Nutrition and exercise in children.

Various surveys and research studies carried out in the scientific world have expressed general consensus on the preventive role of regular physical exercise associated with a correct and adequate diet. A sedentary style of life is the common etiopathogenetic basis of "paramorphism" of the different organs and system that generate the hypokinetic syndrome. Therefore it is important to adopt a correct life style from childhood. For this purpose 1208 subjects aged between 6 and 14 years, in an Italian rural area, have been studied for nutritional habits, anthropometric parameters, nutritional state and motor abilities (Di.S.Co. project:experimental community project for preventing chronic-degenerative diseases). The population was divided in: subjects participating to organized sport activities and sedentary subjects; and then subjects of different ages between 9 and 14 years; results regarding anthropometric parameters, body composition and the predominance of overweight and obesity are given. Our study has shown widespread tendency to incorrect nutrition habits, also in those doing organized sports, and a low attitude to volontary physical activity. School represents a privileged institution for releasing and promoting correct life style beginning from the younger age.

Reinfusion potassium blood cardioplegia versus cold blood reinfusion alone in elective revascularization.

The purpose of this study was to determine if the addition of potassium to reinfusion cold blood cardioplegia (CBC) offers an advantage over cold blood alone. Forty patients matched for age, left ventricular function, extent of coronary disease and number of vessels bypassed were prospectively randomized. Each patient received an initial dose of CBC (10 cc/kg) with potassium. Group I patients (n = 23) received subsequent infusions of CBC (5 cc/kg) containing potassium while Group II patients (n = 17) received cold blood only. The cross-clamp time, mean infusate volume and temperature were not significantly different in the two groups. Following reperfusion, the cardiac index and the CPK isoenzyme release at 0.5, 1, 8, and 12 h after cross-clamp release were not significantly different between the groups. The postoperative appearance of new Q-waves, inotropic agent requirement, and reversal of the lactate dehydrogenase (LDH) isoenzyme ratio were also not significantly different in the two groups. The study demonstrated that following initial arrest with potassium, cold blood is equally as effective as potassium blood cardioplegia in protecting the ischemic myocardium.

Cerebral mucormycosis associated with intravenous drug use: three case reports and review.

We describe three cases of cerebral mucormycosis in intravenous drug users and review 22 previously reported cases. Involvement of the basal ganglia was demonstrated in all but two cases. Seven of the 10 patients tested for antibodies to the human immunodeficiency virus (HIV) were seronegative. Eight of the 25 patients survived and were discharged from the hospital; for 7 of 10 patients, cultures of brain lesions yielded Rhizopus arrhizus. The radiographic findings varied, and in most cases, no or minimal contrast enhancement was seen in the initial computed tomography scans. Although uncommon, the diagnosis of cerebral mucormycosis should be considered when basal ganglia lesions are present in an intravenous drug user, regardless of previous exposure to HIV.

Extracorporeal membrane oxygenation in children after cardiac surgery.

BACKGROUND: From August 1982 to May 1991, 65 children (32 boys), 1 day to 14 years old, received extracorporeal membrane oxygenation (ECMO) 0-50 hours after cardiac surgery. Forty-four (67.7%) were weaned, with 23 (35.4%) survivors. METHODS AND RESULTS: Analysis of 29 pre-ECMO factors including diagnosis, age, sex, blood gas data, systemic pressures, atrial pressures, ventricular function, and renal function was performed. Preoperative systemic ventricular shortening fraction was statistically less in survivors. The need for dialysis and length of support were predictors of survival once ECMO was initiated. There were five late deaths 6 days to 5 years after discharge; none were related to ECMO. The remaining 18 patients have been followed for a mean of 37.5 months (range, 1-85 months). Seventeen are New York Heart Association functional class I, with one patient still hospitalized. CONCLUSIONS: ECMO allows for myocardial recovery in the majority of patients with refractory postcardiotomy failure and permits some patients to survive who would not have otherwise.

A double blind randomized pilot trial of naloxone in the treatment of acute ischemic stroke.

Attention has focused on naloxone, an opiate receptor antagonist, because of its potential benefit in reversing neurological damage after acute cerebral ischemia. To evaluate the safety and possible efficacy of high-dose naloxone in ischemic stroke patients we planned a double blind pilot study. Between January 1989 and May 1990 24 patients were randomly assigned to the naloxone or placebo group according to age and neurological deficit. Naloxone was given in a loading dose of 5 mg/kg over 10 minutes followed by a 24-hour infusion at the rate of 3.5 mg/kg/h. 10 patients experienced minor side effects but none of them had to discontinue the treatment. 9 patients improved: 6 in the naloxone group and 3 in the placebo group, but no significant difference was found using the non parametric Mann-Whitney test. Our study suggests that naloxone is safe at the dose used, but the results do not support the planning of similar trials on a larger scale.

Reliability of echocardiography in the diagnosis of anomalous origin of the left coronary artery from the pulmonary trunk.

Previous studies have indicated that the definitive diagnosis of anomalous origin of the left coronary artery from the pulmonary trunk (ALC) should be made by cardiac catheterization and angiography. This study evaluates echocardiography (two-dimensional, pulsed Doppler, and color flow mapping) as a method to establish the diagnosis of ALC. To diagnose ALC, a modified parasternal short-axis view was used to demonstrate continuity of the ALC with the pulmonary trunk and to detect the retrograde flow through the ALC into the pulmonary trunk. Absence of these imaging characteristics ruled out ALC. From June 1985 to January 1990, 16 patients who presented with or had previously had a dilated poorly contracting left ventricle were prospectively assessed by echocardiography to rule out ALC. Four patients had ALC (age 2 to 120 months, mean +/- 1SD = 32 +/- 59) and 12 patients (age 1 to 192 months, mean +/- 1SD = 57 +/- 80) had myocardiopathy. Two other patients with known ALC were evaluated by an observer unaware of the diagnosis. All coronary anatomy was confirmed by angiography, surgery, or autopsy. The correct diagnosis of coronary anatomy was obtained by echocardiography in all instances without false positive or false negative diagnosis of ALC. Three infants underwent surgical repair of ALC based only on the echocardiographic diagnosis. Echocardiography can be used to establish the diagnosis of ALC. Therefore surgical repair can be undertaken in some critically sick infants based on the echocardiographic diagnosis alone.

Evaluation of the effects of L-acetylcarnitine on senile patients suffering from depression.

Twenty-eight patients aged between 70 and 80 years affected by depressive disturbance as defined by DSM III R (cat. 300.40) were subdivided at random into two homogeneous groups of 14 each. One group was treated with 500 mg three times a day of L-acetylcarnitine (LAC) in tablet form, while the other received placebo. Each patient was evaluated by the Hamilton Rating Scale for Depression, the Beck Depression Inventory, the Sandoz Clinical Assessment--Geriatric, and by clinical global impression. This investigation establishes that LAC is effective in counteracting symptoms of depression in the elderly. Relief of depressive symptomatology is expressed by decreased scores in the Hamilton Rating Scale for Depression and Beck Depression Inventory and by beneficial effects with regard to behavioural aspects.

Multiple penile horns: case report and review.

We report a case of unusually large multiple penile horns following removal of condylomata acuminata. Penile horns can grow rapidly, although malignant degeneration is uncommon. Wide excision with deep biopsy of skin at the base of the lesion probably is appropriate treatment.

Methylmethacrylate upper ureteral obstruction.

A case of upper ureteral obstruction caused by methylmethacrylate cement after fixation of an L2-L3 vertebral fracture is reported. The probable mechanism, radiographic findings, and management of this complication are reviewed.

Neonatal tuberous sclerosis presenting with intractable seizures.

A 2-day old girl with status epilepticus, unresponsive to maximum pharmacological intervention, is reported. Findings of brain and cardiac lesions pointed to the diagnosis of tuberous sclerosis. One of the brain lesions was unusually large, occupying most of the right temporo-parietal lobe.

Nitrogen oxides from waste incineration: control by selective non-catalytic reduction.

An experimental study of the selective non-catalytic reduction (SNCR) process was carried out to determine the efficiency of NOx removal and NH3 mass balance, the NOx reducing reagent used. Experimental tests were conducted on a full-scale SNCR system installed in a hospital waste incineration plant. Anhydrous NH3 was injected at the boiler entrance for NOx removal. Ammonia was analyzed after each flue-gas treatment unit in order to establish its mass balance and NH3 slip in the stack gas was monitored as well. The effective fraction of NH3 for the thermal NOx reduction was calculated from measured values of injected and residual NH3. Results show that a NOx reduction efficiency in the range of 46.7-76.7% is possible at a NH3/NO molar ratio of 0.9-1.5. The fraction of NH3 used in NOx removal was found to decrease with rising NH3/NO molar ratio. The NH3 slip in the stack gas was very low, below permitted limits, even at the higher NH3 dosages used. No direct correlation was found between the NH3/NO molar ratio and the NH3 slip in the stack gas since the major part of the residual NH3 was converted into ammonium salts in the dry scrubbing reactor and subsequently collected in the fabric filter. Moreover, another fraction of NH3 was dissolved in the scrubbing liquor.

A multistate outbreak of Salmonella enterica serotype Baildon associated with domestic raw tomatoes.

Salmonella enterica serotype Baildon, a rare serotype, was recovered from 86 persons in eight states; 87% of illnesses began during a 3-week period ending January 9, 1999. Raw restaurant-prepared tomatoes were implicated in multiple case-control studies. Contamination likely occurred on the farm or during packing; more effective disinfection and prevention strategies are needed.
