[Morphological changes in the kidneys in refluxogenic nephropathy in patients with congenital and acquired vesicoureteral reflux].

Current views of foreign investigators on refluxogenic nephropathy rest on the results of pig experiments. The authors studied human material: kidneys of children who died of pyelonephritis caused by vesicoureteral reflux. Electron-microscopic and other morphological examinations of the kidney pelvic wall provided evidence for a distinct difference in relevant morphological structures observed in congenital versus acquired reflux. Congenital vesicoureteral reflux displays a specific morphological picture suggesting congenital refluxogenic nephropathy, whereas acquired reflux produces a typical picture of ascending pyelonephritis.

[Varicocele in children and adolescents and infertility].

28 adolescents aged 16-18 were examined 2-4 years after correction of varicocele. Some ejaculate characteristics were found subnormal as well as the function of the prostate gland as indicated by reduced concentrations of citric acid and zinc ions. To make definite conclusions on possible effect of varicocele in adolescents on their future fertility further studies are necessary.

[The complications of extracorporeal lithotripsy in children].

Complications of extracorporeal lithotripsy (ECL) observed at long- and short-term follow-up have been analyzed basing on 605 case histories of children aged 4-14. 59 of them had coral nephrolithiasis. Special attention was paid to macrohematuria, obstructive pyelonephritis, progressive chronic pyelonephritis.

[Neuromuscular dysplasia of the ureters in children].

The paper is concerned with disputable aspects of the policy in the diagnosis of neuromuscular ureteral dysplasia (NUD) in children, with indications to NUD conservative treatment and a choice of optimal surgical correction at primary or repeat operations basing on ureteral dilatation. The necessity of the NUD conservative treatment in children with nonaffected renal function has been confirmed by 52 cases of NUD children treated conservatively or surgically. The analysis of surgical outcomes in 34 NUD children and urodynamic pre- and postoperative examinations in 18 of them suggest the following conclusions: 1) the decision on "modelling" of the ureter is feasible only in the course of surgical intervention; 2) in ureteral dilatation less than 5-7 mm "modelling" is unnecessary. In other cases "modelling" is made at primary surgery according to Kalicinski's technique, at resurgery according to Hander. Positive outcomes in primary interventions were registered in 78% of the patients, 62% of the operations were successful in recurrent disease.

[A surgical method for replacing the lower portion of the ureter in children with fixation of the mobilized bladder to the musculus lumborum].

The author used the technique of the bladder mobilization and fixation to musculus lumborum in 18 children with nonfunctional terminal part of the ureter. Before and after surgery comparisons favoured the surgery as it improved renal function, abolished the terminal part uretero-sclerosis and vesicoureteral reflux. Relevant indications, contraindications and the operative procedure are outlined.
