Triamcinolone acetonide mouth rinses for treatment of erosive oral lichen planus: efficacy and risk of fungal over-infection.

OBJECTIVES: To assess the efficacy of triamcinolone acetonide mouthrinse for treatment of erosive oral lichen planus (OLP), and to evaluate the risk of fungal over-infection. PATIENTS AND METHODS: Clinical records of all cases of erosive and erosive-ulcerative OLP treated in our Oral Medicine Service over the period 1993-2003 were reviewed. All patients had been treated with mouthrinses containing triamcinolone acetonide at 0.3% (T1) or 0.5% (T2). Clinical outcome for each patient was classified at 1, 3 and 6 months after start of treatment as total remission (TR), partial remission (PR) or no remission (NR). Fungal over-infection was also recorded. RESULTS: A total of 35 cases were included in the study. Clinical outcomes considering both treatment groups together (T1 + T2) were as follows: at month 1, 28.6% TR, 62.9% PR, 8.6% NR; at month 3, 57.1% TR, 37.1% PR, 5.7% NR; and at month 6, 80.0% TR, 17.1% PR, 2.9% NR. In four patients (11.4%) fungal over-infection was noted during the study period. CONCLUSION: Triamcinolone acetonide mouthrinse is an appropriate treatment for erosive OLP, in view of the high efficacy and low risk of fungal over-infection.

Unconverted chars obtained during biomass gasification on a pilot-scale gasifier as a source of activated carbon production.

Biomass gasification was used to produce activated carbon on a pilot-scale fluidised-bed gasifier. The feedstock included both biomass alone and biomass mixed with coal and coal/granulated plastic wastes. This paper reports the results obtained from four different runs undertaken under various conditions of fuel supply, different ratios of steam/air for the gasification and temperature. These conditions were selected because they led to a significant amount of unconverted chars produced during gasification (from 0.72 to 1.4 kg) which then served as raw material for the production of activated carbon whilst the amount of gas obtained was also high enough for its potential use for different end-use applications. From the analysis of the results obtained, it can be concluded that a reasonable porosity development (mainly in the area of narrow micropores) was obtained by gasifying unblended pine wastes with steam for 4 h, producing about 1.4 kg of good-quality activated carbon (micropore volume of 0.263 cm(3)/g). In other runs, chars with a reduced microporosity development (i.e. 0.180 cm(3)/g) were obtained, however, they could be used as a proper starting material for the chemically activated carbon production.

Mortality due to liver failure and impact on survival of hepatitis virus infections in HIV-infected patients receiving potent antiretroviral therapy.

The aim of the present study was to examine the causes of death, the mortality attributable to liver failure, and the impact of hepatitis virus infections on the survival of a cohort of HIV-infected patients before and after the extensive use of highly active antiretroviral therapy (HAART). Liver disease associated with hepatitis C virus (HCV) seems to be accelerated in patients infected with the human immunodeficiency virus (HIV). On the other hand, the effect of HCV on HIV progression was controversial before the introduction of HAART. However, the last study to report changes in mortality due to liver failure was published in 1997, and the impact of HCV carriage on the survival of HIV-infected patients receiving HAART needs to be clarified. In this investigation, 492 patients who were prescribed antiretroviral drugs between April 1989 and September 2000 were included in the study cohort. The median duration of follow-up of the cohort was 1,392 days. HCV infection was present in 323 (68%). Mortality attributable to AIDS decreased from 4.5 to 1.8 per 100 persons per year. Mortality due to liver failure increased from 0.3 to 0.5 per 100 persons per year ( P<0.01). The survival of patients with and without HCV infection was similar ( P=0.8). Although liver failure is an increasing cause of death among HIV-infected patients receiving HAART, HCV infection has still no impact on the survival of HIV-infected patients.

Synchronous oral leiomyosarcoma and squamous cell carcinoma.

An unusual case of synchronous squamous cell carcinoma and leiomyosarcoma of the oral cavity is reported in a patient without any identified environmental risk or predisposing factors. The invasive squamous cell carcinoma involved the tongue, whereas the leiomyosarcoma was located in the soft palate. No immunostaining was found for human papillomavirus or Epstein-Barr virus, and in situ hybridization showed negativity for human papillomavirus DNA within the tumor cells. Alterations of bcl -2, c-erb -b2 and Rb oncoproteins were not found immunohistochemically. Overexpression of p53 was detected by immunohistochemistry in both tumors, but p53 gene mutations were not found by polymerase chain reaction. Neither loss of heterozygosity of p53 nor microsatellite instability was detected in this patient. The smooth muscle nature of the leiomyosarcoma was confirmed by immunohistochemical methods. To our knowledge, synchronous smooth muscle and epithelial oral tumors have not previously been reported.

[Endoscopic sclerotherapy is useful in Dieulafoy's disease ].

The clinical histories of 14 patients with digestive hemorrhage due to Dieulafoy disease admitted to the authors' hospital over 74 months were retrospectively analyzed. These cases represent 1.18% of the non varicose upper digestive hemorrhages (CI 95%; 0.57-1.79%). Male predominance (6:1) and advanced age (median: 67.5 years) were observed. The ingestion of potentially gastroerosive drugs, smoking and alcoholism were reported in 57, 57 and 7% of the cases, respectively. Six patients (43%) required more than one endoscopy for diagnosis. The lesion responsible for hemorrhage was found at 6 cm or less from the cardias on 11 occasions (78%). Endoscopic sclerotherapy was performed in the vessel by polidocanol at 2% (1.5-10 ml, median: 6). In 10 cases sclerotherapy was preceded by the injection of adrenaline 1/10,000 (2-11 ml, median: 5). Definitive hemostasis was achieved in 11 patients (78%) (two of these cases required further sclerotherapy); 2 patients required surgery, and one patient (7%) died. Of the 13 surviving patients, none presented relapse after follow up of 2-63 months. We can conclude that Dieulafoy disease is an infrequent but severe cause of digestive hemorrhage, being predominantly found in males over the age of 50 years. Repeated endoscopy is often required to determine diagnosis and treatment. Endoscopic sclerotherapy with polidocanol is effective and safe in this disease although surgery may be required and death may occur.

Histological and histochemical characteristics of the lymphocystis disease in gilthead seabream, Sparus aurata L. from the south-atlantic coast of Spain.

Lymphocystis disease of the gilthead seabream, Sparus aurata from the south Atlantic coasts of Spain was studied using various cytochemical methods for nucleic acids, proteins, carbohydrates and lipids. In lymphocystis infected cells, cytoplasm and nucleoli contained RNA. DNA was restricted to the periphery of the nucleus and within the intracytoplasmatic inclusions. During the development of infected cells, proteins rich in different aminoacids were observed in the granular cytoplasm, nucleus/nucleoli, intracytoplasm inclusions and hyaline capsule. Some glycogen was observed in the cytoplasm of these cells. The intracytoplasm inclusions and the hyaline capsule also contain hydrophilic lipids, being noticeable the presence of proteins containing S-S groups. Sulphated sialoglycoproteins and glycolipids and/or phospholipids were also components of the hyaline capsule.

Protein G-horseradish peroxidase based method for light-microscope immunocytochemistry. Application to the pituitary gland of the killifish, Fundulus heteroclitus.

Horseradish peroxidase-protein G conjugate was used to localize anti-human luteinizing hormone and anti-human chorionic gonadotropin primary antibodies bound to gonadotropins in paraffin and Historesin embedded sections of Fundulus heteroclitus pituitaries. The sensitivity and specificity of this method and those obtained after the avidin-biotin-peroxidase complex, the streptavidin-biotin-peroxidase complex and the peroxidase-antiperoxidase procedures were compared. The protein G-horseradish peroxidase method gave a clear immunostaining of gonadotrophic cells with virtually no background. Detection efficiency was reduced in comparison with the other techniques. As for the embedding medium, hydrophilic resin Historesin considerably enhanced the structural detail and the resolution of the immunostaining at light-microscope level.

[Neurophysiology of the development and maturation of the peripheral nervous system].

INTRODUCTION: Peripheral nerve maturation accounts during fetal life and infancy and varies with age. Nerve conduction studies are an objective procedure to investigate the development of the motor and sensory nerves. METHOD: We present a review of peripheral nervous system maturation studies in infancy, including our normal control group of infants from 1 week to 6 years of age. Motor and sensory nerve conduction velocity at birth is about one-half that of the normal adults, increases very quickly during the first year of life and reaches adult values between 3 and 5 years of age. All these electrophysiologic changes parallel the increase in numbers of large myelinated fibres throughout infancy. CONCLUSIONS: Nerve conduction studies are an accurate and non-invasive method of evaluating the peripheral nerve maturation in paediatric population when motor and sensory functions are particularly difficult to assess clinically.
