[Sickle cell anemia in surgical patients. Significance and problems].

Drepanocytosis, a disease notoriously widespread among blacks, is surprisingly frequent in certain Italian islands. Surgical aspects of the disease are discussed: erythrocyte sequestration attacks, pigmentary cholelithiasis, priapism, malleolar ulcers and vaso-occlusive attacks. A knowledge of such aspects is considered fundamental for the purposes of a reasoned differential diagnosis in the field of paediatric pathologies requiring surgery.

[Anesthesia and sickle cell disease].

Sickle cell anaemia is the most common hereditary haemoglobin pathology. It is found in either a homozygous or heterozygous form, associated in the latter case with other haemoglobinopathies. In view of the pathogenesis and the various related imbalances, amply confirmed by others, which can well prove disastrous, the pre, per and post-operative precautions to be adopted in such patients are assessed.

[Experimental intrauterine surgery. Morphological study of lung development in the sheep fetus with congenital diaphragmatic hernia].

Experimental comparison of the lungs of 7 sheep foetuses with surgically induced CDH and 7 controls permitted an assessment to be made of the changes that take place in lung growth, generally described as hypoplasia, through a study of their morphology and histology, and the lung: lamb weight ratio. Changes increased in gravity in function of the duration of hernia. They included: reduced alveolar expansion, fewer generations of bronchi and alveoli, and septal thickening. An increase in the smooth muscle component of 5th-6th generation arteries (i.e. resistance) may offer an explanation of the hypertension characteristic of CDH, and the non-reactivity of these vessels in response to vasodilators. It is also suggested that damage to the mesenchyma can be regarded as the sole cause of the changes in lung growth observed in CDH. Early treatment before these changes become irreversible is thus advisable.

[Experimental intrauterine surgery: the creation of a congenital diaphragmatic hernia in a sheep fetus].

Up to 50% of the neonates operated during the first hours of life for a congenital diaphragmatic hernia die. The presence of a severe lung hypoplasia, which is at the origin of hypoxia, acidosis, increased pulmonary vascular resistance and right to left shunt, explains the poor clinical results, in spite of surgical success and intensive therapy. A modern approach to the problem includes treatment with pulmonary vasodilator drugs, whose effects are still discussed. Perhaps the prenatal recognition of the defect could improve prognosis. Lots of experience are needed to know more on the subject. This could be obtained with an experimental approach on newborn animals with a CDH. The article relates authors experience in the creation of a CDH in a foetus of sheep, by means of a surgical intervention during its intrauterine life.

[Hirschsprung's disease in adolescents and adults].

Megacolon must be considered as a diagnosis in any case of intractable constipation also in the adult. Besides the Hirschsprung's disease and the idiopathic megacolon, which have no apparent organic cause, many other pathologic conditions may produce a marked dilatation of the large bowel. The differential diagnosis depends, after a valuation of the clinical and radiologic features, also from enzymatic, histologic and functional tests. The Hirschsprung's disease can be observed after the infancy in: 1) Patients in whom diagnosis is made only in adult age; 2) Patients with a diagnosis made in infancy or childhood, with moderate symptoms, treated with a conservative methods, who are getting worse; 3) Adult relapsers after a previous inadequate operation. Group II also includes the patients who need an emergency operation, because of a perforation or a volvolus of a dilatated loop. We report three cases of adult Hirschsprung's disease and two adult "idiopathic megacolon". The patients with aganglionic megacolon were admitted to our Hospital respectively for an abdominal palpable mass, a volvolus and incontinence. The two patients with idiopathic megacolon showed the same clinical picture of those with Hirschsprung's disease. We choose a surgical therapy in all five patients, with good long-term results.

[Euglycemizing effect of partial jejuno-ileal resection in rats treated with streptozotocin].

Resection of about 50% of the small intestine was performed in rats diabetised with streptozotocin. The first results show that this operation leads to normalisation of sugar metabolism that is partly referable to partial malabsorption, but above all to loss of weight. Current knowledge with regard to the relation between diabetes and obesity is discussed and an account is given of the modern surgical approaches to the treatment of major obesity. Lastly, the suggestion is made that limited digestive bypass may be indicated in cases of insulin-independent diabetes where weight loss and control of blood sugar cannot readily be achieved by conventional medical management.

Suprarenal abscess in a newborn.

A case report of a new born with unilateral suprarenal abscess extending in to the right kidney is represented. Both the adrenal gland and the kidney had to be surgically removed. Early diagnosis is important, so the surgical intervention, the only successful therapy, can be kept as conservative as possible.

[Possibilities of surgical correction of so-called "irreparable" bile duct atresia].

Atresia of the bile ducts has always been regarded as congenital. Recent clinical and experimental findings, however, suggest that it may be acquired. In the absence of a more precise definition, classification is still based on the anatomopathological distinction between forms that can and cannot be corrected. An account is given of the possibilities offered by surgery in the correction of so-called irreparable forms and the results obtained with hepatoenterostomy (Kasai's operation).

Ureterosigmoidostomy and cancer of the colon. Report of a case.

A possible and quite frequent complication of uretero-sigmoidostomy can be the late development of a colic cancer at the site of the ureteric reimplantation. On the basis of the literature and of a personal observation, the accent is put on what can be done to discover the tumor in any early phase; the possibility that the increased risk of cancer should limit the colic urinary diversions for an exstrophy of the bladder is also discussed.

[Hepatic hamartoma. Description of a case in a child].

A case of hepatic hamartoma in a child of 13 months, treated with atypical resection of the right lobe of the liver, is reported. The tumour was of whitish colour and weighed 800 g. Its size was 16 X 9 X 10 cm and its section revealed a hyaline-fibrous appearance with reddish areas and others of cystic appearance. At follow-up, the child did not complain of digestive trouble and grew normally.

[Scintiscanning with technetium-99m pertechnetate in the diagnosis of Meckel's diverticulum].

Personal experience with technetium99m pertecnetate scanning in the diagnosis of Meckel's diverticulum is reported. The data reported indicate that the scan can be considered positive when the lower right quadrant of the abdomen shows a definite accumulation of radioactivity. Of 12 children examined, 4 responses were positive. These 4 patients were submitted to surgery and this confirmed the presence of Meckel's diverticulum containing gastric mucosa, and of an ulcer in the adjacent ileal mucosa. After discussing the clinical value of the investigation and having examined the problem of false positives and false negatives, in relation to reported data, it is concluded that scanning with technetium has a vital role to play in the diagnosis of haemorrhage due to Meckel's diverticulum.

A radiopharmaceutical in the diagnosis of congenital biliary atresia.

Diethyl-HIDA, a new hepatobiliary radiopharmaceutical labeled with 99mTc, was used in the preoperative diagnosis of biliary atresia and in the postoperative evaluation of hepatic-porto-enterostomy function. The good results obtained in the four first cases suggest extensive use of the method, with the aim of a more precocious and more attendable diagnosis.

Gastric volvulus associated with congenital diaphragmatic hernia, wandering spleen, and intrathoracic left kidney: CT findings.

We present an unusual case of gastric volvulus associated with wandering spleen, a delayed manifestation of congenital diaphragmatic hernia and left intrathoracic kidney. Gastric volvulus should be considered in any infant with unexplained vomiting and left diaphragmatic anomaly: in these patients, developmental disorders of the peritoneal visceral attachments of the left upper abdomen may coexist. The absence of ligamentous connections between the stomach, posterior abdominal wall, and spleen result in wandering spleen. We emphasize prompt surgical therapy to avoid gastric and splenic necrosis. Radiologic findings and the appearance of this complex congenital malformation are reported.

[Intestinal invagination and Yersinia enterocolitica infections].

After a short review of literature about the few previous reports a case of intussusception associated with Yersinia enterocolitica in a 11 month-old infant is reported. The causal significance of mesenteric lymphoadenomegaly is discussed. Intussusception as the first clinical sign of Yersinia infection is suggested.
