Genetic determinants of efficiency of magnetic laser therapy of essential hypertension.

The efficiency of magnetic laser therapy was evaluated in patients with essential hypertension (stages I and II). The role of angiotensinogen, angiotensin-converting enzyme, type II bradykinin receptor, and endothelial nitrogen oxide synthetase gene polymorphism in the realization of hypotensive effect of magnetic laser therapy was evaluated. The hypotensive effect of magnetic laser therapy depends on the polymorphism of the studied genes and is maximum in patients with MM polymorphism of angiotensinogen gene and DD polymorphism of angiotensin-converting enzyme gene. Additive interaction between angiotensin-converting enzyme and angiotensinogen genes in the formation of hemodynamic effects of magnetic laser therapy was detected.

[Impact of angiotensin-converting enzyme, angiotensinogen, endothelial NO synthase, and bradykinin receptor B2 gene polymorphisms on myocardium in patients with hypertension and in athletes].

We investigated the role of gene polymorphisms in angiotensin-converting enzyme (ACE), angiotensinogen, endothelial NO (eNO) synthase, and bradykinin receptor B2 in determining the cardiovascular system structure and function in hypertension and "athletic heart" syndrome. Using a PCR-based method, 114 hypertensive patients and 94 athletes were genotyped for I/D polymorphism of ACE, M235T angiotensinogen (ANG), Glu298 Asp endothelial synthase (eNOS), and type 2 receptor for bradykinin (BDKR2). Echocardiography and a 24 hour blood pressure monitoring being performed. The (+)-allel of BDKR2 gene was associated with the left ventricular hypertrophy and greater wall thickness in athletes and hypertensive subjects. The hypertensive patients, that were homozygous for Glu298 allele of eNOS, demonstrated a lower level of diastolic blood pressure than did those with Glu298 Asp and Asp298 Asp genotypes. At the same time, the ACE and AND gene polymorphisms displayed no association with the cardiac structure and function.

[A prospective study of hypolipidemic therapy in young women after myocardial infarction].

A 3-year prospective trial has been performed of a course of ischemic heart disease (IHD) in postmyocardial infarction women (PIW) on antianginal treatment alone vs women on hypolipidemic drugs. IHD ran a less severe course in young PIW than in menopausal IHD. Aggravation of IHD occurred more frequently in women with previous MI and in hypertension. Comparison of the two groups of women demonstrated that hypolipidemic drugs improved a course of IHD in the women. In menopausal women taking hypolipidemic drugs IHD improvement occurred 6.2 times more frequently while aggravation 2.3 times less frequently than in women on standard antianginal treatment. Young women benefited from hypolipidemic therapy 1.6 times more frequently.

[Left ventricular diastolic function in patients with bronchial asthma].

The aim of the study was to evaluate the diastolic function of the left ventricle (LV) in patients with bronchial asthma (BA) in relation to the phase and severity of the disease. Seventy four patients with BA, including 16 with mild persistent BA, 39 with moderate BA, and 19 with severe BA were examined in different phases of the disease. Echocardiography and Doppler study of intracardiac blood flow were performed and external respiratory function was studied. Thirty six healthy persons matched by age, gender, body weight, and blood pressure values made up a control group. Most characteristics of LV diastolic relaxation were found to become much worse in BA, which is due to bronchial obstruction, hypoxemia, dilatation, hypertrophy, and altered diastolic filling of the right ventricle (RV). LV diastolic filling improved when an exacerbation of BA attenuated. Worsening BA severity was ascertained to lead to decreased early LV filling, which is associated with enhanced bronchial obstruction, progressive RV hypertrophy and dilation, concentrated LV remodeling. The most likely mechanism of impaired transmitral blood flow in progressive BA is the impaired LV and RV interaction and pulmonary hemodynamic rearrangement. The revealed LV diastolic dysfunction in patients with BA of various severity and in different phases of the disease were not found to depend on heart rate changes.

[Risk factors of ischemic heart disease, data of exercise tests, 24-hour ECG monitoring and coronary angiography in young women with history of myocardial infarction].

Most frequent risk factors in a group of 40 premenopausal women with history of myocardial infarction were smoking (70%) and hypertension (62.5%). Reaction to bicycle exercise tests was negative in 42% and ischemic (mainly pain combined with ST-segment changes) in 50% of these women. Ischemic ECG changes were found during both exercise test and 24-hour ECG monitoring in 30% and were not found at all in 40% of women. Angiography revealed coronary artery stenoses in 58% of women.

[Change in central hemodynamics in lymphoma patients].

AIM: To elucidate features of hemodynamics in patients with lymphoma and hemodynamic effects of polychemotherapy. MATERIAL AND METHODS: Parameters of central hemodynamics were studied echocardiographycally in 42 patients with lymphoma. RESULTS: In a definite rise in the levels of tumor progression markers, changes in the immunity, impaired hemopoiesis there was rearrangement of hemodynamics (development of tachycardia, trend to excentric remodeling of the left ventricle). The above changes in hemodynamics enhanced in conduction of polychemotherapy. CONCLUSION: To avoid cardiotoxicity of polychemotherapy, lymphoma patients should receive cardioprotectors.

[Efficacy of climate therapy in patients with chronic cardiac failure at a climate-balneological resort].

The aim of our study was to estimate clinical effects of spa therapy (ST) at Sochi-Macesta resort in patients with heart failure (HF). Spa therapy was given to 116 patients with HF functional class (FC) 0-II by NYHA classification. It is shown that ST leads to improvement of the patients' clinical status, reduction of HF FC (from 1.32 +/- 0.09 to 0.66 +/- 0.01, p < 0.001), to increased exercise tolerance (p < 0.05) and better 6-minute walk test results (from 460.3 +/- 11.4 m to 511.2 +/- 11.8 m, p < 0.01), higher myocardial coronary reserve, favourable shifts in blood pressure. ST also has a cardiotonic effect and corrects vegetative status, there was also antiatherogenic effect and an increase in blood coagulation potential. The received data allow us to conclude that ST can be an effective tool of medical rehabilitation in patients with initial stages of heart failure.

[Metabolic determinants of efficacy of infrared laser therapy in hypertensive patients with combined pathology].

By a distinct pathogenetic direction of a therapeutic action of laser therapy (LT) on different regulators of blood pressure (BP) and metabolism, 109 patients with essential hypertension (EH) and atherosclerosis and/or diabetes mellitus were studied for LT efficacy depending on metabolis disorders. LT demonstrated metabolic neutrality and unefficacy in patients with multiple marked disorders of fat metabolism and hyperglycemia. Metabolic factors determining LT efficacy comprise hypercholesterinemia, hypertriglyceridemia and hyperglycemia. The factorial analysis points to essential factor restructuring in metabolic disorders. The obtained equation of multiple regression allows prognostication of the degree of a fall of mean BP in response to LT depending on the degree of metabolic disorders.
