The Melancholia Scale and the Newcastle Scales. Item-combinations and inter-observer reliability.

The reliability of the total scores on three rating scales (Melancholia Scale and the two Newcastle Scales) and the algorithms leading to the Feighner, Research Diagnostic Criteria, and the DSM-III subtypes of depression have been compared. The degree of inter-observer agreements for the various item-combinations was significantly higher than would be expected by chance. The average agreement for each assessment system ranged from 80 to 93 per cent. This 7 to 20 per cent lack of total agreement probably reflects the limitation of clinical assessments including the influence of halo effects.

Comparison of metrizamide and iodophendylate for cerebral ventriculography: a long-term ultrastructural study of the ventricular wall in the rat.

Cerebral ventriculography was performed on adult albino rats to compare the effect of metrizamide and iodophendylate on the ependymal wall. The animals were killed by vascular perfusion 1 month and 4 months after ventriculography. The ependymal layer and the underlying brain tissue were examined with the electron microscope. After the use of metrizamide the tissue appeared completely normal. The use of iodophendylate provoked accumulation of phagocytozing macrophages on the ependymal wall and storage of iodophendylate in the ependyma and underlying brain tissue. These changes were present both 1 and 4 months after ventriculography.

Pituitary oncocytic tumor with Cushing's disease.

Pituitary adenomas composed of oncocytes are rare. We report a young female with an atypical oncocytic tumor of the pituitary. The histology was unusual because the oncocytic cells--in addition to an abundance of abnormal mitochondria--contained a conspicuous smooth endoplasmic reticulum. The tumor grew invasively and the patient developed adrenal hypercorticism.

CSF dopamine increased in depression: CSF dopamine, noradrenaline and their metabolites in depressed patients and in controls.

Some studies report reduced levels of the dopamine metabolite HVA in CSF in depression. In the present study including 24 depressed patients and 10 controls, we found significantly increased concentrations of total CSF dopamine in depressed patients. This finding suggests a dysfunction in central dopamine turnover in depression. No differences in CSF levels of noradrenaline or the amine metabolites homovanillic acid and 3-methoxy-4-hydroxy-phenylglycol were seen when comparing depressed patients with controls.

5-HT and 5-HIAA in cerebrospinal fluid in depression.

CSF 5-HT and 5-HIAA were measured in endogenously depressed patients (ICD-9) (n = 23) and controls (n = 11). Distribution of sex, age and body height was similar in the two groups. Non-parametric statistics were used. In depressed patients CSF 5-HT concentrations were found to be higher (P less than or equal to 0.01) than in controls. A further classification of the depressed patients by the Newcastle Scale showed that the highest values were found in the endogenous group compared to the non-endogenous group (P less than or equal to 0.02). CSF 5-HIAA was found to be equal in the two groups, even when pairs matched for height were compared. No relation between clinical recovery due to drug treatment and changes in CSF 5-HT was seen. Our data support a possible involvement of 5-HT in the biology of depression, but the anatomical and functional levels of a serotonin derangement are still unknown.
