[Milk fluoridization efficacy in a controlled study and dental caries experience dynamics in conditions of wide availability of local F-containing means].

Cross-over, controlled, randomized and triple-blind study of F-milk efficacy and monitoring of F excretion with urine under conditions of wide availability of fluoride containing toothpastes was performed in Voronezh during last 10 years. F-milk project increasingly covered annually from 10 to 15 thousand of pre-school children. From them 335 children (11 kindergarteners -- age 6 years) regularly consuming F-milk during 3 years and 175 children (6 kindergarteners -- age 6 years) of the control group were examined. Statistics of the received data with 2 tail t-test has shown that between dmf (1.59+/-1.82) in children consuming F-milk and dmf (2.58+/-2.67) in children of the control group there is significant difference (p<0.001) with 96% of the statistical power. When implementing F-milk for 10 years running there were collected and analyzed data on caries experience in children 3, 6, 9 and 12 years (2004 vs. 1994). There is considerable reduction of caries incidence in all age groups in 2004, probably due to F containing toothpastes wide use and very active hygienic education of the population in Voronezh. At the same time dmf index in 6 year old children receiving F-milk was by 39% lower than in their contemporaries from the control group. F-intake monitoring in children 3, 4, 5 and 6 years of age has shown that F-milk (daily appr. 0.2 l with the level 2.5 ppm) use is a very effective method of F-deficiency elimination, leaving out any possibility of F-overdosage exceeding its physiologic needs.

[Monitoring of fluoride levels in children receiving fluorinated milk for a long time and time course of their permanent teeth].

Measurements of fluorine concentrations in 1320 urine samples collected from 440 children in the city of Maikop showed low levels of fluorine (0.43-0.72 mg) in this group in comparison with the groups receiving prophylactic treatment with optimal safe doses of fluorine with milk (0.88-1.19 mg). Consumption of physiological amounts of fluorine does not lead to its accumulation in the body even after long (up to 5 years) consumption of fluorinated milk. Permanent ratio between the values in the control and prophylactic groups allows less frequent monitoring of fluorine levels: during the first month of prevention and then after 2 and 4 years of prophylaxis with fluorinated milk. Clinical evaluation of anticaries efficiency of fluorinated milk in 920 children aged 6-8 years (309 in the control and 611 in prophylactic groups) showed a significant decrease of caries prevalence (by 12.5-24.5%) and intensity in permanent teeth (by 50.6-56.7% by the CDL index and by 47.6-52.9% by the CDLp index) after 3-5 years of regular consumption of fluorinated milk.

[Alternative methods for prevention and treatment of dental caries using laser and magnetic laser exposure].

Alternative methods for prevention and treatment of dental caries are presented, based on the use of laser and magnetic laser exposure (patent No. 2053818, in Russia) with a new generation laser device with Optodan microprocessor monitoring (patent No. 2014107, Russia). The methods are intended for wide clinical application in children and adolescents in pedodontic departments and outpatient centers and particularly for group and individual use in dental rooms at school.

[The monitoring of the level of body fluoride intake in children participating in 3 milk fluoridation projects in Russia].

In Russia three milk fluoridation projects have been implemented since 1994 (cities Smolensk, Voronezh and Maykop) covering more than 10,000 children living in low fluoride areas and having high risk of caries development both in temporary and permanent dentitions. Both preliminary and monitoring studies of fluoride excretion with urine and fluoride consumption were done. Total number of subjects--663 children 3-6-years of age attending kindergartens in these three cities; 2602 urine samples were collected, twice analyzed for fluoride content and processed in accordance with the method approved by the Russian Ministry of Health. 24-month monitoring of milk fluoridation projects in Russia did really show and confirm three main facts: the concentration of fluoride 2.5 mg/l in F-milk products for children 2-6 year-olds in low fluoride areas of Russia is really optimal concentration when daily milk consumption is about 200 ml; there is no such phenomena as detectable accumulation of metabolic fluoride under physiological conditions even after 2 years of regular fluoridated milk consumption; developed technique of fluoride consumption determination proved to be a very useful tool for planning and monitoring preventive programmes based on systemic use of fluoride.

[Demineralization behavior, enamel permeability and the morphological characteristics in relationship to the stage of eruption of the permanent teeth].

The posteruptive maturation of enamel is an important factor in caries disposition. In an experimental study the demineralization of enamel as well as its permeability and its morphological features in dependence of eruption stage were examined on extracted premolars. Chemical analysis showed variations in the Ca/P molar ratio. It is possible, that the instability of Ca-ions in crystalline configuration decreased during the time of eruption. The permeability of enamel decreased between 5 and 11.8%. It appears to be useful to concentrate preventive measure on this phase of dentition development.

[Effects of inorganic and organic fluorides combined with molybdenum and chlorohexidine upon processes of metabolism, fluoride content, and cariosity of the teeth of Wistar rats (author's transl)].

A study has been made to determine the inhibitory action upon the progress of dental caries of two trace elements (fluorine and molybdenum) combined with chlorohexidine with the goal of increasing the efficiency of fluoride treatment. However, the results obtained show that the chosen combination of trace elements did not lead to a substantial increase in cariostatic action. Combined use of fluorides and chlorohexidine was seen to exert an inhibitory action on the progress of dental caries.

[Experimental animal studies of the caries-protective activity of the anticarious agent Fluorprotektor (author's transl)].

In two caries model experiments on Wistar rats, a significant reduction in caries was observed following local application of Fluorprotektor and Duraphat. There appears to be no difference between the caries-protective properties of these two agents. The accumulation of fluoride in the superficial enamel was tenfold to elevenfold for the two lacquers. Residual Fluorprotektor was found on teeth even after twenty-four days from the application thereto.

[Experimental assessment of the effectiveness of general and local caries preventives by the use of complex factor analyses (author's transl)].

Using the method of complex factor analysis, it was possible to characterize the results of experiments conducted with a view to determining the anticariogenic effects upon 104 Wistar rats of fluoridated water, fluoride-containing gel, and remineralizing solutions. The anticariogenic effects were highest for fluoridated drinking water, less satisfactory for fluoride-containing gels, and minimal for remineralizing solutions. Simultaneous application of these anticarious agents was seen to exert an increased inhibitory action upon the progress of dental caries. A typical example is used to show that the use of complex factor analysis yields additional results showing the interaction of the factors being studied and substantially reduces the number of test animals and the time required for experimentation.

[Fluoride contents of rat molars following the administration of amine-fluoride-containing caries preventives (author's transl)].

Experiments on fifty Wistar rats were conducted with the object of studying the incorporation of fluoride into the enamel of molars subsequent to the administration of amine-fluoride-containing caries preventives. - Use of two toothpastes with low fluoride contents of 0.125% and 0.18% F, respectively, and two higher-fluoride (1% F) solutions resulted in a marked increase in the concentration of fluoride in the superficial layers of dental enamel.
