[Endoscopic sclerotherapy of esophageal varicose veins in portal hypertension].

The results of endoscopic sclerotherapy (EST) in the patients with portal hypertension (PH) are analyzed. Efficacy of methods in the treatment of gastroesophageal varicosity in urgent situations (bleeding) and "cold" period was evaluated. Schemes of staged minimally-invasive surgical correction of PH syndrome are described. Etoxisclerol is regarded as the most appropriate sclerosant. It is concluded that EST is an effective method of treatment in PH, it is alternative to "open" abdominal surgery and may improve results of treatment in this group of patients.

[Methodological and technological aspects of relaparoscopy].

Overall 3457 laparoscopic procedures after open and laparoscopic abdominal operations have been performed including 270 relaparoscopies due to intraabdominal postoperative complications. Basing on their own experience, the authors give methodological and technological recommendations allowing to increase diagnostic and surgical efficacy of relaparoscopy and to reduce the rate of complications.

[Evolution of approaches to surgical treatment of complicated cholecystitis].

More than 17 thousand surgeries for calculous cholecystitis and its complications including 560 repeated surgeries on the major bile ducts were performed. Based on this experience three stages in treatment of complicated cholecystitis are distinguished. Principal changes of surgical policy are the criteria of this division. From 1993 a tendency towards mini-invasive surgery in complicated cholecystitis was developing. Optimal terms of surgery, adequate scope improved technique improve significantly results of surgical treatment of complicated cholelithiasis.

[Role of reflux in evolution and prognosis of acute thrombophlebitis of lower extremities varicose veins].

Role of venous reflux in evolution and prediction of acute varicotrombophlebitis (AVTP) has been evaluated with coloured duplex scanning performed as daily monitoring. It is demonstrated that venous reflux determines the direction and rate of thromb formation, it potential of embolism, promotes formation of floating thrombi. Study of venous reflux permits to predict the evolution of AVTP. It is one of the key elements which influences the thrombus formation and determines policy of treatment of varicose disease complicated with AVTP.

[Acute thrombophlebitis in the territory of saphena magna vein].

Problems of strategy and policy of surgical treatment of acute thrombophlebitis in the territory of saphena magna vein are discussed. Results of urgent radical combined venectomy in 76 patients are presented. A gas technique of subfascial dissection of perforant veins (SEPS) was used in 52 patients for the first time both for diagnosis and adequate treatment of thrombosed perforants. Spread of thrombosis from superficial veins to perforants was revealed in 14.3% patients with C3-C5 class of venous dysfunction (CEAP classification). Isolated and local forms of crus varicothrombophlebitis that must be urgently operated are distinguished. The mean hospital stay was 8.7 bed-days.

[Minimally invasive technologies in diagnosis and treatment of the algesic form of adhesive disease].

The indications to dissection of commissures in the algesic form of adhesive disease are determined. An experience with performing 36 laparoscopic operations is described. In 34 cases adhesiolysis was fulfilled under conditions of the I-III degree of the commissural process. Technical problems appeared while dissecting commissures in 2 patients with the IV degree of the commissural process due to pronounced dystopia of organs of the abdominal cavity. No complications were registered during operations. Control examination of 29 patients did not reveal recurrent diseases.

[Laparoscopic surgery in adhesive disease].

Experience in 2277 laparoscopic surgeries performed in patients with history of conventional and minimally invasive surgeries is analyzed. Regularities of commissures formation after some surgeries are determined. Viscero-parietal commissures (VPC) which are the main cause of possible intraoperative complications were revealed in 1746 (76,7%) patients. Diagnostic value of special ultrasonic examination was 95,5%, on the average. Laparoscopic adhesiolysis in different types of adhesive disease was attempted in 195 patients, 170 (87,2%) procedures were finished successfully. Application of Interceed (TC-7) on parietal peritoneum was used in 14 patients to prevent repeated formation of commissures.

[Papillovirsungocystoduodenoplasty in the treatment of cysts of the pancreatic head].

Experience in the treatment of cystic lesions of the pancreatic head is analyzed. The method of internal drainage of the pancreatic head cyst with transduodenal approach is described. This methods includes three stages: papillosphincteroplasty, deep virsungotomy with virsungoplasty and cystovirsungoplasty through ducts. The method is named papillovirsungocystoduodenoplasty (PVCDP). Results of 6 successful PVCDP are presented. There were neither recurrence of the cyst nor lethal outcomes.

[Differential surgical treatment of liver cirrhosis].

The work analyzes the data of treatment of 144 patients with the cirrhotic process in the liver. The patients were subjected to 196 different operations. Surgical interventions were fulfilled depending on the stage of cirrhosis, chronic hepatitis--precirrhosis included. To diagnoze and determine the stage of the cirrhotic process the authors used a complex of investigations including the summary assessment of the liver function by the formula developed in the clinic, radioisotopic diagnosis, laparoscopy with target puncture biopsy, splenoportography, lymphography. The cirrhosis stage and the degree of hepatic insufficiency determined the choice of periarterial hepatic neurectomy. Marginal resection of the liver was performed in chronic hepatitis and the initial stage of cirrhosis.
