Remnants of the fourth branchial arch are uncommon clinical entities. They may present in either the neck or chest in association with aortic arch development. They are encountered more frequently in younger individuals as an asymptomatic left-sided neck mass, recurrent neck abscess or suppurative thyroiditis. In view of the rarity of this lesion, we thought it worthwhile to present the case of a 25-year-old female with a diagnosed fourth branchial cleft cyst in her left lower neck. The embryology and management of these cases is discussed.

The paper describes a large pigmented maxillary tumor in a 46-day-old infant. The histological examination showed the tumor to be a melanotic neuroectodermal tumor of infancy (MNTI). No raised urinary levels of vanilmandelic acid could be shown. The tumor was surgically removed and during a 5 1/2-year follow-up period there was no clinical evidence of recurrence. The origin, behavior and treatment of these rare tumors are discussed.
Measurements of intraocular pressure (IOP) by applanation tonometry in twelve patients undergoing lens extraction showed that a normocapnic anaesthetic technique using 0.4% trichloroethylene with controlled ventilation of the lungs (IPPV) with large tidal volumes (14 ml/kg) reduced IOP by 13--20%. There was only a small reduction in arterial pressure. Normocapnia was easy to achieve by use of the single-limb co-axial Penlon (Bain type) anaesthetic breathing circuit in conjunction with an electrically-driven, small and inexpensive lung ventilator. The anaesthetic technique described using trichloroethylene is suitable for lens extraction surgery when it is desired to avoid a halothane anaesthetic for any reason.
Measurement of intraocular pressure (IOP) by applanation tonometry in 23 patients undergoing lens extraction showed that a normocapnic general anaesthetic technique with controlled ventilation of the lungs (IPPV) reliably reduced the IOP by 50% for the duration of the operative period. This was not associated with large falls in systemic arterial pressure which are often a feature of spontaneously breathing halothane anaesthesia for eye surgery. Normocapnia and IPPV were easy to achieve by use of the single-limb coaxial Bain anaesthetic breathing circuit in conjunction with an electrically driven, small, and inexpensive ventilator while the anesthetic mixture of 33% oxygen and 0.5% halothane in nitrous oxide was delivered at a rate of 70 ml/kg body weight per minute.

Alcohol was shown to enhance the carcinogenic effect of DMBA applied to hamster buccal pouch. DMBA animals given alcohol developed dysplastic leukoplakic lesions and epidermoid carcinomas 2 weeks ahead of water-drinking control animals. The tumors also grew to larger size, were more invasive, and were more anaplastic histologically. Forty young adult Syrian hamsters were divided into four groups (alcohol-DMBA, DMBA control, untreated control, and alcohol control) and were killed after 10, 12, and 14 weeks of DMBA applications three times weekly. Alcohol was given as a 10 percent solution instead of drinking water.
The psychoanalysis of a case of an unusual perversion demonstrated the historical determinants of the ritual. Drive and defense, bisexual disposition, and identifications were represented. At the beginning of the analysis the patient appeared to have borderline character traits; but as the analysis unfolded, an oedipal situation was revealed and character development occurred. The factors that determined the particular outcome for this patient, instead of homosexuality, or transsexualism, are discussed. In some cases of perversion, such as the one herein described, only psychoanalytic exploration can determine whether a level of personality structure and object relations had been reached that allows for the analysis of the perversion and a shift to heterosexuality and mature object relations.
The significant feature of the reported case was that of a sinister-looking mandibular tumor highly suggestive of a metastatic lesion or of osteogenic sarcoma. A biopsy specimen and results of a thorough medical work-up showed the lesion to be multiple myeloma. The myeloma cells apparently have the capability to stimulate osteoblastic activity and new bone formation. This capability should be kept in mind when making the differential diagnosis of osteoblastic bony lesions of the jaws.

A case of osteochondroma of the coronoid process is presented. The proper diagnosis of ill-defined symptoms in the temporomandibular joint requires conformity between clinical manifestations and radiologic findings. The importance of a thorough radiologic examination is stressed for the correct diagnosis of these rare, insidious, slow-growing tumors. A short review of the literature is presented, and the treatment and surgical approach to these tumors are discussed.
A case of osteomyelitis of the maxilla in a healthy infant is presented, the etiology of which seems to have been an upper respiratory tract infection. There was no evidence of lowered resistance and immunologic studies did not reveal any abnormality. Lincomycin seems to be the antibiotic drug of choice. In the present case, it was used, effectively, in conjunction with other antibiotics and surgical drainage.
An extremely rare tumor of the oral cavity, liposarcoma, is reported. The tumor was located in the submucosal layers of the cheek and protruded into the mouth. To the best of our knowledge this is the fourth reported instance of liposarcoma of the cheek in the world literature. Combined surgical excision and radiotherapy proved a success as judged by a 3-year follow-up.

In some patients receiving megadose infusions of methotrexate (MTX), the rate of drug clearance decreases with each subsequent infusion. This can be modified by administration of fluids as outlined in the Recommended Protocol. Monitoring of each treatment is needed to assess requirements of fluids since these cannot be adequately predicted from data of previous infusions.
