Case report of a 51 years old woman with an acute generalized pustulosis. The histopathological findings in the epidermis were those of pustular psoriasis. But vascular findings in the corium were suggestive of a drug reaction, as was the history of a drug-induced onset of the disease in the course of an infect of the upper respiratory tract. Thus exanthematic pustular psoriasis may be considered as both diseases, psoriasis and drug reaction.
A 28 year old African student had acquired a mycetoma of his left foot after an injury in 1975. As the surgical and antimycotic therapy had no effect, he was brought to Düsseldorf for the amputation of his foot. Madurella mycetomi was isolated from the skin, pus and bone. Before the amputation was made a last attempt to treat him with the antimycotic agent miconasol nitrate, administered intraarterially through the arteria tibialis posterior, which had been shown i arteriography to perfuse the whole foot. On the 23rd day of therapy pain and fever indicated bacterial infection. The fungus could still be isolated from the bone tissue. The therapy was terminated on the 30th day. The catheter was blocked and amputation of the food became inevitable. The patient received an artificial limb and is able to move freely. In similar cases of mycotic bone infection an attempt with intraarterial infusion of an antimycotic agent should be made immediately.
The report concerns a southern Italian female patient of--at her death--67 years, who developed in 1975 within several months Spiegler basaliomas of the scalp. The tumors increased in size and number, and developed immediately to dedifferentiated, relapsing, metastasizing and finally lethal tumors which resisted to each therapy. Histologically only the structural rests of Spiegler basaliomas could be seen, but the cellular picture demonstrated completely dedifferentiated carcinomas. In this observation there was no dominant inheritance, as it is known in Spiegler basaliomas. The late manifestation of the tumors with immediate malignancy, the lacking affection of other members of the family, the high familiar incidence of internal carcinomas, and, finally, the combination with other associated symptoms (brachydactyly, racket nails, the latter ones also in the mother) revealed this disorder as a new syndrome of Spiegler carcinomas with associated symptoms.
Prurigo occurs in the acute form of the disease (strophulus), in the subacute frorm (prurigo simplex subacuta), and the chronic form (prurigo nodularis Hyde). Transition stages between the subacute and the chronic form of the disease are common. Strophulus is characterized by few papulovesicles, which are disseminated on the trunk. Likewise, the lesions of prurigo simplex subacuta may be disseminated, or they may be acneiform, or are localised on the extremities only. Typically, prurigo nodularis Hyde occurs on the lower extremities. Strophulus is caused by the bites of insects or by gastrointestinal disorders. The causes of prurigo simplex subacuta include metabolic and endocrine disorders as well as skin diseases. The occurrence of the latter is demonstrated by the combination of atopic dermatitis and prurigo (prurigo diathetique Besnier). The metabolic and endocrine disorders include diabetes mellitus, hyperlipidemia, diseases of the liver, the stomach and the bowels, finally increased androgene levels in women. The same pathological conditions are found in prurigo nodularis Hyde. The extreme chronicity of the prurigo is most probably caused by the inefficiency of the therapy of internal diseases.
The assumption of an irreparable damage of the fetus can result from prenatal examination or from the known mode of inheritance of a genodermatosis. Xeroderma pigmentosum, sex-linked ichthyosis vulgaris and - provided an appropriate test wil be developed - Fabry's disease are absolute indications for interruption after prenatal examination. Neurofibromatosis, Gorlin's syndrome, systematized elastorrhexis and Mal de Meleda are relative indications when the genodermatosis is feared to deteriorate the mother's health. Bourneville's disease, neurofibromatosis, Gorlin's syndrome, the dominant forms of systematized elastorrhexis and Mal de Meleda are relative indications as to the risk to give birth to a sick child.
The purposes of this study were: 1) to determine in dogs under steady-state conditions whether prednisolone clearance increases with increasing dose as has been suggested from results obtained under nonsteady-state conditions in man and 2) to determine whether the prednisolone to prednisone conversion, apparently resulting from 11 beta-hydroxydehydrogenase, is saturable. Six dogs were given four different zero-order infusions of prednisolone resulting in steady-state levels from 23 to 5884 ng/ml.Prednisone, total and free prednisolone levels were assessed by high-pressure liquid chromatography and equilibrium dialysis. Total and unbound prednisolone clearances were calculated by dividing the infusion rate by the total or unbound prednisolone concentration at steady state. Both total and unbound prednisolone clearance exhibited a slight tendency to decrease (P < .05) with increasing prednisolone steady-state levels. The conversion of prednisolone to prednisone, defined by the prednisolone/prednisone ratio, exhibited saturable characteristics which, when data for all dogs was fitted to a Michaelis-Menten type equation (r2 = 0.938), gave a Km of 658 ng/ml, i.e., total prednisolone concentration at half-maximal saturation.
Multiple malignant Spiegler tumors were observed on the scalp of a 67-year-old female patient with brachydactyly and racket-nails affecting all fingers of both hands. The occurrence of similar clinical features in the family of the patient is suggestive of a hereditary disorder. The light and electron microscopical findings are described.
Since patients vary in their response to furosemide, a pharmacokinetic study of this drug was undertaken in 7 patients with severe congestive heart failure. The extent of oral availability, determined by the ratio of areas under the oral to intravenous plasma concentration-time curve, averaged 61%, but varied widely from patient to patient, from 34 to 80%. Absorption of the drug was often delayed, with a lag time before appearance in the plasma averaging 28 +/- (SD) 15 min. The volume of distribution of furosemide (averaging 114 ml/kg) was not diminished in the presence of congestive failure; likewise, drug clearance and elimination half-lives are similar to values reported in normal subjects. Urinary flow rates 1--2 h following dosing were significantly less after oral than intravenous dosing, but flow rates over 4, 8 or 24 h after dosing were similar for both routes of administration.

Though particular immunological findings are reported in some cases of psoriasis (increased IgA-, IgE- and IgG-levels, decreased IgM-level, and occurrence of antiglobulin-factors), psoriasis should not be considered as an autoimmune disease. Likewise psoriasis is not a general disease, nor is it a systemic disease. The vascular alterations in the upper corium which are quite regularly found in the apparently-non-affected skin, are probably restricted to the extracerbations of the disease.
100 healthy persons were investigated to find out whether there is a correlation between a lymphocyte stimulation (3H-thymidine incorporation in the lymphocyte transformation test) with phytohaemagglutinin, pokeweed mitogen and concanavalin A and the in vivo reactivity to intracutaneous phytohaemagglutinin application (measured as medium diameter of infiltrates). Further we searched for an influence of positive or negative reactivity to the specific antigens trichophytin, candidin and streptokinase-streptodornase. 1. There is a good qualitative correlation in normal persons between in vitro lymphocyte stimulation by phytohaemagglutinin, pokeweed mitogen and concanavalin A and the medium diameter of infiltrates after intracutaneous application of 2 mug phytohaemagglutinin. Using lower doses of phytohaemagglutinin no such correlation could be found. A quantitative correlation between the medium diameter of infiltrates and lymphocyte transformation by phytohaemagglutinin and pokeweed mitogen cannot be demonstrated. For concanavalin A there is a negative correlation significant at the 99% level. 2. In vivo reactivity to phytohaemagglutinin does not differ in persons with negative and positive reactions to the specific antigens. 3. Lymphocyte stimulation by phytohaemagglutinin, pokeweed mitogen and concanavalin A of persons with positive reactions to trichophytin, candidin and streptokinase-streptodornase is significantly greater than in those with negative reactivity. 4. The results indicate in vivo phytohaemagglutinin testing as a good screening method to judge cell-mediated immunocompetence. The general applicability for differentiation of immunodeficient and immunocompetent patients is to be further investigated.
