A microprocessor card software server to support the Quebec health microprocessor card project.

The Quebec Health Smart Card Project is advocating the use of a memory card software server[1] (SCAM) to implement a portable medical record (PMR) on a smart card. The PMR is viewed as an object that can be manipulated by SCAM's services. In fact, we can talk about a pseudo-object-oriented approach. This software architecture provides a flexible and evolutive way to manage and optimize the PMR. SCAM is a generic software server; it can manage smart cards as well as optical (laser) cards or other types of memory cards. But, in the specific case of the Quebec Health Card Project, SCAM is used to provide services between physicians' or pharmacists' software and IBM smart card technology. We propose to expose the concepts and techniques used to provide a generic environment to deal with smart cards (and more generally with memory cards), to obtain a dynamic an evolutive PMR, to raise the system global security level and the data integrity, to optimize significantly the management of the PMR, and to provide statistic information about the use of the PMR.

Bile-duct cyst with cystlithiasis: a case report.

Cystic dilatation of the biliary tree is an uncommon congenital condition, most often discovered in infancy. It should be suspected in an adult who has a history, over a long period, of intermittent pain in the right upper quadrant, episodes of jaundice or cholangitis, or both, and has a palpable mass in the right hypochondrium. Ultrasonography and especially endoscopic retrograde cholangiopancreatography allow the biliary cyst to be diagnosed preoperatively more often than previously. The authors describe the case of a 60-year-old woman with congenital cystic dilatation of extra- and intrahepatic bile ducts with cystlithiasis that followed an unusual course. Of the various theories of causation, Babbitt 's is now the most widely accepted, especially since the advent of endoscopic retrograde cholangiopancreatography. The presence of stones is an uncommon and late complication. The treatment is surgical, excision of the biliary cysts being the treatment of choice; however, Roux-en-Y choledochocystojejunostomy is performed when excision is technically impossible.

Intraoperative fiberoptic colonoscopy--"the difficult polypectomy".

An alternative to transcolonic polypectomy, transabdominally directed, transanal colonoscopic polypectomy, for patients who require polyp removal but in whose cases standard colonoscopic approach has failed is presented. The early results are excellent, and the method also can be applied to other patients who have colonic polyps who undergo procedures such as hysterectomy and cholecystectomy.

Optical memory card applicability for implementing a portable medical record.

The implementation of portable record based on laser card technology is discussed in terms of data structures for quick access and software tools to reinforce security and confidentiality as required by medical data. Experimental results from a field test with a laser card technology are reviewed with regard to implementation in a large-scale and public information system. An efficient data structure is described for storage purposes, and some proposals are put forward to secure stored data.
