Safety and ergonomics evaluation of hybrid systems in Hong Kong.

This study is aimed at understanding common safety and ergonomics problems with robot installation in Hong Kong. Information was collected through direct observations and personal interviews with robot users from advanced hybrid manufacturing systems. The application areas of robots and types of safety devices employed were investigated. The ages, types of jobs, daily working hours and working conditions of personnel were examined. Fire precaution measures, types and varieties of training programs for users were analyzed. The results showed that although investments were made for installation of robots for improving production activity, limited attention was paid to the health, safety, and ergonomics aspects in the hybrid systems. There were also cases reported where no formal guidelines for robot manipulation and operation were provided. It seems that management and users paid attention to robot function and capability more than robot safety during robotization process. Conclusions were made by the authors that efforts should immediately be made to adopt recommendations on the safety devices and facilities and training requirements for improving users' health and safety before fatal accidents occur.

Photocatalytic thin film cascade reactor for treatment of organic compounds in wastewater.

The photocatalytic oxidation of benzoic acid was investigated in a pilot scale-cascade photoreactor. The photoreactor consists of an array of UV lamps (40 W, 365 nm) illuminating a cascade of three inclined 316 stainless steel plates, on which titanium dioxide (TiO2) was immobilized by electrophoretic deposition. The percentage removal of total organic carbon (TOC) of liquid samples was determined. The photocatalytic process was affected by several operating parameters. Increasing the solution temperature was found to reduce the dissolved oxygen (DO) level and to decrease the rate of the degradation process. The Langmuir-Hinshelwood equation was found to be accurate for modeling the degradation of benzoic acid with initial concentrations of 50 ppm, 75 ppm and 100 ppm. The rate of removal of TOC was positively affected by UV light intensity, but appeared to be independent of solution flowrate in the range examined. Control experiments confirmed that the effects of adsorption of the solute onto the TiO2 catalysts and photolytic degradation were negligible.

Cardiopulmonary exercise testing in heart transplant candidates.

INTRODUCTION: We evaluated the results of patients with congestive heart failure (CHF) who underwent cardiopulmonary exercise testing (CPET) as part of their assessment for heart transplantation in order to examine the relationship between exercise capacity and resting indices of left ventricular function in these patients. MATERIALS AND METHODS: Twenty-seven ambulatory heart transplant candidates underwent CPET using a cycle ergometer and an incremental work-rate protocol till symptom-limitation. These patients included 24 men and 3 women with a mean age of 42.3 years. The aetiology of CHF was coronary artery disease in 14 patients, dilated cardiomyopathy in 11 patients, and congenital heart disease in 2 patients. Mean resting left ventricular ejection fraction (LVEF) was 19% (range 7% to 36%). RESULTS: Thirteen of the 27 tests performed were maximal studies and all except 2 subjects attained a measurable anaerobic threshold during CPET. Of the 13 maximal tests, the causes of exercise limitation were cardiomyopathy in 3 patients, ischaemic heart disease in 2 patients, significant oxygen desaturation in 2 patients, ventilatory limitation due to obstructive lung disease in 1 patient, ventilatory limitation secondary to a restrictive lung disease in 3 patients, and combined obstructive ventilatory and cardiovascular limitation in 2 patients. There was no significant correlation between resting LVEF and peak VO2 percent predicted (r = 0.14, P = 0.49). CONCLUSIONS: Exercise intolerance in patients with CHF may not be related to limited cardiac reserve and non-cardiac causes of exertional symptoms should also be considered. CPET is useful for the evaluation of functional capacity and mechanisms of exercise intolerance in patients with CHF.

Consequences of the photodynamic treatment of resting and activated peripheral T lymphocytes.

The impact of the immunomodulatory photosensitizer benzoporphyrin derivative monoacid ring A (BPD-MA, verteporfin) and visible light on the survival and surface receptor pattern of resting and activated murine T cells was evaluated. T cells treated for 48 h with immobilized anti-CD3 monoclonal antibody upregulated expression of the interleukin-2 receptor alpha-chain (CD25), transferrin receptor (CD71), the apoptosis-regulating Fas receptor (CD95), contained a greater level of the anti-apoptotic protein Bcl-2 and accumulated significantly more BPD-MA than their unactivated counterparts. Activated T cells displayed a modestly greater susceptibility to the photodynamic induction of DNA fragmentation than resting T cells. Resting T cells treated with sub-lethal levels of BPD-MA and light did not exhibit changes in surface levels of CD3, CD4, CD8, CD28, CD45 or T cell receptor (TCR) beta-chain structures. However, levels of major histocompatibility complex (MHC) class I antigens were decreased while the density of Thy-1.2 (CD90) increased on these cells. Photodynamically treated T cells failed to express optimal CD25 levels when exposed to the mitogenic anti-CD3 antibody. Activated T cells treated with sub-lethal levels of BPD-MA and light exhibited lower CD25 levels, a temporary block in cell cycle transition, but unaltered expression of MHC Class I, CD3, CD4, CD8, CD45, CD54, CD71, CD122 (IL-2R beta-chain) or TCR beta-chain antigens 24 h afterward. Resting and activated T lymphocytes differ in susceptibility to PDT-mediated apoptosis but both types are sensitive to anti-proliferative effects the treatment exerts at sub-lethal photosensitizer levels. The marked sensitivity of activated T cells to photodynamic inactivation likely contributes to the immunomodulatory action of BPD-MA.

Photodynamic therapy and immunity.

Photodynamic therapy (PDT) is widely recognized as a technique with which to treat malignant tumors that are accessible to an activating light source. PDT utilizes light absorbing compounds that catalyse the formation of cytotoxic oxygen species to produce the antitumor effect subsequent to direct light irradiation. PDT also exhibits immunomodulatory attributes. The photodynamic treatment of solid tumors triggers a large influx of granulocytes and macrophages into the region, leading to T-cell mediated anti tumor immunity against residual cancer. In apparent contrast, the application of levels of light less than those required for tumor ablation and over a larger body surface lessened disease severity when applied in murine autoimmune models. Furthermore, PDT is inhibitory for immunologically-mediated reactions to topically applied chemical haptens. The capacity of PDT to influence immune responses appears related to its capacity to target activated T lymphocytes as well as influence the immunostimulatory attributes of antigen presenting cells.

Revising and validating the random search model for competitive search.

A random search model was fit to a total of 2592 visual search times on a single-target detection task. By using a competing homogeneous background and uniform stimulus material, specifying viewing distance, controlling the presentation of search task material, and eliminating some options for extreme search strategies, very high correlation coefficients were found when a random search model was fit to both the individual data and to pooled data. A response time parameter was incorporated into the traditional random-search model and very good predictions of search performance were obtained.

Inhibition of contact hypersensitivity with different analogs of benzoporphyrin derivative.

Four structural analogs of benzoporphyrin derivative (BPD), a potent anti-tumor photosensitizer, were evaluated for their capacity to influence the immunologically-mediated contact hypersensitivity (CHS) response against the hapten 2,4-dinitrofluorobenzene (DNFB). Immunocompetent hairless strain mice received BPD monoacid ring A (BPD-MA, verteporfin) and returned to normal housing conditions or treated with 690 nm red light (transcutaneous photodynamic therapy, PDT). Unexpectedly, we found that mice given BPD-MA exhibited significantly reduced CHS ear swelling responses to DNFB upon antigenic challenge, whether or not they had been treated with PDT. A significant reduction in the CHS response to DNFB was observed when BPD-MA or PDT was given 48 or 24 h prior to, on the same day, or 24 or 72 h after DNFB sensitization. However, the magnitude of the CHS response was unaffected if these treatments were given 96 h after DNFB sensitization, 24 h before challenge with DNFB. Significantly reduced CHS responses also occurred in Balb/c mice given BPD-MA with or without PDT. Mice given BPD-MA but retained in total darkness throughout the experimental period generated full-fledged ear swelling responses to DNFB indicating that CHS suppression with BPD-MA was light dependent. BPD monoacid ring B (BPD-MB) strongly reduced the CHS response of Balb/c mice kept under ambient light while BPD diacid ring A (BPD-DA) and BPD diacid ring B (BPD-DB) also lowered the CHS response but were less effective than the monoacid forms. Other photosensitizers including Photofrin, tin etiopurpurin, and zinc phthalocyanine did not alter the CHS response of Balb/c mice maintained under ambient light. The ability of different BPD analogs to inhibit the CHS response in mice held under ambient light conditions appears related to the potent photosensitizing activity of these compounds.

From maximum surgical blood ordering schedule to unlimited computer crossmatching: evolution of blood transfusion for surgical patients at a tertiary hospital in Hong Kong.

The maximum surgical blood ordering schedule (MSBOS) was developed to permit efficient use of blood stocks and reduce blood wastage due to outdating. Success of the MSBOS required full cooperation between surgeons, anaesthetists, blood bankers and house officers. However, clinical staff sometimes disregarded the system or distrusted the ability of the laboratory to provide blood in an emergency. This led to frustration and wastage of laboratory staff effort. We developed a computer crossmatch system that could overcome the shortcomings of the MSBOS. The advantages of this system are reported.

A novel system for providing compatible blood to patients during surgery: "self-service" electronic blood banking by nursing staff.

BACKGROUND: A good blood bank must be able to provide compatible blood units promptly to operating room patients with minimal wastage. A "self-service" by nursing staff blood banking system that is safe, efficient, and well-accepted has been developed. STUDY DESIGN AND METHODS: Specific blood units are no longer assigned to surgical patients who have a negative pretransfusion antibody screen, irrespective of the type of surgery. A computer-generated list of the serial numbers of all group-identical blood units currently in the blood bank inventory is provided for each patient. The units themselves are not labeled with a patient's name. The group O list will be provided for group O patients, the group A list for group A patients, and so forth. Should the patient require transfusion during surgery, the operating room nurses go to the refrigerator, remove any group-identical unit, and check the serial number of the unit against the serial numbers on the patient's list. If the serial number is on that list, the blood bank will accept responsibility for compatibility. The system was implemented in 1995. RESULTS: Since implementation, a total of 2154 patients have undergone operations at this hospital. Thirty-two patients received more than 10 units of red cells each. There were no transfusion errors. The crossmatch-to-transfusion ratio was reduced from 1.67 to 1.12. Turnaround time for supplying additional or urgent units to patients in operating room was shortened from 33 to 2.5 minutes. There was no incidence of a blood unit's serial number not being on the list. Work by nurses and technical staff was reduced by nearly 50 percent. CONCLUSION: The "self-service" (by nursing staff) blood banking system described is safe and efficient. It saves staff time and can be easily set up.

Transcutaneous photodynamic therapy alters the development of an adoptively transferred form of murine experimental autoimmune encephalomyelitis.

Transcutaneous photodynamic therapy (PDT), utilizing benzoporphyrin derivative monoacid ring A (BPD, verteporfin) and whole-body light exposure, was assessed for its capacity to modify the course of adoptively transferred experimental autoimmune encephalomyelitis (EAE) in PL mice. Using a novel cell culture technique to facilitate the induction of this neurodegenerative condition, disease signs commenced 3-4 weeks after the transfer of myelin basic protein (MBP)-reactive lymph node or spleen cells to naive syngeneic recipients. Mice administered MBP-sensitized lymph node cells preincubated with BPD followed by whole-body 690 nm light irradiation (15 J/cm2) did not display symptoms of EAE. Although almost all animals given MBP-sensitized spleen cells developed EAE, mice given BPD (1 mg/kg) and the light treatment 24, 48 or 120 h after spleen cell transfer exhibited significantly less severe disease symptoms than control animals. Mice given the photodynamic treatment 24 h after spleen cell transfer also exhibited a significantly later disease onset than the control animals. Treatment of mice with PDT 24 h prior to spleen cell transfer did not influence subsequent disease severity but modestly delayed its onset. In the absence of directed light, BPD did not influence the development of EAE. Spinal cord tissues were evaluated for the presence of T cell receptor (TCR) V alpha 4 mRNA transcripts that specifically encode for the TCR alpha-chain of MBP-reactive T cells of PL mice. Using the polymerase chain reaction, V alpha 4 TCR mRNA transcripts were present in spinal cord samples prepared from almost all control mice but in only about one-half of spinal cord samples prepared from mice treated with PDT 24 h after spleen cell transfer. These observations indicated that PDT had limited the expansion of MBP-specific V alpha 4+ T cells within the central nervous system. Transcutaneous PDT represents a new technique with which to approach the treatment of autoimmune disease.

Development of an anthropometric database for Hong Kong Chinese CAD operators.

With the increasing popularity of using computer-aided design (CAD) in Hong Kong, it is time to look into the design of a suitable workplace for the CAD operators working in the industries. This can be achieved by applying anthropometric data into the design for enhancing performance and reducing musculoskeletal problems. In order to avoid any mismatches in anthropometric dimensions, which is believed to be one of the main causes of fatigue and occupational illness among workers, in the workplace design for them, eleven relevant body dimensions from a group of 150 Hong Kong male adults with ages ranging from 18 to 28 years were collected and analyzed to develop a computerized anthropometric database for Hong Kong CAD operators. The eleven anthropometric dimensions measured were: shoulder rest height, elbow-fingertip length, shoulder breadth, hip breadth, sitting eye height, elbow rest height, third lumbar disc-pan length, thigh clearance height, popliteal height, buttock-popliteal length, and stature. The computerized database can provide some useful statistics such as mean, standard deviation and relevant percentiles of all the measured dimensions. With the aid of these statistics, a new workplace for CAD operators was designed which improved performance substantially.

Sensitivity of activated murine peritoneal macrophages to photodynamic killing with benzoporphyrin derivative.

This study compared the ability of highly purified resting and activated DBA/2 mouse peritoneal macrophages to survive treatment with the photosensitizer benzoporphyrin derivative (BPD, verteporfin) and light. Culture of macrophages with recombinant murine interferon-gamma (rIFN-gamma, 100 U/mL) for 72 h imparted a phenotypic and functional activation by dramatically increasing cell surface expression of major histocompatibility complex Class II (Ia) molecules and the formation of nitric oxide. The rIFN-gamma-activated macrophages were significantly (P < 0.05) more sensitive (lethal dose to cause a 50% reduction in cell survival, LD50 = 14.4 +/- 1.1 ng/mL) to photodynamic killing with BPD and light (10 J/cm2) than cells (LD50 = 18.2 +/- 2.0 ng/mL) cultured in medium alone. In contrast, macrophages treated with different concentrations of bacterial lipopolysaccharide (LPS) were as resistant or more resistant to photodynamic killing than cells cultured in medium alone. No cytotoxic effect of BPD was detected in cultures containing the drug but protected from light. Comparable amounts of BPD were taken up in vitro by unactivated and rIFN-gamma-activated macrophages, as detected by flow cytometric analysis. However, cells cultured with LPS (10 micrograms/mL) took up more BPD than macrophages cultured in medium alone or with rIFN-gamma. The DBA/2 P815 mastocytoma cells took up greater amounts of the drug and were subsequently more vulnerable to treatment with BPD and light (LD50 = 6.9 ng/mL) than macrophages cultured under any condition. The explanation for the increased vulnerability of rIFN-gamma-activated macrophages and the greater resistance of LPS-activated macrophages, relative to medium-cultured macrophages, to photodynamic killing with BPD is uncertain.(ABSTRACT TRUNCATED AT 250 WORDS)

Inter-relationships between visual lobe dimensions, search times and eye movement parameters for a competition search task.

The inter-relationships between visual lobe dimensions, search times and eye movement parameters were investigated for a competition search task with 18 subjects. The subjects had previously participated in a visual lobe measurement and two search performance experiments using the same set of stimulus material. Using an Eye Mark Recorder, the average search times of subjects were found to be comparatively longer than for previous search tasks not using the recorder, and relative search performance ranks of the subjects were altered. The method of stimulus presentation and the search area were different in this experiment, so we cannot be certain as to which factor or factors caused the lengthening of search times and changes in ranking. All subjects achieved a 100% hit rate with no mis-classification of targets. A very high correlation coefficient was obtained for cumulative detection percentage and pooled search times when fitted to a random search model. Average search times were found to be significantly correlated with fixation durations as well as with the number of fixations. Contrary to expectation, none of the visual lobe dimensions was found to correlate significantly with average search times nor with eye movement parameters.

Thyroid hormone, L-T3, stimulates the expression of the angiotensinogen gene in cultured opossum kidney (OK) cells.

Angiotensinogen (ANG) messenger RNA is expressed in opossum kidney (OK) proximal tubular cells. To examine whether thyroid hormone, L-T3, could stimulate the expression of the ANG gene in OK proximal tubular cells, fusion genes, consisting of various lengths of the 5'-flanking region of the rat angiotensinogen gene linked to a human growth hormone reporter gene, were constructed and introduced into OK cells. As a negative control, they were introduced into a nonkidney cell line, a human choriocarcinoma cell line (JEG-3). The level of the expression of fusion genes in these cells were determined by the level of immunoreactive human growth hormone secreted into the culture medium. The expression of ANG-growth hormone (ANG-GH) fusion genes pOGH (ANG N-1498/+18), pOGH (ANG N-688/+18), pOGH (ANG N-110/+18), pOGH (ANG N-53/+18), and pOGH (ANG N-35/+18) was 226-, 4.5-, 1.0-, 12-, and 2.5-fold higher than promoterless pOGH in the expression of growth hormone activity in OK cells. No significant expression of any of these ANG-GH fusion genes over the promoterless pOGH was observed in JEG-3 cells. The addition of L-T3 stimulates the expression of pOGH (ANG N-1498/+18) in a dose-dependent manner with a maximal and half-maximal effect at 10(-7) M and at 10(-8) to 10(-9) M, respectively. Thyroid hormone (10(-7) M) also stimulates the expression of pOGH (ANG N-688/+18) but not pOGH (ANG N-110/+18), pOGH (ANG N-53/+18), or pOGH (ANG N-35/+18).(ABSTRACT TRUNCATED AT 250 WORDS)

Molecular cloning and expression of the rat angiotensinogen gene.

To identify tissue- and hormonal-specific DNA control cis-elements in the rat gene, we have constructed fusion genes consisting of various lengths of the 5'-flanking region of the rat angiotensinogen gene linked to a human growth hormone (hGH) reporter gene and have introduced them into a subclone of rat pancreatic islet tumor cell line (1056A) which expresses the highest level of angiotensinogen mRNA. As a negative control, we have also introduced them into a human choriocarcinoma cell line (JEG-3), which does not express the endogenous angiotensinogen gene. The level of the expression of these fusion genes in these cells was determined by the level of immunoreactive hGH secreted into the culture medium. The expression of angiotensinogen-growth hormone (ANG-GH) fusion genes, pOGH (ANG N-1498/+18), pOGH (ANG N-688/+18), pOGH (ANG N-110/+18), pOGH (ANG N-53/+18), and pOGH (ANG N-35/+18) was 1.0, 1.8, 1.5, 12.0 and 3.0-fold higher, respectively, than the promoterless growth hormone expression vector (pOGH). The addition of dexamethasone (10(-6) M), aldosterone (10(-5) M), and thyroid hormone, L-T3 (10(-7) M), stimulated the expression of pOGH (ANG N-1498/+18) by 4.0-, 2.5-, and 2.0-fold above the control level, respectively. Combination of dexamethasone (10(-6) M), L-T3 (10(-7) M), and ethinyl-estradiol (10(-6) M) stimulated the expression of the pOGH (ANG N-1498/+18) to greater than 10-fold over the control. Ethinyl-estradiol (10(-6) M) or progesterone (10(-6) M) alone had no effect on the expression of the pOGH (ANG N-1498/+18).(ABSTRACT TRUNCATED AT 250 WORDS)

Indolent nonseminomatous germ cell tumor of testis. Prolonged survival of patient with persistent metastatic disease.

The complete response rate of disseminated nonseminomatous germ cell tumors (NSGCT) of the testes with current aggressive chemotherapy and surgical resection of residual disease is between 70 and 80 per cent. Those patients who do not attain complete response tend to have short survivals. A case is presented of a forty-one-year-old white man who has had nearly continuous evidence of metastatic embryonal carcinoma for more than eleven years. Although NSGCTs are characterized by rapid proliferation, early metastasis, high response rate to chemotherapy, and rapid death if uncontrolled, this case demonstrates an indolent form of disease with poor response to chemotherapy and yet prolonged survival in spite of uncontrolled disease. This is the first reported case of indolent metastatic germ cell neoplasm with survival of more than ten years.
