Sarcoidosis in untreated chronic hepatitis C virus infection.

Chronic hepatitis C has been implicated in the pathogenesis of sarcoidosis in several cases of patients treated with interferon-alpha. On the other hand, only in a few cases previously has a possible link between sarcoidosis and untreated chronic hepatitis C virus infection been demonstrated. We report on a patient with chronic hepatitis C who developed cutaneous sarcoidosis without prior interferon-alpha treatment. We hypothesize that viral persistence seen in chronic hepatitis C virus infection might be one of the potential factors that trigger cellular immune response in granulomatous reactions as seen in sarcoidosis, in genetically predisposed patients.

Transfusion-mediated Yersinia enterocolitica septicemia in an adult patient with beta-thalassemia.

We report a case of transfusion-mediated Yersinia enterocolitica septicemia in a 43-y-old woman with homozygous beta-thalassemia. Two h after transfusion of 3 units of red blood cells the patient suffered high-grade fever and shaking chills. Y. enterocolitica serotype O3 grew in blood cultures. Prolonged treatment with i.v. ceftriaxone plus ciprofloxacin led to a favorable outcome. Transfusion-associated Y. enterocolitica septicemia has not previously been reported in an adult beta-thalassemic patient from the Mediterranean area. Our report is particularly important, because of the high incidence of chronically transfused thalassemic patients in Mediterranean countries.

Cytogenetic study of 11 gastric adenocarcinomas.

We describe the cytogenetic findings in 11 gastric cancer cases. Nine cases showed a variable number of numerical and structural aberrations, while two cases had a normal karyotype. The chromosomes most frequently involved are chromosome 3 in six cases, 6 in four cases, and 13 in three cases. They mainly exhibited structural aberrations. An additional segment on chromosome 3p was observed in three cases, while two cases had a del(3q). A der(5) chromosome resulting from a translocation between chromosomes 3 and 5 was observed in two cases. Both cases had also a del(6q), a der(12), and an add(17p) chromosome as common abnormalities. Deletion of 6q chromosome distal to band 6q21 was observed in three cases. Trisomy 8 as a sole anomaly was found in one case. Our cytogenetic findings are discussed in relation to the reported in the literature data.

Cytogenetic study in multiple myeloma.

Ten patients with multiple myeloma (MM) were studied cytogenetically, using the G-banding technique. It was found that five patients had a normal karyotype, whereas five patients exhibited various chromosomal abnormalities. The presence of marker chromosomes was a consistent finding. Among those, the 14q+ marker chromosome was present in three cases, partial or complete trisomy for 1q was detected in four cases, and chromosome #6 was involved in two cases. In one case the 14q+ marker chromosome was determined to result from a translocation between chromosomes #11 and #14. In one patient with Bence Jones kappa multiple myeloma, there was a translocation between chromosomes #2 and #8.

Correlation of haematological, electron microscopic and cytogenetic findings in 20 patients with preleukaemia.

A combined haematological, ultrastructural and chromosomal study was made in 20 patients with preleukaemia. There was evidence of specific correlations between (a) Hb F levels and pancytopenia or profound anaemia, (b) chromosomal and electron microscopic findings and (c) thrombocytosis and ringed sideroblasts. The above findings might be of importance in identifying subgroups of preleukaemic patients with different prognosis.

Vaccination against hepatitis B virus in workers of a general hospital in Athens.

BACKGROUND: Protection against hepatitis B virus infection by vaccination is considered to be an important preventive measure for health care workers. OBJECTIVES: Investigation of vaccination coverage against hepatitis B virus in health care workers of a regional general hospital in Athens and assessment of predictive factors possibly associated with the likelihood of vaccination. METHODS: In a cross-sectional study, a questionnaire survey was carried out on 175 health care workers of a regional general hospital in Athens. The questionnaire included questions concerning socio-demographic factors, HBV vaccination status and reasons for non-vaccination, as well as questions about knowledge of routes of transmission and the complications of HBV infection. Compliance with preventive practices against HBV infection was also assessed. RESULTS: Overall vaccination coverage was 57.1%. Logistic regression analysis revealed that occupation and use of gloves were independently associated with the likelihood of vaccination against HBV Doctors recorded an odds ratio (OR) for vaccination of 4.45 in comparison with nurses/paramedics. Health care workers who wore gloves always/most times demonstrated an odds ratio of 2.79 for vaccination against HBV in comparison with those who never/rarely used them. CONCLUSIONS: Our study illustrates the characteristics of health care workers with lower prevalence of vaccination against HBV in a representative sample of these workers in a regional general hospital in Athens: nurses/paramedics, those with low level of compliance with use of gloves, and those in non surgical departments. Educational interventions as part of a vaccination programme and continuing education on universal precautions could help increase HBV vaccination coverage among health care workers of the hospital under study.
