[Dilated myocardial disease as sequela of chronic Lyme carditis].

A report is given on a 39-year-old man, who developed, after a tick bite, erythema, progressing symptoms and, after 3 years, showed the typical clinical and echocardiographic signs of a dilative cardiomyopathy. A serological test for Lyme Borreliosis was positive. Most of the symptoms disappeared after high-dose penicillin therapy.

[Recurrent, atypical uterine prolapse after the end of early puerperium in a cow of the German black pied breed (clinical case report)].

A case of a recurrent incomplete uterine prolapse in a two year-old cow of the German Black Pied breed delivered into the clinic at the 16th day after parturition is described. An inversio uteri that possibly relapsed very soon following reposition of an uterine prolapse post partum is presumably the reason of the prolapsus uteri beyond the early puerperal period. The prolapsed left uterine horn was enclosed tightly by the cervix. Due to severe inflammatory changes a reduction was not practicable.

[Drainage in surgery of the heart and great vessels].

Based upon some recent experience with pericardial drainage a comparison is established between the surgical procedure and the needle-punction; the best approach to the pericardium, depending on the nature of the effusion is described. In the second part, the value of various types of drainage used since 19 years in the cardiac surgery is reviewed on the basis of our own experience with more than 7000 cases.

[Ventricular arrhythmias in coronary heart disease. Analysis by long-term-ecg, coronary arteriography and left ventriculography (author's transl)].

A dual channel long-term-Ecg monitoring system was used to investigate arrhythmias in 90 patients undergoing diagnostic coronary arteriography. There was no significant difference of ventricular arrhythmias during an observation period of 10 hours before and after cardiac catheterization. The incidence of ventricular premature beats (VPB) was significantly higher in coronary patients than in normal controls (normal coronary arteries and normal ventriculograms) or in a group of 24 healthy soldiers. In patients with coronary artery disease (CAD) the incidence of VPB's was 82 %. There was, however, no significant correlation between the severity of CAD and the number of VPB's although patients with 3-vessel disease showed a tendency to produce more extopic beats. In contrast, patients with generalized or regional left ventricular akinesis (systolic shortening less than 15 %) showed a significantly higher number of VPB's than patients with normal or hypokinetic ventricles. In addition the analysis of qualitative criteria of ventricular arrhythmias with regard to their prognostic significance following Lown's definition showed the same close correlation with ventricular dysfunction. In the group of patients with VPB's of high incidence and prognostic importance cardiac index and ejection fraction was significantly decreased and left ventricular enddiastolic pressure significantly elevated. In conclusion, coronary patients with left ventricular akinesis are prone not only to produce congestive heart failure, but also life-threatening ventricular arrhythmias.

[Changes of the phosphatides and their fatty acids in the retina and in the fasciculus opticus after retinal detachment: investigations of human and animal retinae (author's transl)].

In this study the effect of an experimentally provoked retinal detachment on the pattern of the phosphatides and fatty acids of the retina and the optic nerve of adult rabbits was investigated. The analysis was performed one month, and 4 months after the operation, and the values were related to the findings in control animals of the same age, and of the age of one day and 30 days respectively. In this way changes in the total lipide, in the phosphatides and in their fatty acids could be revealed, with a tendency towards the developmental stage of the 20th up to the 30th day of life. Between retina and optic nerve no difference was found neither temporally nor regarding the quantitative reaction. In the detached human retinae the same reactions can be proved as in the animal experiment. The relationship of these findings to the recovery of the operatively reatached human retina is discussed.

[Lesion of the retinal fatty acids following intravitreal injection of iron. An experimental study on the retinal lesions in cases of siderosis bulbi (author's transl)].

In this study the alterations of the pattern of the fatty acids of the retinal phosphatides after intravitreal injection of ferrous ammonium sulphate are treated. In vitro and in vivo a significant decrease of the highly unsaturated fatty acids of the C22 type occurs. This lesion spreads from the region of direct contact between iron and retina towards the periphery within 24 hs. The reliability of the experiment is ensured by the proof of analogous changes in human globes affected with siderosis. Finally, the production of lipid peroxides as a cause of the breakdown of the retinal function is discussed.

[Risk of recurrence in HELLP syndrome].

The recurrence risk of HELLP-syndrome is reported to be between 2.6% and 24%. But yet, there are no accurate case reports about this topic available. In a retrospective study, 25 patients, which had suffered from a pre partum HELLP-syndrome, were interviewed about possible subsequent pregnancies. In 7 patients 8 pregnancies were found, which began between 5 and 55 months after the HELLP-syndrome. No recurrence of a HELLP-syndrome was observed in these 7 patients. Although in 71% a hypertension had been present during the HELLP-syndrome, only 1 patient had an elevated blood pressure in the subsequent pregnancy. Whilst all of the patients with HELLP-syndrome had been delivered by cesarean section, 50% of the patients were delivered vaginally in the subsequent pregnancy. There was no evidence of maternal or neonatal complications related to HELLP-syndrome. Nevertheless, even if the recurrence risk seems to be low, pregnancies after HELLP syndrome should be observed carefully.
