[The health situation of the non-institutionalized elderly urban population of Castro Uridiales and the differentiating characteristics of those attending the health center frequently].

OBJECTIVES: Main: to describe the health situation of the urban non-institutionalised population of 75 or over in Castro Urdiales. Secondary. To analyse the differences in the over-user group. DESIGN: Descriptive, crossover. SETTING: Primary Care Health Centre. PATIENTS: People in the urban area aged 75 or over and with the following inclusion criteria: not institutionalised, who had been at least 6 months in the town, and for whom there was data for locating them. Over-users: people in the upper third of attendance (9 or more consultations per year). MEASUREMENTS AND MAIN RESULTS: A "Comprehensive Geriatric Assessment" was performed, cognitive state (Pfeiffer) and social assessment. The over-user group functioned better, had a better cognitive state and consumed more medication. CONCLUSIONS: The results coincided with other studies in most of the items analysed. It is important to be aware of the worse functional and cognitive condition of the group which attended the Health Centre least. Sub-groups of the elderly still need to be studied.

[Relationship between family perception and alcohol consumption].

OBJECTIVES: To establish the prevalence of alcohol consumption and dependence, and its relationship to perception of family function. DESIGN: Descriptive, crossover study. SETTING: Urban area. Jumilla Health Centre (Murcia). PARTICIPANTS: 434 Health Service users were surveyed. INTERVENTIONS: Demographic data, determination of alcohol consumption, CAGE and APGAR questionnaires. MEASUREMENTS AND MAIN RESULTS: There was a high prevalence of alcohol consumption (77%) and excessive consumption (22.9%). Excessive alcohol consumption was mainly among men and active agrarian workers. The relationship between alcohol consumption and family dysfunction was not significant, but was significant between CAGE and family dysfunction. CONCLUSIONS: Despite the high prevalence of alcohol consumption, family dynamics are not usually changed significantly. This only happens in cases of alcohol dependency syndrome.

[Tobacco and cancer of the lung. A case-control study].

The relationship between cigarette smoking and risk of lung cancer has been analyzed in a case control study of 98 cancer patients and 108 age matched healthy controls. This relationship has been studied based on age when smoking started, number of cigarettes per day and minimum number of cigarettes smoked, and smoke inhalation. The excess rate (ER) of lung cancer amongst those who started smoking before age 20 was 3.47 (C.I. 95% 1.33 + 9.05) above those who started smoking later. There is a progressive increase in ER as the mean daily number of cigarettes and the minimum number of cigarettes smoked increase. The ER in those who inhale smoke was 2.4 (C.I. 95% 1.6 - 5.05) when compared to those who did not inhale smoke. It is essential to quantify the magnitude of the smoking habit as well as its duration since these variables are going to determine the risk of acquiring the disease. [Sleep changes produced by environmental noise].

This article reports the results of a survey trying to evaluate different aspects related with ambient noise and its effects on selected urban communities. A total of 263 residents in 5 different neighbourhoods of the city of Valencia were interviewed. For each one of the neighbourhoods, level of ambient noise were independently registered during 24 hours during some days of a week. 40% of the interviewees reported having some difficulty for getting to sleep and 59% of these with difficulties identified the traffic noise as the causation. Also, 49% reported to awake sometime during the night (24% of them pointed to the traffic noise as the major cause). All these problems were reported more frequently in the neighbourhoods with a greater level of ambient noise. The probably high number of people affected by sleep disturbances related with ambient noise during the night makes necessary to adopt adequate correcting actions.

[Cost-benefit analysis of fluoridating the public water supply of the city of Málaga].

The aim of the present study was to assess the cost-benefit of the program of fluoridation of the public water supply in the city of Málaga. Marginal benefits and the rate cost/benefit were the two parameters used to evaluate the cost-benefit. We have considered all the information available about similar experiences performed in other countries and about the particular aspects of the city in which this program was going to be set up. The cost of fluoridation per inhabitant per year and the effects of the cost of fluoridation on the price of the water supplied were also estimated. The program for the fluoridation of the public water supply in Málaga is profitable from the first year, as for each peseta spent we will benefit 2.10 pesetas. After 20 years of public water fluoridation the net benefit of this program will reach between 519 and 5,300 millions pesetas.

[Plasma levels of HGH, insulin and NEFA in diabetic children (author's transl)].

Plasmatic levels of HGH, insulin and NEFA are measured after oral glucose loading in 14 diabetic children and are compared with those obtained in 7 normal children. The average values of HGH are higher in the diabetics at 0', 30', 60', and 190', and on the contrary the release of HGH which is produced at 90' and 120' with a glucose load is increased in the normal controls. None of these differences are significant. The insulin and NEFA response observed in the normal cases is not observed in the diabetics where the obtained values do not show variation with the administration of glucose.

[The evaluation of the effects of ambient noise on the residents of the historic center of Valencia].

BACKGROUND: The acoustic contamination in residential areas is a generalized problem to solve it, it is necessary to have an available precise information about its magnitude and effects upon the population. METHODS: We show the first results of a study on the acoustic contamination in the historical center of Valencia and about its effects upon that district residents. A noise map has been made measuring the daily sound level in 135 points regularly distributed. At the same time, 418 telephone polls among the residents in that district has been carried out. RESULTS: The noise levels, existing in the studied zone, are very high, it has been obtained an average equivalent sound level (Leq) of 71.6 decibels A (dBA). About a 60% of the surveyed persons declare to be "troubled", because of the noise and one out of four surveyed persons find it difficult to get to sleep for the same reason. Road traffic is the main source of trouble. There has been observed a relation between certain personal characteristics in the surveyed persons and the answers obtained, although it does not reach an statistical significance. CONCLUSIONS: In the historical center of Valencia, there exists an important problem of environmental noise and the daily levels exceed clearly the recommended limits. It seems that traffic is the main source of noise probably combined with a deficient urban structure which does not allow a fluid vehicular traffic. The residents in this zone are aware of the problem and a great proportion of them declare to be damaged in a greater or smaller magnitude. Although the sensitivity to noise could be in relation with certain personal factors in the surveyed persons.

[Molecular epidemiology of Salmonella enterica poisoning: correlation of the serotype and protein profile and plasmid DNA analysis].

Food-borne diseases nosocomial outbreaks due to Salmonella enterica strains have a high incidence nowadays. We describe here an outbreak that occurred in July 1990 in the Malaga University Hospital, that only involves one shift of health care workers. Salmonella enterica was detected in stool samples from HCW and a first analysis revealed the presence of two different antibiotic susceptibility patterns (resistotypes) among isolated strains. Two months after the outbreak started, the CNVIS (Majadahonda, Spain) confirmed the presence of two different serotypes (bovismorbificans and enteritidis). The delayed availability of the results (due to the lack of specific sera needed in most of the clinical Laboratories) was responsible for finishing only a partial and restricted epidemiologic study in terms of source identification for one particular serotype as well as for the antimicrobial resistance studies. The molecular identification techniques, particularly the electrophoretic protein profile could overcome these problems.
