence of transient ischaemic attacks (TIA) with that of pentoxifylline (1200 mg/day, group B), 36 patients received the combination and 30 pentoxifylline. There was no statistically significant difference between the groups as regards age, sex, blood pressure, site of origin of TIA, and incidence of other risk factors. The incidence of recurrent TIAs during 1 year of follow-up was 28% in group A and 10% in group B; this difference was significant (p less than 0.05). The incidence of permanent strokes was similar in the two groups but distinctly lower (4.5%) than that usually reported after untreated TIA.

A randomized clinical trial of pentoxifylline and antiaggregants in recent transient ischemic attacks (TIA). A one year follow-up.

A multicenter comparative trial was conducted in patients with transient ischemic attacks (TIA) to study the preventive capacity of a combination of acetylsalicylic acid and dipyridamol (1,050 mg + 150 mg/day - group A) and of pentoxifylline (1,200 mg/day - group P) in the reduction of morbidity rates. Sixty-six patients, 36 on A and 30 on P, were evaluated. There was no statistically significant difference between both groups as regards age, sex, blood pressure, localisation of TIA and incidence of risk factors. Incidence of new ischemic events during a one year follow up period in the A-group was 28% compared to 10% in the P-group, this difference being statistically significant in favour of P (p less than 0.05). Stroke incidence was similar in both groups but distinctly lower (4.5%) than the natural frequency in TIA.

[Rupture of the transplanted kidney].

Five cases of kidney grafts followed by rupture are described. The explanation of the syndrome accompanying such an occurrence is deduced from personal observations as well as from literature data. As a matter of fact, the aetiopathology of this syndrome cannot be with an isolated cause, for several multifactorial immunological and non-immunological components may be involved.

[Arteriovenous fistulas for hemodialysis. Experience in 366 cases].

The surgical techniques adopted in performing 366 a-v fistulas for hemodialysis in 346 patients over ten years' experience are presented. A survey of the results obtained is then made as well as the evaluation of the same, based upon functional and clinical parameters. The technical details sometimes adopted, the occlusions and the principles drawn out for personal experience in this field are described.

Monitoring and modulation of recipient immune responsiveness to prevent kidney graft rejection in the early post-transplant period.

In 41 consecutive living and cadaver donor renal transplant recipients, immunological monitoring was performed 2--3 times a week for the first two post-transplant months. Monitoring consisted of: 1) Circulating T and B cell levels (E-EAC Rosette assay) 2) T cell reactivity (PHA-Con A) 3) LMC and ADCC reactivity Rejection was diagnosed by standard techniques including radioisotope renal scans and biopsy in some cases. Immunosuppression consisted of prednisone, imuran, cyclophosphamide and horse ALG. In 32 rejection episodes in the first two months, 22 (68%) were associated with a rise in T cell levels. Rejection activity also correlated with an augmented PHA mitogenesis count of 20 +/- 5%. There was no positive correlation between Con A mitogenesis and rejection. There was also no correlation between rejection and circulating B cell levels. There was no significant correlation between a positive ADCC and graft rejection. Futhermore a positive ADCC in association with a negative LMC resulted in excellent long-term graft function. In conclusion, an excellent correlation of levels of circulating T cells and T cell reactivity with early in vivo rejection was shown.
