The radial forearm flap for reconstruction of the upper extremity.

The radial forearm flap, owing to its good-caliber arteries of long length and equally well distributed venous system, has proved very reliable. It has not only earned its place and recognition in reconstructive hand surgery, but also has emerged as a workhorse for the microvascular surgeon. We have used 14 radial forearm flaps for upper extremity reconstruction, and we present herein our experience. The technique of extracorporeal tissue transfer, which has been published elsewhere, was used in two patients and is detailed. Four representative patients are presented.

Nasolabial myocutaneous island flap for reconstruction of palatal fistula (a case report).

With better understanding of microcirculation of the flaps and with the introduction of the concept of myocutaneous flaps, nasolabial flap has regained its past popularity. Although it has been used increasingly for oral defects, its use for reconstruction of palatal defects has not been reported. Large anterior fistulae of palate are commonly produced when a wide cleft of palate is being repaired in an adult patient, especially by a beginner in plastic surgery, and are difficult to treat surgically. We have used superiorly based nasolabial myocutaneous island flap in one such case and report it here.

Extracorporeal tissue transfer for extensive facial defects.

The use of extracorporeal tissue transfer has been extended in cases of extensive facial defects. Four cases with such defects have been performed successfully. The technique is described in brief and three out of four cases are detailed.

Nerve apportionment in microneural surgery.

The nerve apportionment technique consists of dissecting out several fascicles of a donor nerve, leaving the remainder intact. This avoids the problem of sensory loss in the distribution of the donor nerve. The technique may be applied either for microneural interfascicular nerve grafting or to provide a recipient area for a sensory nerve from a transferred flap.

Simplified microvascular clamps.

A simple method to produce an effective new pattern of microvascular clamp, which is easily assembled from inexpensive and widely available components, is presented.

Reconstruction of partial rectal and sacral loss by gracilis and gluteus maximus muscle island transfer. Report of a case.

This study presents a rare case of extensive tissue loss from the posterior pelvic wall (gluteal, sacral, and perianal region) and from the intrapelvic organs (rectum) following unusual trauma. This patient underwent successful reconstruction by a method that has not been previously described. Rectal wall reconstruction was performed using gracilis muscle transfer, and sacral reconstruction was performed by gluteus maximus muscle island transfer. Three-year follow-up demonstrated a normal life pattern.
