The Departments of Medicine and Geriatrics were integrated in the Ogwr Health Unit in 1984. Since then there has been a non-age-related acute admissions policy which has led to early, intensive investigations and appropriate treatment of all patients according to need rather than age. A retrospective analysis of the use of thrombolytic agents over a 12-month period in patients aged 65 or over was undertaken to assess whether our principles extended to care of elderly patients admitted with acute myocardial infarction. High rates of prescription of thrombolytic agents in the elderly were achieved by endorsing the philosophy that age contributes little to the prediction of patient outcome and should not be considered in patient management.

Fifty-seven patients with small cell lung cancer were treated with cisplatin and etoposide alternating with vincristine, doxorubicin and cyclophosphamide. Seven patients were withdrawn because of drug toxicity. Of 50 evaluable patients 21 had limited and 29 extensive disease of whom 19 and 21 respectively achieved an objective response. Seven patients survived for longer than 2 years of whom 5 remained alive and tumour free 121-167 weeks from the start of treatment. Patients with limited disease and a complete response had a median survival of 72 weeks and the median survival of the whole group was 43 weeks. Seven out of 22 (32%) of patients who obtained a complete response relapsed and died with solitary evidence of cerebral metastases. This indicates the need for reassessment of the role of prophylactic cranial irradiation in patients achieving a complete response with effective systemic chemotherapy.
Of a total of 35 instances of painless myocardial infarction encountered in a District General Hospital over a 15-year period, ten occurred in seven patients admitted with acute exacerbations of asthma. There was a marked preponderance of anterior infarcts. No instance was found during a prospective study of 49 acute asthmatic admissions of 36 patients, during one of these 15 years. Although uncommon, the association of painless myocardial infarction and asthma has important implications for treatment.
