On the distribution and elimination of haloperidol in cholecystectomized patients.

A single oral dose of haloperidol was given to 19 patients prior to cholecystectomy. The concentration of haloperidol was assessed in serum, urine, fatty tissue, liver tissue, choledochal bile and bile bladder contents. The biological half-life of the drug in the serum was 25.1 +/- 12.9 hours, which is comparable to that in healthy volunteers. Only minute amounts of the drug were excreted with the urine. The drug concentration was only slightly elevated in choledochal bile but in the bile bladder contents it was about 11 times higher than that in the serum. The low concentration of haloperidol in the bile indicated that there was no substantial enterohepatic recirculation of the drug in this patient material. The concentration of haloperidol in fatty tissue was about 20 times, and in liver tissue 900 times higher than that in the serum. The corresponding figures for intramuscular administration of haloperidol were 10 and 325, respectively. A possible relationship between the side-effects and the route of administration is discussed.

Serum haloperidol determinations in psychiatric patients. Comparison of methods and correlation with serum prolactin level.

Twenty-one serum samples from 11 schizophrenic patients receiving long-term haloperidol therapy were analyzed for haloperidol concentrations by two different radioimmunoassays (RIAs) and gas chromatography (GC). There was a good correspondence between the RIA and GC values over a wide range of drug concentrations. However, compared with the specific GC technique, both RIA methods overestimated haloperidol concentrations, reflecting differences in the specificities of the two RIA antibodies. One of the RIA methods had the requisite specificity for application to patients treated with long-term haloperidol therapy, although further methodological refinement will be required for its general clinical application. Haloperidol values determined by GC and RIA analyses correlated highly with prolactin concentrations in the same samples, suggesting that the usefulness of prolactin measurement as an "in vivo bioassay" for circulating levels of haloperidol should be further explored.

Abscesses following prolonged intramuscular administration of perphenazine enantate.

In an investigation of 109 patients on neuroleptics in a vegetable oil base, four of the patients developed abscesses at the site of deposits of perphenazine enantate given intramuscularly in large doses every 1-4 weeks. Observations suggested that the risk of such a complication could be minimized by limiting the frequency of administration to once a fortnight, by limiting the dose injected, by frequent variation of the site of injection, and by regular use of a disinfectant.
