Psychosomatic consultation-liaison service in a medical out-patient department. Experience with a random sample of patients.

A sample of 103 patients was chosen at random from those attending the Hamburg University medical out-patient clinic. As well as undergoing the usual diagnostic procedures, these patients were interviewed to establish whether psychosomatic factors were also involved. The results show that a high percentage of the patients was willing to accept that psychological and/or social factors played a role in their illness. There was, however, a significant negative correlation between this willingness and the length of time the illness had persisted. In 50% of the patients the clinical diagnostic procedures revealed no signs of a pathological organic condition. Approximately one quarter of the patients were suffering from psychosomatic disorders in the strict sense of the word. Almost a third of the patients received psychotherapeutic help after the diagnostic procedures had been completed. The vital importance of cooperation from the outset between medical clinicians and psychotherapists is discussed in the light of empirical findings on iatrogenic chronification and its financial and personal implications for the medical services and their patients.

[Stress-induced haemorrhages in intensive care patients (author's transl)].

An analysis of the case material of the intensive care unit of the Department of Surgery showed haemorrhages from acute gastro-duodenal lesions in 8% of the cases. Control of the bleeding by therapeutic lavage with suspensions of antacids was the more effective the higher the frequency of application. Prophylactic lavage with antiacids in patients liable to develop gastric bleeding succeeded in reducing the incidence of this complication to 1%. Prophylactic administration of Cimetidine had the same effect.

[Some somatic and psychological predictors of psychopathological disorders after cardiac surgery (author's transl)].

Some new predictors of postoperative psychosis in open-heart surgery have been identified in a multifactorial study of somatic and psychological factors: Somatic predictors are lower body weight, higher blood levels of urea nitrogen and creatinine, preoperative use of tranquilizers, especially in patients with double valve replacement or congenital heart disease and preoperative cerebral embolism in mitral valve disease. Bypass-time has not proved to be a valid predictor. Psychological predictors are distress provoked by severity and duration of illness, repeated delay of surgery as well as the manner of psychological coping with the illness.

[A new method for sampling and sample application in the ultra-trace of air by gas chromatography-mass spectrometry (author's transl)].

A new method is described for sampling and sample application in the ultra-trace analysis of air by gas chromatography-mass spectrometry. In this new technique, a preseparation occurs on the gas chromatographic column during sample application, since the atmospheric constituents that are co-condensed during sample application we call this method "Druckaufgabe" (pressure application). The method is not only of theoretical value. It has a potentially wide application to occupational and environmental problems.

[Seroepidemiological studies related to the association of genital herpes to cervical cancer (author's transl)].

Blood sera of 290 patients were examined for the presence of antibodies against herpes simplex virus type 1 and type 2 (HSV-1 and HSV-2): 1. lues - seronegative individuals, 2. patients with specific lues antibodies, and 3. patients with so-called carcinoma in situ and invasive carcinoma of the uterine cervix. The group of syphilis-seropositive individuals served as control for the group with cervical lesions, because in both groups certain socioepidemiologic factors (promiscuity, early sexual intercourse) appear to be prevalent. The average value for HSV-2-antibodies was significantly higher in patients with cervical lesions than that in lues-seropositive individuals. Also, the average value was higher in the lues-seropositive group than that in the lues-seronegative group. The frequency of HSV-2-antibodies was highest in the group of patients with so-called carcinoma in situ and invasive carcinoma, less in the lues-seropositive group, and even more less in the lues-seronegative group. These results make it unlikely that the connection between cervical carcinoma and HSV-2-infection is a common or "nonsense"-correlation. They suggest a more narrow correlation between both diseases, but leave open whether it consists of either a causal relationship between herpes infection and the development of cervical carcinoma, or a predilection for herpes infection in the cervical region by the carcinomatous tissue.

[Changes of the family in the 20th century].

This article deals with the changes in socialisation patterns and adaptation to the conditions imposed by society, in internal familiar structures, and in the general attitude to life, that took place in the course of our present century. The empirical basis of this study was supplied by an investigation conducted with students of medicine representing three generations, i.e. persons born in 1905, 1934 and 1962. Against the background of the sociocultural and economic developments that took place in the Federal Republic of Germany, emphasis is on the following points: 1. The father of the family has been deprived of his power, and women have developed a new self-understanding; 2. the generation barriers are no longer impenetrable; 3. the family as such has lost a great deal of its functional importance due to the institutionalisation of care and welfare aspects.

[Psychosomatic-psychosocial need and care status in an acute-care hospital--results of a survey of internal medicine medical staff].

A questionnaire submitted in 1986 to ward physicians in internal medicine in 9 out of 11 public hospitals (eight General Hospitals and the University Hospital) in the city of Hamburg documents their remarkable awareness of psychosocial problems. Noticeable, however, is the comparably low assessment of the prevalence of psychosomatic diseases. The desire for assistance through a psychosomatic consultation service is much greater than actual rates of request. The status of psychosomatic care is judged to be very unsatisfactory. The collected data and the broad agreement to integrate psychosomatic forms of organization into the care of medical in-patients lead to the conclusion that there is an encouragingly great readiness to co-operate. Considering the very different experiences found in already existing models, the results indicate the necessity of a more in-depth examination of the process of co-operation between somatics and psychosomatics.
