[Contractile function of ureter in its anomalies in children].

A study on the testing unit was made of a contractile function of ureteral fragments removed at reconstructive operations for vesicoureteral reflux (VUR), neuromuscular dysplasia (NMD) and pelvoureteral segment stricture (PUSS). The study parameters (wall rigidity, amplitude of the contractions, potassium depolarization contracture, responses to adrenoreactive drugs) in patients with VUR were close to normal, whereas in those with NMD or PUSS changed significantly, especially in PUSS patients. Ureteral contractility impairment was heterogenic in all the groups, varying from intact to lost and did not correlate either with age or the disease stage. The value of functional studies of the ureter in reconstructive surgery for specifying the disease pathogenesis, mechanisms of functional decline and individual prognosis of functional efficiency of the operative intervention is discussed.

[Urodynamic disorders of upper urinary tracts in neonates and infants: diagnosis and therapeutic policy].

Infants with obstructive and functional disorders of urodynamics (52 and 46 neonates, respectively) were examined for differentiation of the two disorders. An original scheme of such examination is proposed. Grounds are given for deciding on one- or two-staged procedure, age-specific technique of transcutaneous puncture nephrostoma is outlined. As shown by 23 subcutaneous ureterostomas, the effect of such treatment is poor.

[Effects of focused shock waves on children's kidney: experimental study].

The effects of focused shock waves produced by URAT-P2 unit on renal tissue was studied in 120 immature rats with 100-120 g b.w. Morphological material obtained after the procedure and 3 months after it was studied with reference to the following parameters of the impact: interelectrode spacing, capacitor capacitance, capacitor charging voltage, pressure in the therapeutic focus, number of impulses. It was found that one of the essential damaging factors of the shock waves is spontaneous widening of the interelectrode spacing in the course of the fragmentation which entails an increase in the pressure amplitude in the center of the therapeutic focus with resultant prolongation of the impulse.

[Results of upper urinary tract malformations correction in adults operated in childhood].

3-13-year follow-up results are presented for 83 adult patients operated in childhood for ureterohydronephrosis, hydronephrosis, vesicoureteral reflux. Positive results were obtained in adults operated in early childhood when renal and urinary compensatory mechanisms were stronger. In hydronephrosis and vesicoureteral reflux the results were better than in ureterohydronephrosis. Combined clinical, morphological and experimental investigations have revealed a direct relationship between renal, ureteral functional reserves in adult patients and age of the correcting operation in childhood, degree of intactness of the kidneys and urodynamics, regenerative ability of leiomyocytes.

[Treatment of solitary renal cysts in children].

The authors formulate therapeutic policy basing on clinical, ultrasound and parenchymal morphological studies in 34 children with solitary renal cysts. Operative intervention is indicated in renal function decline manifesting with enlargement of the cyst and reduction of the cystic-renal coefficient. The kind of the operation is decided by location of the cyst and its characteristics obtained at radiodiagnosis. In slow growth of the cyst and no further decline in its function the patients should be followed up as a risk group.

[Percutaneous paracentric nephrostomy in infants].

The paper describes the technique of transcutaneous paracentetic nephrostomy in infants, emphasizes advantages of this method in infants versus the others used to correct supravesical obstruction (open nephrostomy, ureterocutaneostomy, kidney catheterization, internal stenting), describes relevant indications. The nephrostomy was performed in 16 infants. The results show effectiveness and safety of this method.

[Urodynamics of the upper urinary tracts after ureteroplasty (experimental data)].

An experimental study on 12 mongrel dogs was made for assessment of urodynamics of the upper urinary tracts after reconstructive surgery. Ureterohydronephrosis was modelled by cryodestruction, confirmed by x-ray and radionuclide examinations. Urodynamics of the upper urinary tracts was studied by measurements of the intraluminal pressure and at multichannel impedance ureterography under thiopental anesthesia. It was found that reestablishment of normal urine passage after ureteroplasty is not accompanied by urodynamic recovery. Preoperative hyperfunction of the upper urinary tracts and their postoperative hypofunction appeared unfavourable prognostic signs for ureteroplasty outcomes.

[Pathogenetic bases for choosing treatment for vesicoureteral reflux in children].

600 children treated surgically or conservatively for vesico-ureteral reflux (VUR) at the age 3 months to 15 years were followed up for maximum 28 years. The course of compensatory processes in the kidney with considering the cause of VUR development at different stages of life (childhood, adolescence, adult life) and the degree of renal function loss were specified. Morphologic studies of renal parenchyma help choosing treatment of VUR in children depending on the cause of its development: malformation of ostium ureteris and intramural ureter, inflammation in the urinary bladder.

[Postoperative cystitis in children].

Operative intervention on the urinary bladder in children entails postoperative reactive cystitis. Biopsies of urinary bladder mucosa were studied morphologically. Basing on these morphological findings and clinical evidence, three leading variants of the mucosa changes were distinguished: focal squamous cell metaplasia; excess of the lymphoid tissue and lymphangiectasia; hemangiectasia. These morphological changes aggravate the course of postoperative cystitis. UHF-therapy and magnetic-laser radiation beginning postoperative day 4 and 7, respectively, reduce inflammation developing in postoperative cystitis. This indicates effectiveness of the above combined therapy which can be also used for prevention of acute inflammation episodes in postoperative period.
