[Tympanoplasty using chondro-perichondral graft. Indications, techniques and results. Apropos of a series of 127 cases].

In an attempt to avoid recurrence, the authors have used chondro-perichondral grafts for more than 10 years for tympanoplasty of retraction pouches. Early results were published in 1987. This new retrospective series of 127 tympanoplasties, performed from 1987 to 1991 reports the anatomic and functional results obtained and the technical modalities used. After a mean follow-up of 18 months, the results appear encouraging. Indications of this chondro-perichondral graft have been widened to cases of unsuccessful first intention myringoplasty and to certain cases of progressive perforations occurring in a setting of inflammation or tubal dysfunction. The results must be confirmed on the basis of long term outcomes.

[D-penicillamine in the treatment of recurrent laryngeal stenosis in children].

Five children with recurrent tracheal stenosis after use of an endolaryngeal calibrator and/or a laryngoplasty were treated with D-penicillamine and chloroquine. The relevant literature relating to biological processes of normal and pathologic healing is reviewed, particularly with respect to the effect of D-penicillamine on collagen synthesis. Decannulation was possible in 4 of the children reported presently, including three in whom medical treatment was combined with use of an endolaryngeal calibrator.

[Tympanic retraction pockets. Evaluation after 3 years of our therapeutic regimen. Apropos of 106 cases].

A retrospective study of 106 retraction pockets has been achieved: Part of them was treated with ventilation tubes (23 cases) and an other part was treated by tympanoplasty (83 cases). Our purpose was to verify if our indications were wright. We concluded that it is correct to operate the fixed retraction pockets and it is not dangerous. Concerned the indication of ventilation tubes in non-fixed retraction pockets we do not concluded but propose a complementary study.

[Functional surgery of major aplasia of the ear. Apropos of 109 patients].

Based on data from 109 patients operated upon for 125 major aplasias of ear, clinical and operative aspects and results of surgery are analysed. Functional prognosis is related essentially to the nature of the stapes, normal in 66% of cases, and the quality of healing of the newly formed canal. One auditive result out of four in cases of unilateral aplasia and one out of two in bilateral cases constituted the proportion of patients obtaining socially valuable postoperative auditive gain. Functional surgery for aplasia of ear should therefore be reserved mainly for bilateral forms because of the lack of any alternative to apparatus wearing.

[Laryngeal dyspnea of infectious origin in the adult. Apropos of 54 cases].

Principal etiologies in 54 patients with acute laryngeal dyspnea of infectious origin were epiglottitis (35 cases), glotto-subglottal laryngitis (8 cases) and adjacent infections (9 cases). These cases are reviewed together with the relevant published literature and various conclusions drawn. The affection is serious (35 deaths out of 425 cases in the literature), and requires careful examination, immediate treatment and routine hospital care under surveillance. Apart from cases where the condition of the patient necessitates urgent intubation, those cases with rapidly evolving signs over 24 hours must be admitted to intensive care since worsening of dyspnea can lead to delayed decompensation. Wide spectrum antibiotic therapy is necessary for patients with epiglottitis and adjacent infective ulcerations since many germs may be implicated, while for the always benign glotto-subglottal laryngitis Amoxicillin + clavulinic acid is sufficient.

[Functional surgery of cancer of the hypopharynx. Hemilaryngopharyngectomy, posterior pharyngectomy by bilateral cervical approach].

The authors report on 48 patients operated by hemi-laryngo-pharyngectomy for spinous carcinoma of the piriform sinus. They try to precise the indications by analysing the factors influencing survival and relapses. They compare surgery to the other methods. In a second part, they report 3 cases of posterior pharyngectomy by bilateral cervical incision.

[Choice of the 3d portion of the jejunum for reconstruction of the pharynx in circular pharyngolaryngectomies. Indications and advantages].

Many procedures are used for reconstruction of a pharyngeal gutter after circular pharyngolaryngectomy, interrupter of digestive continuity. The operation chosen was used of third part of jejunum for reconstruction, and its indications and limitations are described. The choice was based on anatomic data from dissection of 100 cadavers and values for different measurements performed (length of transplant, height of mesentery, caliber and length of vessels), and more particularly on constancy and reliability of vascularization of this digestive segment. To these advantages are added bacteriologic, technical, histologic and physiologic reasons. Results are analyzed of 11 operations carried out in the same department, from among a larger group comprising now 45 cases. Postoperative course was usually uncomplicated with rapid restoration of deglutition (a mean of 11.77 days in 9 of the 11 cases). Preoperative radiotherapy is not a contraindication and postoperative radiotherapy should be given routinely as a complement to surgery. It is usually well tolerated and transplant necrosis is not provoked. Nevertheless, the seriousness of this type of lesion is underlined by the high incidence of carcinologic failures, with an elevated risk of local and regional recurrence and of distant metastases.

[Functional radical neck excision. Evaluation of 10 years' experience. Apropos of 434 cases].

A total of 434 functional évidements of neck were conducted in 313 patients, taking into account all primary localizations. Details of the procedure used are described while emphasizing the need for an évidement tending towards one as complete as a radical operation. Surgery was usually elective (89% of cases) but in 11% considered as essential. A high proportion of lymph nodes classified as no were, however, found to be invaded (22.8%, including 27.5% of these with capsule rupture). Despite this, glandular recurrence on the side of functional évidement was extremely low (less than 7%) even when recurrence was epithelial or involved a second localization. These findings emphasize the validity of functional évidement and confirm published data, known to be difficult to analyze. Although the value of this operation with respect to the cancer is well established, the actual functional interest, particularly in relation, to external branch of XI th nerve is debatable.
