Disseminated histoplasmosis presenting with cutaneous lesions in a patient with acquired immunodeficiency syndrome.

OBJECTIVE: Presentation of a case of disseminated histoplasmosis, observed in a non-endemic area, in which cutaneous lesions and fever were the dominant clinical signs of the infection. CASE: A 54-year-old homosexual man with acquired immunodeficiency syndrome (AIDS) related Kaposi's sarcoma presented with cutaneous lesions and fever due to disseminated histoplasmosis. The patient was successfully treated with itraconazole 200 mg/day. He died after 8 months from AIDS dementia complex: disseminated histoplasmosis relapse was not observed. CONCLUSION: The case shows that infection with Histoplasma capsulatum must be considered by dermatologists in HIV/AIDS patients, even in non-endemic areas.

AIDS-Related Opportunistic Infections↗

[Evaluation of the anti-comedo effect of azelaic acid using the technique of horny layer biopsy and scanning electron microscopy].

Hyperkeratosis of the follicular channel is the most common finding in acne skin. The hyperkeratosis may represent an altercated keratinization process or the consequence of the abnormal sebum production and excretion. Several review demonstrated the anti-comedo activity of azelaic acid in acneic skin. This action may be due either to an anti-hormonal effect or to a change in keratin production. The aim of this work is to investigate the anti-comedo activity of 20% azelaic acid cream topically applied in a group of teen-agers affected by acne. A horny layer biopsy with cyanoacrylate glue was performed before and after four months of treatment with azelaic acid cream in ten acne patients. About 1 cm square horny layer biopsy was metallized and observed at the scanning electron microscopy to count the number of comedos. With this method we detected a reduction of about 26% of the comedos after four months of azelaic acid treatment. The result we obtained is in accordance with previous works about the anti-comedo activity of azelaic acid.

[Trisomy 18: phenotypic and biochemical variability].

We report a case of Edward's syndrome showing some symptoms infrequently described in trisomy 18. The authors suggest that the phenotypic expression of symptoms rarely observed in the syndrome may be better interpreted as non specific consequence of the chromosomal imbalance, rather than directly related to genes on chromosome 18. A gene dosage effect for the enzyme Peptidase A, whose gene is mapped on chromosome 18, was also observed.

Aspartic Acid Endopeptidases↗

[The gene-dose effect for peptidase A on the erythrocytes of a patient with Edwards' syndrome].

The study of gene/dosage effect may be essential in tracing the pathogenetic steps which lead from an unbalanced chromosome anomaly to a pathological phenotype. We present a newborn with a clinical and pathological picture compatible with a diagnosis of Edwards' syndrome. Chromosome analysis on lymphocytes and fibroblasts confirmed the diagnosis showing the presence of trisomy 18. On chromosome 18 the enzyme peptidase A is mapped, and we demonstrated the gene/dosage effect for Pep A in the erythrocytes of our patient.

[Gene-dosage effect of esterase D enzyme on erythrocytes in a patient with Patau's syndrome].

The study of gene/dosage effect may be essential in tracing the pathogenetic steps which lead from an unbalanced chromosome anomaly to a pathological phenotype. We present a newborn with a clinical and pathological picture compatible with a diagnosis of Patau Syndrome. A chromosome analysis confirmed the diagnosis showing the presence of trisomy 13. On chromosome 13 the enzyme Esterase D (ESD) is mapped, and we demonstrated the gene/dosage effect for ESD in the erythrocytes of our patient.
