Defective aggregation in cirrhosis is independent of in vivo platelet activation.

BACKGROUND/AIMS: Platelet function abnormalities contribute to the hemostatic defect in patients with cirrhosis. In this study we evaluated the occurrence of in vivo platelet activation as a possible mechanism of defective platelet aggregation in patients with cirrhosis. METHODS: Nine patients with severe (Child B-C) cirrhosis and defective platelet aggregation were studied in comparison with age- and sex-matched healthy controls. The presence of activated platelets in the bloodstream was evaluated by fluorescence-activated flow cytometry using antibodies directed against activation-dependent platelet proteins and by measuring plasma levels of beta-thromboglobulin and platelet factor 4. Urinary levels of 11-dehydro-TXB2 and of 2,3-dinor-TXB2 were assayed by radioimmunoassay following chromatographic separation. RESULTS: In unstimulated platelets, the expression of both GMP 140 and GP 53 was not significantly different in patients with cirrhosis and in controls. After stimulation with ADP and epinephrine, expression of activation-dependent antigens was lower in platelets from patients (GMP 140: 0.64 +/- 0.09 vs 0.73 +/- 0.04, p = 0.02; GP 53: 0.41 +/- 0.13 vs 0.54 +/- 0.14). Plasma levels of beta-thromboglobulin and platelet factor 4, as indexes of in vivo platelet activation, were also comparable in the two groups of subjects. Urinary levels of 11-dehydro-TXB2 and of 2,3-dinor-TXB2, the major systemic metabolites of TXA2, were significantly higher in patients with cirrhosis (1807 +/- 518 vs 341 +/- 121 ng/pg creatinine and 693 +/- 512 vs 205 (93 ng/pg creatinine, respectively, p < 0.001). However, increased excretion of TXB2 metabolites was also observed in three patients with chronic autoimmune thrombocytopenia. CONCLUSIONS: These data indicate that circulating platelets are not activated in cirrhosis, and that defective aggregation is most likely dependent on the alteration of the transmembrane signaling pathways. The increased urinary excretion of systemic TXA2 metabolites may be related to increased intrasplenic platelet destruction.

Detection of prosthetic vascular graft infection using avidin/indium-111-biotin scintigraphy.

UNLABELLED: Prosthetic vascular graft infection, though rare, carries high morbidity and mortality rates; therefore, timely diagnosis is important. Patients, however, often present with vague symptoms, and radiological investigations are frequently inconclusive. These factors may lead to prolonged periods of observation and hospitalization, with the resultant increase in costs and complication rates, before reaching a final diagnosis. This prospective study evaluates the use of nonspecific avidin/111In-biotin imaging in diagnosing prosthetic vascular graft infection. METHODS: Twenty-five patients with a total of 29 grafts were investigated. Eighteen patients (19 grafts) had low probability of disease, whereas the remaining 7 patients (10 grafts) warranted surgical exploration based on clinical, laboratory or radiological evidence. Avidin was first injected intravenously and then followed 24 hr later by administration of 111In-biotin. Whole-body images were obtained 10 min and 2 hr postinjection of 111In-labeled biotin. SPECT imaging was performed 1 hr postinjection. Increased uptake along part or the whole length of the graft was considered evidence of graft infection. RESULTS: Avidin/111In-biotin scintigraphy correctly identified all infected grafts, as confirmed by culturing surgical specimens. In contrast, infection was correctly excluded in all but one of the grafts, and long-term follow-up was used to assess the presence of infection in patients who did not undergo surgical intervention. CONCLUSION: Avidin/111In-biotin scintigraphy is a simple and accurate imaging method for the routine diagnosis of vascular graft infection, and it may have a role in identifying the disease process in its initial stages, thus improving prognosis.

[Acute myocardial infarction in bacterial endocarditis].

The authors report on a 47-years old woman with bacterial endocarditis involving both the mitral and aortic valves. At first echocardiographic examination, the mitral vegetation was small, while the aortic one was large highly mobile. Despite adequate antibiotic therapy, the aortic vegetation had become bigger and the valve regurgitation, initially mild to moderate, resulted severe and was associated with left heart failure. While awaiting surgery, the patient sustained an acute non Q wave myocardial infarction with ST segment elevation in inferior and anterolateral leads, complicated by ventricular arrhythmias. Thirty-six hours later, the patient received mitral and aortic valve replacement: at surgical view, the aortic vegetations was found to be very close to the right coronary orifice. After a period of further antibiotic therapy, the woman discharged and at a six months follow-up, she was fairly well. The authors review the mechanisms of acute coronary insufficiency in infective endocarditis and suggest an embolic pathogenesis in the case reported. Taking into account the possible life threatening embolic complications, it seems reasonable not to delay surgery when antibiotic therapy fails to reduce the size and mobility of valve vegetations.

[Effectiveness and tolerability of oral liquid ferrous gluconate in iron-deficiency anemia in pregnancy and in the immediate post-partum period: comparison with other liquid or solid formulations containing bivalent or trivalent iron].

UNLABELLED: From the early months of pregnancy and even more so later, women suffer a deficiency of iron along with a decline in their red blood cell count. It is also now clear that women who take iron supplements during pregnancy do not suffer the same post-natal reduction in hemoglobin and ferritin as those who don't make it. A study was therefore conducted on 40 women aged 20-35, with iron-deficiency anaemia during or immediately after pregnancy all of whom presented Hb < 10 gr/dl, Ht < 33% and serum iron < 60 micrograms/dl. All women with pregnancy-related pathological conditions, pre-existing on concomitant disease (Type I diabetes, heart diseases etc.) were excluded from the study. The women whose blood chemical parameters were largely homogeneous at the start of the study were divided into four treatment groups of 10 patients each and were treated as follows: Group A with oral liquid ferrous gluconate (75 mg per diem in 2 vials a day); Group B with solid ferrous gluconate (80 mg per diem in a single effervescent tablet); Group C with solid ferrous sulphate (105 mg per diem in a single tablet); and Group D with ferric protein succinylate (80 mg per diem in 2 vials a day). All were given iron treatment for 30 days. Treatment efficacy was analysed by comparing basal and final parameters using the T-test for paired dependent samples. The tolerance of the 4 treatment protocols was assessed by the analysis of any side effects such as nausea, vomiting, epigastric pain, diarrhoea, constipation or other disorders reported by patients during treatment. RESULTS: Analysis of the therapeutic efficacy parameters (red blood cells, hemoglobin, hematocrit and serum iron) showed significant improvements but no statistically significant differences between the groups. However, the Group A patients treated with oral doses of liquid ferrous gluconate received a significantly lower cumulative dose of iron elements than the other groups: in detail 150 mg (p < 0.05) less than Groups B and D; 900 mg (< 0.001) less than Group C. By the end of treatment the Group A patients revealed significant increases versus basal values in red blood cells (p < 0.001) 1,051,000 per mm3 or 33%, in Hb (p < 0.001) 2.83 gr/dl or 32%, in Ht (p < 0.001) 8.32% or 32%, in serum iron (p < 0.05) 19.5 micrograms/dl or 61%. The same group also showed an increase in Ferritin amounting to 7.8 micrograms/dl or 24% of the basal value. As to safety, only Group A patients reported no side effects and produced no drop-outs. Gastrointestinal and other aspecific side effects caused 1 drop-out each in Groups B and C and 2 drop-outs in Group D. CONCLUSION: Numerous preparations containing bivalent or trivalent iron are available for the treatment of iron-deficiency anaemia during or immediately after pregnancy. It has been shown that preparations containing ferrous salts (+2) are more easily absorbed than those containing ferric salts (+3) since the former can be immediately absorbed by the duodenal mucosa. The study reported here reveals that oral ferrous gluconate in liquid form is more effective and above all better tolerated than other solid or liquid formulations containing elementary iron.

[Usefulness of the new sequences of magnetic resonance in the study of hepatic hydatidosis].

We investigated the role of new MR Imaging techniques for the diagnosis, characterization and staging of hepatic hydatid disease. We examined 21 patients (30 hydatid cysts), 7 men and 14 women, ranging in age 26 to 74 years, with known hydatid disease. MR examinations were carried out on a 0.5T superconductive magnet (Philips Gyroscan T5, Philips Medical System) with the following imaging protocol: T1w (TR/TE/NEX: 300/10/4) SE, T2w (TR/TE/NEX: 3000/120/6) TSE and fat suppressed (SPIR technique) T2w (TR/TE/NEX: 3000/120/6) sequences. MR Angiography examinations were performed with 2D Time of Flight sequences (TR = 33 ms; TE = 6.9 ms; flip angle = 60 degrees; slice thickness = 4.0 mm with 2.0 mm overlapping; matrix = 256 x 256; number of slices = 45-50; acquisition time = 4 min 19 s), while MR cholangiography was performed with 3D, fat suppressed (SPIR) Turbo Spin-echo (TSE) sequences (TR = 3000 ms, TE = 700 ms, ETL = 12, acq. time = 5 min 48 s). MRI correctly detected all the hydatid cysts on both T1- and T2-weighted images. Characterization was correct in all the cysts larger than 3 cm, where typical signs consistent with hydatid disease were detected. MRA images always showed the inferior vena cava and the splenoportal system. The portal vessels were demonstrated only up to the first branches. In 3 cases an extrinsic compression of the inferior vena cava was diagnosed. MRC, performed in 7 cases, showed normal main bile duct caliber in 6 cases, while in another case, where a cyst ruptured inside the bile ducts, the communication between the cyst and the bile ducts was clearly demonstrated. In conclusion, MR Imaging is a valuable tool in the study of liver hydatid disease. Moreover, the availability of such new MR techniques as MRC and MRA, greatly improves the diagnostic role of MR imaging, especially when studying complications and before surgery.

[Quantification of mesenteric flow with magnetic resonance imaging before and after a meal: results in healthy volunteers].

Several literature studies showed that total intestinal blood flow approximates superior mesenteric vein flow. Today, new accurate techniques can be used to measure blood flow. We investigated MR capabilities in measuring mesenteric vein flow, to assess total intestinal blood flow. Nine healthy volunteers were examined before and after a meal with a phase-contrast technique to measure blood flow. Flow speed and quantity can be measured positioning a ROI inside the vessel for speed evaluation and around the vessel for flow quantitation. Superior mesenteric blood flow exhibited a three-fold increase after a meal relative to pre-meal values. Cine phase-contrast MRI was a useful tool to measure mesenteric flow in healthy volunteers both before and after a meal and can therefore be suggested for the noninvasive examination of patients with a suspected chronic mesenteric blood supply deficiency.

[Emergency surgical revascularization in acute myocardial infarct. The preliminary results of a prospective study].

In this paper we describe 1-year experience with a perspective operative protocol of emergency myocardial revascularization in extensive acute myocardial infarction (AMI). Entry criteria were: age < 75 years; anterior AMI with ST segment elevation > 4 leads, infero-postero-lateral or inferior and right ventricular AMI, within 6 hours from symptom onset. After coronary arteriography, an emergency staff, composed by cardiologists and cardiac surgeons, addresses the patients to coronary artery bypass grafting (CABG) or to percutaneous transluminal coronary angioplasty (PTCA). From November 1994 to November 1995, 35 patients were enrolled: 19 (mean age 54.3 +/- 9.7 years) underwent CABG and 16 were treated with PTCA. Myocardial protection was such as to restore energetic substrates and to prevent reperfusion injury: surgical technique consisted of antegrade-retrograde substrate-enriched blood cardioplegic solution delivery, early cardioplegic delivery on the infarcting area via a saphenous graft, retrograde controlled reperfusion before aortic unclamping and then prolonged reperfusion of the infarcted myocardium. In 8 patients (mean age 50.9 +/- 8.6 years), with anterior AMI and stable hemodynamics, a left internal thoracic artery graft was used, performing the prolonged controlled reperfusion retrogradely before aortic unclamping. In hospital death occurred in 1/19 (5.3%) patients because of cerebral hemorrhage. At a mean follow-up of 5.1 +/- 3.7 months 17 patients (94.4%) were in NYHA functional class I-II and 1 patient (5.6%) complained of effort angina, that was well controlled with medical therapy. Left ventricular ejection fraction calculated by echocardiography preoperatively, before discharge and at follow-up was respectively 39.3 +/- 12.7, 43.1 +/- 8.9 and 43.4 +/- 9.0%. In the last 8 consecutive patients thermodilution and transesophageal echocardiography monitoring were performed preoperatively and 12 hours after CABG: in all cases ejection fraction and cardiac index increased after CABG, from 42.2 +/- 13.5 to 48.6 +/- 14.3% (p = 0.01) and from 2.8 +/- 0.5 to 3.4 +/- 0.6 l/min/m2 (p = 0.005), respectively. The preliminary results show the effectiveness of this perspective protocol in the management of critically ill patients with extensive AMI.

Recombinant human erythropoietin in the anemia associated with multiple myeloma or non-Hodgkin's lymphoma: dose finding and identification of predictors of response.

Previous phase I-II clinical trials have shown that recombinant human erythropoietin (rHuEpo) can ameliorate anemia in a portion of patients with multiple myeloma (MM) and non-Hodgkin's lymphoma (NHL). Therefore, we performed a randomized controlled multicenter study to define the optimal initial dosage and to identify predictors of response to rHuEpo. A total of 146 patients who had hemoglobin (Hb) levels < or = 11 g/dL and who had no need for transfusion at the time of enrollment entered this trial. Patients were randomized to receive 1,000 U (n = 31), 2,000 U (n = 29), 5,000 U (n = 31), or 10,000 U (n = 26) of rHuEpo daily subcutaneously for 8 weeks or to receive no therapy (n = 29). Of the patients, 84 suffered from MM and 62 from low- to intermediate-grade NHL, including chronic lymphocytic leukemia; 116 of 146 (79%) received chemotherapy during the study. The mean baseline Hb level was 9.4 +/- 1.0 g/dL. The median serum Epo level was 32 mU/mL, and endogenous Epo production was found to be defective in 77% of the patients, as judged by a value for the ratio of observed-to-predicted serum Epo levels (O/P ratio) of < or = 0.9. An intention-to-treat analysis was performed to evaluate treatment efficacy. The median average increase in Hb levels per week was 0.04 g/dL in the control group and -0.04 (P = .57), 0.22 (P = .05), 0.43 (P = .01), and 0.58 (P = .0001) g/dL in the 1,000 U, 2,000 U, 5,000 U, and 10,000 U groups, respectively (P values versus control). The probability of response (delta Hb > or = 2 g/dL) increased steadily and, after 8 weeks, reached 31% (2,000 U), 61% (5,000 U), and 62% (10,000 U), respectively. Regression analysis using Cox's proportional hazard model and classification and regression tree analysis showed that serum Epo levels and the O/P ratio were the most important factors predicting response in patients receiving 5,000 or 10,000 U. Approximately three quarters of patients presenting with Epo levels inappropriately low for the degree of anemia responded to rHuEpo, whereas only one quarter of those with adequate Epo levels did so. Classification and regression tree analysis also showed that doses of 2,000 U daily were effective in patients with an average platelet count greater than 150 x 10(9)/L. About 50% of these patients are expected to respond to rHuEpo. Thus, rHuEpo was safe and effective in ameliorating the anemia of MM and NHL patients who showed defective endogenous Epo production. From a practical point of view, we conclude that the decision to use rHuEpo in an individual anemic patient with MM or NHL should be based on serum Epo levels, whereas the choice of the initial dosage should be based on residual marrow function.

Exogenous growth hormone administration does not inhibit the growth hormone response to hexarelin in normal men.

Administration of exogenous human growth hormone (GH) blunts the GH response to physiological as well as pharmacological stimuli, including GH-releasing hormone (GHRH). Hexarelin (Hex) is a new synthetic GH-releasing peptide (GHRP) similar to GHRP-6 with potent GH-releasing activity in animals and men. To determine whether the short-term administration of GH inhibits the Hex-induced GH release, we measured the GH response to Hex (2 micrograms/kg iv) in five normal adult males (age 26-32 yr) three h after an iv bolus of rhGH (2 IU) or saline. Mean incremental change of serum GH from value at time 0 was 47.5 +/- 5.5 and 41.5 +/- 4.1 micrograms/l after saline + Hex and GH + Hex, respectively. Mean incremental area under the curve over baseline was 3216 +/- 586 and 3735 +/- 506 micrograms.min.1 after saline + Hex and GH + Hex, respectively. One of the proposed mechanism of action of GHRPs is to serve as functional somatostatin (SRIH) antagonists, and it is known that GH feeds backs on the hypothalamus to stimulate SRIH release. Therefore, we speculate that antagonisms of SRIH function by Hex prevented the inhibitory effect of exogenous GH, thus lending further support to the hypotheses that SRIH is involved in the feedback regulation of GH secretion, and that GHRPs action involves inhibition of SRIH function.

Avidin and 111In-labelled biotin scan: a new radioisotopic method for localising vascular graft infection.

OBJECTIVES: To evaluate a new imaging technique, for diagnosis of prosthetic vascular graft infection. Avidin is a protein which accumulates nonspecifically at sites of inflammation or infection. Due to its extremely low dissociation constant with biotin sites of infection can be imaged, using avidin as a pre-target, followed by injection of 111In-labelled biotin. This technique is much simpler than the common scintigraphic methods which employ labelling of blood components and its target-to-background ratio is greater than the methods employing radiolabelled proteins. DESIGN: Prospective clinical study. SETTING: A single department of vascular surgery and one of nuclear medicine of a Northern Italian hospital. MATERIALS: Between May 1993 and May 1994, 31 grafts in 26 patients were studied; the series included 23 men and three women with a mean age of 65.5 years (range 54-76 years). The prosthetic graft (Dacron -16, ePTFE -15) were: aortoaortic 5, aortobifemoral 15, aortoiliac 1, and femoropopliteal 10. Sixteen patients were suspected of having a vascular graft infection (Group A), the other 10 patients served as controls (Group B). 20 mg of Avidin were injected iv, followed 24 h later by i.v. injection of 500 micrograms of Biotin labelled with 74 MBq of 111In. CHIEF OUTCOME MEASURES: Whole-body imaging was performed at 10 min and 2 h post-injection, along with SPECT imaging when indicated. Scan results were correlated with the traditional imaging modalities and the clinical outcome of the patients. MAIN RESULTS: In Group A: two patients (three grafts) were excluded from the study, there were six true-positives, one false-positive and 11 true-negatives. Results in Group B: 10 true-negatives. The overall sensitivity was 100%, the specificity 95%, the accuracy 96%, the positive predictive value 86% and the negative predictive value 100%. CONCLUSIONS: These data suggest that Avidin/111In-labelled Biotin scintigraphy is a useful non invasive diagnostic method for early diagnosis of suspected prosthetic vascular graft infection.

Advantages of delayed sternal closure in cardiac-compromised adult patients.

Severe hemodynamic dysfunction may follow closure of the median sternotomy in patients with myocardial edema, cardiac dilatation, postcardiotomy shock, or raised end-expiratory alveolar pressure. Open sternotomy and delayed sternal closure (DSC) is a well described adjunct in complicated cardiac operations, which is more widely applied in neonates. In this article we report our results in using open sternotomy in eight adult patients from January 1994 to February 1995 (excluding patients who needed ventricular assistance devices [VADs]). Three patients died in hospital: 1 case of multiorgan failure; 1 cases of refractory low cardiac output syndrome; and 1 case of respiratory distress syndrome. Our experience confirms that DSC is an effective means of dealing with postoperative hemodynamic impairment. Furthermore, this technique may represent an intermediate step between intraaortic balloon counterpulsation and VADs and should be given a role in the prophylaxis of low postoperative cardiac output and multiorgan failure, particularly when contraindications to VAD exist.

Mural thrombi in abdominal aortic aneurysms: MR imaging characterization--useful before endovascular treatment?

PURPOSE: To evaluate magnetic resonance (MR) imaging accuracy in the characterization of mural thrombi in abdominal aortic aneurysms (AAAs). MATERIALS AND METHODS: Forty-five patients (37 men, eight women; mean age, 68 years) with an AAA with mural thrombus thicker than 1 cm at sonography underwent T1- and T2-weighted spin-echo MR imaging. The thrombi were prospectively classified as one of three signal intensity (SI) categories: category 1 corresponds to low SI on both T1- and T2-weighted images; category 2, high SI on T1-and T2-weighted images; and category 3, inhomogeneous SI with hyperintense areas on both T1- and T2-weighted images. During surgery, thrombi were classified as organized, unorganized, and partially organized. MR imaging and surgical findings were compared. RESULTS: SI category 1 corresponded to organized thrombi in 24 of 24 patients. Category 2 corresponded to unorganized thrombi in 11 of 11 patients. In category 3, focal hyperintense areas corresponded (both for presence and location) to unorganized portions of partially organized thrombi in 10 of 10 patients. CONCLUSION: MR imaging is accurate in AAA thrombus characterization.

Circulating CFU-E during hematopoietic recovery after allogeneic bone marrow transplantation: relationship to erythroid engraftment.

The more mature erythroid progenitor assayable in vitro, the colony-forming unit-erythroid (CFU-E), is normally found in the bone marrow (BM) but is virtually absent from peripheral blood (PB), unlike the more immature progenitor, the burst-forming unit-erythroid (BFU-E). We report on the detection of CFU-E in the PB of six of 18 patients during hematopoietic recovery following allogeneic bone marrow transplantation (BMT); three of six patients with PB CFU-E were under treatment with recombinant human erythropoietin (rhEpo) as well as six of 12 who did not present with PB CFU-E. PB CFU-E were found as early as day 14 following BMT, reached a peak on day 28, and were still detectable on day 60. The presence of PB CFU-E was associated with signs of stimulated erythroid engraftment--an accelerated reticulocyte recovery, an increased number of reticulocytes, higher levels of serum transferrin receptor, and a reduction in transfusional requirements were found in these patients compared to those without PB CFU-E. The numbers of PB and BM BFU-E were similar in the two groups, as well as the numbers of PB and BM CFU-granulocyte/macrophage (CFU-GM) and multipotential CFU (CFU-GEMM); on the other hand, the percentage of BM BFU-E in S phase of the cell cycle was higher in the group of patients with PB CFU-E. While there was no difference between the two groups in serum Epo levels assayed on days 14 and 28 after BMT, patients with PB CFU-E had higher Epo levels in serum samples collected before starting the BMT procedure. These data suggest that the appearance of circulating CFU-E early after BMT is characteristic of a group of patients with an accelerated erythroid engraftment, although the mechanisms leading to the circulation of CFU-E after BMT remain unclear.

A new ePTFE stretch graft for aorto-iliac reconstructions. Surgical evaluation and one year follow-up with magnetic resonance imaging.

Since its introduction at the beginning of the 60s Dacron has proved to be a reliable and durable substitute for aortic reconstructions. It is however susceptible to late thrombosis, infection and aneurysmal dilatation in a significant number of cases. ePTFE vascular prosthesis, introduced in 1972, have proved to perform well for peripheral reconstructions and was used in aortoiliac position since 1980. In 1991 a new type of ePTFE rendered longitudinally extensible (Gore-Tex Stretch) was introduced into clinical practice. The aim of this study is to evaluate surgical performance, short term patency and interaction with periprosthetic tissues of Gore-Tex Stretch straight and bifurcated prosthesis. Between October 1991 and December 1993, 59 patients underwent aortic reconstruction at our Institution either for infrarenal aortic aneurysm (19 patients, 16 males, 3 females, mean age 68.8 +/- 8.5 years) or for aortoiliac occlusive disease (40 patients, 36 males, 4 females, mean age 60.5 +/- 7.2 years). Forty-eight bifurcated and eleven straight grafts were implanted using Gore-Tex sutures. Preoperative workup included Ultrasonography, Magnetic Resonance Imaging (MRI) in patients with aneurysms and Intra Arterial Digital Subtraction Arteriography (DSA) in patients with occlusive disease. Intraoperative parameters were recorded. Postoperative follow-up included ultrasonographic evaluation at 1st and 7th day, 15th postoperative week and at 3 monthly intervals; Intra Venous DSA before dismission and MRI at the 1st and 15th postoperative week. Intraoperatively the grafts proved to be soft and flexible with good handling and suturing characteristics. Perioperative bleeding was 480 +/- 299 ml for occlusive disease cases and 633 +/- 314 for aneurysm cases. No operative mortality was recorded.(ABSTRACT TRUNCATED AT 250 WORDS)
