Platelets attenuate oxidant-induced permeability in endothelial monolayers: glutathione-dependent mechanisms.

We studied the effects of adding washed human platelets or platelets with nonintact glutathione redox cycles to endothelial cell monolayers treated with glucose oxidase to initiate oxidant stress and increase permeability. Changes in 125I-labeled albumin transmonolayer movement were used as the index of monolayer permeability. Washed human platelets attenuated oxidant-induced increases in albumin flux. Platelets treated with 1,3-bis(2-chloroethyl)-1-nitrosurea, 1-chloro-2,4-dinitrobenzene, or buthionine sulfoximine to inhibit selective enzymatic steps in the glutathione redox cycle decreased permeability to a lesser degree. We conclude that 1) washed human platelets attenuate monolayer permeability defects in aortic endothelial monolayers exposed to glucose oxidase and 2) the protective effects of platelets are partially dependent on an intact platelet glutathione redox cycle.

Effect of xanthine-related compounds on a theophylline assay using theophylline oxidase.

OBJECTIVE: This study investigated a new, nonimmunologic, enzymatic-based theophylline assay for the possible interference of a clinically achievable serum theophylline concentration by four xanthine-related compounds: pentoxifylline, 1-methylxanthine, allopurinol, and oxypurinol. RATIONALE: The enzyme theophylline oxidase is used in this assay kit to convert theophylline to 1,3 dimethyluric acid in the presence of cytochrome C. Cytochrome C is reduced to ferrocyte C, which is then measured by ultraviolet spectrophotometry at one minute and again at five minutes. The rate of appearance of ferrocyte C over this time is then related to a one-point 20-micrograms/mL (111-mumol/L) theophylline standard provided with the kit and the concentration of theophylline is calculated. Xanthine-related compounds have the potential to interact with theophylline oxidase via: (1) structural similarity, and (2) inhibition of xanthine oxidase, which increases theophylline metabolite concentrations. DESIGN: Human serum was spiked with known, therapeutically achievable concentrations of pentoxifylline, 1-methylxanthine, allopurinol, and oxypurinol, both alone and in combination with theophylline in a concentration of 15 micrograms/mL (83.3 mumol/L). Two controls were used: human serum and human serum spiked with theophylline. Data were analyzed statistically with ANOVA, and clinically with +/- 10 percent control criteria. RESULTS: Means (+/- SEM) for the control samples were 2.0 (0.2) micrograms/mL for serum alone and 15.3 (0.3) micrograms/mL for theophylline-spiked serum. For the test specimens, the results ranged from 2.3 to 2.8 micrograms/mL and from 13.9 to 16.6 micrograms/mL when xanthine-related compounds were added to serum without and with theophylline, respectively. CONCLUSIONS: The results were not statistically or clinically significant for the four xanthine-related compounds at the concentrations studied.

Pleural fluid analysis in malignant mesothelioma. Prognostic implications.

Pleural fluid characteristics of 26 patients diagnosed with malignant mesothelioma over an 18-year period were reviewed and compared with those of patients with effusions due to other malignancies. Survival from time of initial thoracentesis was directly correlated with pleural pH and decreased pleural fluid/serum glucose ratios but was not related to protein or LDH concentration. Decreased pleural fluid pH was statistically related to a decreased pleural fluid/serum glucose ratio. Patients with low pH (less than 7.30) pleural effusions had statistically decreased mean survival and decreased mean pleural fluid/serum glucose ratios compared with patients with normal pH (greater than or equal to 7.30). Patients with malignant mesothelioma pleural effusions had statistically decreased mean pleural fluid pH compared with patients with other malignant pleural effusions. Decreased levels of pleural fluid pH and pleural fluid/serum glucose ratios have negative prognostic significance and probably correlate with the degree of tumor bulk affecting the pleural surface.

Renal vein thrombosis. Initial manifestation of Goodpasture's syndrome.

We report a patient who presented with renal vein thrombosis and nephrosis that progressed to alveolar hemorrhage and renal failure. Renal biopsy immunofluorescence and serum antiglomerular basement membrane antibody titer studies confirmed the diagnosis of Goodpasture's syndrome. To our knowledge, this is the first report of renal vein thrombosis as the initial presentation of Goodpasture's syndrome.

Hypothyroidism and pleural effusions.

Serous effusions have been thought to be an unusual complication of hypothyroidism and most commonly have been associated with ascites, pericardial fluid and heart failure. Pleural fluid as an isolated finding in hypothyroidism is apparently rare and complete analysis of these hypothyroid-associated pleural effusions has not been described. To determine the frequency, chemical characteristics and clinical associations of hypothyroidism and pleural effusions, the medical records of 128 patients with hypothyroidism (defined by an increased serum TSH concentration) were reviewed. The majority of effusions in patients with hypothyroidism were due to other diseases. Effusions solely due to hypothyroidism appeared to be a real entity. These effusions were borderline between exudates and transudates and showed little evidence of inflammation.
