Results of treatment of diverticulosis of the small intestine in 59 patients are presented. Operations for urgent indications were performed on 12 patients. In 12 patients diverticulosis was associated with other surgical diseases of organs of the abdominal cavity. Planned operations were performed on 35 patients, 3 of them having total diverticulosis. The authors describe their tactics and original methods of treatment of total diverticulosis.
Operations were performed on 30 patients with scarred constrictions of the distal portion of the choledochus and dilatation of the common bile duct with the clinical picture of mechanical jaundice and cholangitis. In none of the patients concrements were detected in the gallbladder and duct. The authors do not consider the peripheral diverticula to be the decisive factor in pathogenesis of cholelithiasis in this category of patients.
The importance of duodenoscopy in the diagnosis of duodenal diverticula is illustrated by rich clinical material. It proved successful in 75.5% of patients. Retrograde choledochopancreatography is valuable in studying the relationship between the diverticulum of the descending part of the duodenum and the pancreatic and hepatic ducts.
The authors discuss the problem of diagnostics, treatment and remote results in patients with carcinoma of the gastric stump. Operations were performed on 387 of 552 patients having malignant tumours of the stomach. Carcinoma of the gastric stump was found in 21 patients. Radical operations were fulfilled in 12 patients (with one lethal outcome). Five-year survival after radical operations was noted in 2 patients.

The authors examined 467 patients at remote terms after different variants of gastric resection for ulcer disease. It was found that most complete information concerning the state of the gastric stump mucosa can be obtained by the endoscopic examination. A comparative assessment of changes of the mucosa has shown advantages of the Billroth-I gastric resection.

The authors have elaborated a method of the formation of anastomoses on hollow organs of the gastro-intestinal tract which consists in placing precision interrupted extramucosal sutures. It allows to adapt all the layers of the organ sutured. So, the tender mucosa is intact, muscular and serous layers are healing by first intention. The method was developed in experiment and tested in 192 patients with good results.

The authors describe results of using their original method of the operative exclusion of the colon altered by diverticulosis in 27 patients. Good immediate and remote results were noted.
The authors analyze their experience with 140 operations performed for sliding hernias of the esophageal opening of the diaphragm. In 101 patients the lateral esophagofundoplication was performed by the method of B. V. Petrovski and N. N. Kanshin, in 39 patients the operation being the lateral esophagofundoplication by the authors' modification. There were no cases of postoperative lethality. Postoperative dysphagia appeared in 6 patients. Long-term results were studied in 93 patients (good results in 50 patients, satisfactory--in 26 patients, unsatisfactory--in 17 patients). Recurrent hernias were the cause of unsatisfactory results in 7 patients, incompetence of the cardia--in 10 patients.
Results of treatment of 865 patients with adhesive disease are presented, 217 of them were subjected to life-saving operations for acute commissural ileus. Planned operations were made on 336 patients. Due to bad long-term results of the Noble and Chidls-Fillips operations the author prefers simple dissection of commissures. Adhesive disease appears in 2% of cases after operations irrespective of the prophylactic measures. The adhesive process is a natural feature of the organism and it should not be disturbed, since the adhesive disease makes its appearance not due to the predisposition but because the commissures disturb passage along the intestine.
The work generalized an experience with the treatment of 747 patients with diverticula of the duodenum. Operations were performed on 326 of them (43.6%). In 172 patients the diverticula were associated with other diseases of organs of the abdominal cavity. Such patients were subjected to associated operations. The dependence of the clinical picture of inflamed diverticula on their localization is described. The author determines the surgeons' strategy, indications and volume of the surgery in different localizations of the diverticula. A new operation of exclusion of the duodenum allowing the postoperative lethality to be decreased from 5.5% to 1% was developed and introduced into practice.

The work presents results of treatment of 123 patients with a severe dumping-syndrome following resection of the stomach for ulcer disease. The author considers that there is not any predisposition for the dumping-syndrome in resection of the stomach. This pathology is a result of inconsiderable errors in operation and can be prevented by strict observation of the rules of performing the operation. The reconstructive operation aimed at the creation of favorable conditions for the functioning of the gastric stump is indicated if the conservative treatment failed during 6-10 months.
