As-needed psychotropic medications: prevalence, indications, and results.

1. Approximately 50% of inpatients are receiving p.r.n. psychotropic medications. 2. The most prevalent reasons reported in the literature for giving p.r.n. medications were to prevent/contain agitation, physical aggression, disruptive behavior in children, and aggressive behavior in children. 3. Nurses are responsible for providing a safe milieu; nurses must have the option of giving p.r.n. medications to protect patients and staff members.

Computer simulations as a measure of nursing students' decision-making skills.

Use of computer simulations was studied as a strategy to evaluate clinical decision-making skills of 64 senior baccalaureate nursing students who were randomly assigned to experimental and control groups. All students completed Kolb's Learning Style Inventory, a pre- and posttest that was a computer assisted instruction (CAI) clinical simulation specific to the student's clinical placement, and a 36 item attitudinal and evaluative questionnaire developed for the study. The experimental group complete three additional simulations over a 7 week period. Change scores from pre- and posttests were analyzed to determine: 1) if students completing a CAI series demonstrated higher achievement on information-gathering and decision-making skills than students who did not complete the series; 2) if there was a correlation between achievement and clinical grades; and 3) if there was a relationship between achievement and a particular learning style. Although all students demonstrated that significant learning had occurred during the semester, there was no difference between the students completing the full CAI series and those who did not. Students who made significant improvements in decision-making scores also earned higher clinical grades. Success on the CAI series was found to be independent of learning style. Most students expressed positive attitudes about CAI.

Psychiatric consultation. Improving the work environment.

1. The consultant initially must negotiate with the leader to clarify the need for and purpose of consultation. 2. Confidentiality is maintained by sharing group data without revealing identities of individual nurses. 3. Recommendations can be given as options from which the group can choose. 4. Consultation is a process using skills of establishing rapport, interviewing, extrapolating themes, and giving feedback.

Role perceptions of divorcing parents.

Using interview data from a convenience sample of 101 divorcing parents, we examined the gender roles of parents during the transition from marriage to divorce. We found that the women, who most often initiated divorce, were changing their roles and that this was related to marital dissatisfactions. Factor analysis indicated that real differences existed in how mothers and fathers perceived the co-parental relationship. Mothers saw the relationship as supportive at times and conflictual at times. Fathers tended to view the relationship as helpful only if it did not contain conflict. Improved communication in the coparental relationship after divorce seemed to be related to changes in traditional female and male roles.

A community-based program for emotionally disturbed children and youth.

The children and youth program is a multidisciplinary multi-agency project to train professionals to provide community-based services for emotionally disturbed children. A coordinating committee provides the central structure to process referrals and establish treatment teams. Nursing has played a significant role in developing this project and in providing services such as crisis intervention, in-home family therapy/family education, and team collaboration/case management.
