Hemolytic-uremic syndrome with involvement of basal ganglia and cerebellum.

Hemolytic-uremic syndrome is a microangiopathy often associated with neurologic symptoms. Several patients with persistent lesions in cerebrum and basal ganglia have been reported. We present two children with bilateral basal ganglia and additional unilateral cerebellar lesions in magnetic resonance imaging. These resolved completely in one child. In the other child there were still residuals after 11 weeks. The neurologic symptoms of both improved after several therapeutic plasma exchanges and disappeared after months.

99mTc-ENS: a new radiopharmaceutical for aerosol lung scintigraphy. Comparison between different freeze-dried formulations.

UNLABELLED: Exogenous natural surfactant (ENS) labeled with 99mTc(99mTc-ENS) is a new radiopharmaceutical for pulmonary aerosol scintigraphy. In this study, different freeze-dried formulations were evaluated to develop a suitable and long-storage method for the ENS, the nonradioactive precursor of this radiopharmaceutical. METHODS: Two freeze-dried formulations were evaluated: the sterile ENS suspension-stannous chloride altogether lyophilized (chlorlioENS) and the lyophilized sterile ENS suspension with the addition of stannous chloride as a solid drug (lioENS). These precursors were stored at room temperature for 3 mo and then labeled with 99mTc. For comparative purposes, the sterile ENS suspension with the addition of stannous chloride labeled with 99mTc(99mTc-chlorENS) was also studied. The quality controls for each radiopharmaceutical were performed by an ascending paper chromatography to determine the labeling yield percentages. The study was performed in 30 female Sprague Dawley rats, which inhaled each radiopharmaceutical by nebulization. Twenty-five minutes after the aerosol inhalation, the animals were killed to extract their organs and measure their activity in a gamma spectrometer. The data are given as the percentage of activity concentration (C%) for each organ. RESULTS: The physicochemical properties of lioENS were adequate for a freeze-dried product. The labeling yields for 99mTc-lioENS and for 99mTc-chlorENS were always greater than 95% even after nebulization. The results of the biologic distribution studies showed that the activity concentration found in lungs for these radiopharmaceuticals were 95.7% +/- 2.6% and 96.7% +/- 2.6% respectively, results that do not differ statistically. On the other hand, the activity concentration found in lungs for the 99mTc-chlorlioENS (31.3% +/- 11.1%) and its labeling yield percentages (<10%) are statistically different (P < 0.05) from the results obtained with the two radiopharmaceuticals mentioned above. CONCLUSION: Taking into account the lioENS physicochemical properties, its long shelf life and that 99mTc-lioENS shows the same radiochemical and radiopharmacological behavior of the 99mTc-chlorENS, it can be concluded that the 99mTc-lioENS can be used for aerosol lung scintigraphy.

[Zinc: concepts on an essential micronutrient].

It is now well known that zinc is an essential micronutrient. Even though much information is available, there are many points that remain to be studied. zinc is absorbed by the intestine and transported in the plasma by albumin, forming a small exchangeable pool, which is rapidly exhausted even in cases of mild deficiency. zinc is essential for the activity of about 70 enzymes among other functions. Therefore, zinc deficiency produces a great number of clinical disorders, the symptoms of which may range between mild up to serious dysfunctions. These problems are corrected by a dietary supplementation with zinc. The lack of a reliable method to determine the zinc nutritional status reflects the problem to estimate the metal recommended dietary allowances, which are difficulty met specially by children, old persons and pregnant women which constitute the main risk groups. Although our knowledge concerning zinc toxicity is scarce, it is well known that the amounts of zinc that produces toxic effects are much higher than those that are contained in regular diets as well as in diets supplemented with this metal. Therefore, the need of the development of effective strategies, like food fortification with proper zinc compounds, appears as an attractive alternative in order to prevent and/or correct the deficiency of this vital element and to improve the health and the life quality of the whole population.

Oxidative damage, plasma antioxidant capacity, and glucemic control in elderly NIDDM patients.

A study of oxidative damage was made in elderly noninsulin-dependent diabetes mellitus (NIDDM) patients. A statistically significant increase in glucose and fructosamine was found in fasting NIDDM patients, as well as an increase in the oxidation induced by tert-butyl hydroperoxide. The Total Reactive Antioxidant Potential (TRAP) of the plasma was much reduced (p < .02) and the uricemia was unchanged. The erythrocytes of diabetic patients show greater basal oxidation products (p < .05), and the susceptibility of the diabetic erythrocytes to oxidation injury was also shown to increase in the oxidation induced by t-BOOH (p < .05). Linear regression studies showed that TRAP was associated directly with uric acid (p < .05) and inversely with fructosamine and with glucose (p < .03 and p < .05 respectively) in patients with NIDDM, but not in the controls. The levels of fructosamine were found to be related to the basal damage of the red blood cells (direct correlation, p < .001). This study suggest an useful approach to diabetic oxidative stress for clinical settings.

A spectrophotometric method for the determination of hydroperoxides in liposomes.

A modification of the Asakawa-Matsushita iodometric assay method for the determination of the content of lipid hydroperoxides was developed which permits the simultaneous processing of many samples of high lipid content. The method has the advantages of simplicity as well as good reproducibility, so it is not necessary to process standards with each determination. Our technique exceeds the sensitivity attained with other spectrophotometric determinations reported in the literature. The method requires the total elimination of water from the samples, and this was accomplished using an azeotropic mixture of ethanol:water of 96:4. The results obtained with liposomes indicate that the method is applicable to biological material limited to small volume samples, ranging 5-50 microliters. We want to emphasize that this method permits the study of the peroxidation process as function of time.

[The pulmonary surfactant system: physiology, pathologies associated with its alteration and exogenous administration as therapeutic and diagnostic agent].

Pulmonary surfactant is a lipoproteic mixture synthesized and secreted by alveolar type II cells. Its principal property is to reduce the surface tension by lining on the alveolar surface. Surfactant deficiency is the major factor responsible for the respiratory distress syndrome of the newborn (RDS) and the adult respiratory distress syndrome (ARDS). Since 1980, the exogenous administration of surfactant for the treatment of these syndromes is being studied. In this work the exogenous surfactant preparations, the delivery techniques and the dosing schedule is discussed. The utilization of the exogenous natural surfactant (ENS) as precursor of a radiopharmaceutical labeled with 99mTc (99mTc-ENS) for aerial lung scintigraphy is also discussed.

[Outcome after vitrectomy in rhegmatogenous retinal detachment and dense vitreous opacities].

BACKGROUND: In some cases of rhegmatogenous retinal detachment dense vitreous opacities can delay the diagnosis and prevent a scleral buckling procedure. In these cases a primary vitrectomy is necessary. These results of vitrectomy are of current interest, because intra- and postoperative complications are comparable to the group of patients with retinal detachment and clear media, in which primary vitrectomy is discussed today as an alternative method. PATIENTS AND METHODS: After redetachment and trauma were excluded, we reviewed the charts of 40 patients (40 eyes), who underwent vitrectomy and gas endotamponade for rhegmatogenous retinal detachment and dense vitreous opacities. In 31 cases vitreous hemorrhage and in 9 cases other dense non inflammatory vitreous opacities were present. Intraoperatively detected PVR (1), giant tear (1) and retinal holes located at the posterior pole (1) were excluded. The remaining 37 eyes were examined retrospectively. The median follow-up time was 12 months. RESULTS: In 32 of 37 eyes the retina was reattached after the first vitrectomy (86.5%). In 5 eyes a redetachment occurred, in 3 of them caused by PVR. After revitrectomy successful reattachment could be achieved in all cases. As intraoperative complications iatrogenic retinal break formation could be observed in 3 cases and lens-touch in 1 case. Postoperatively secondary tears in attached retina developed in 2 cases, in 1 case an arterial branch occlusion and in 1 case a Macular-Pucker. During the follow-up period a cataract developed in 12 of the 21 phacic patients. CONCLUSIONS: The reattachment rate of primary vitrectomy in eyes with retinal detachment and associated dense vitreous opacities is slightly lower compared to the rate of scleral buckling procedures in cases of retinal detachment with clear media. Comparable good or even better results may be expected for primary vitrectomy in cases of retinal detachment without vitreous opacities. Since the risk of intra- and postoperative complications in our group of patients is small it is promising also for primary vitrectomy in cases with good view on the fundus.

Differential effect of purified spruce chitinases and beta-1,3-glucanases on the activity of elicitors from ectomycorrhizal fungi.

Two chitinases (EC 3.2.1.14) and two beta-1,3-glucanases (EC 3.2.1.39) were purified from the culture medium of spruce (Picea abines [L.] Karst.) cells to study their role in modifying elicitors, cell walls, growth, and hyphal morphology of ectomycorrhizal fungi. The 36-kD class I chitinase (isoelectric point [pl] 8.0) and the 28-kD chitinase (pl 8.7) decreased the activity of elicitor preparations from Hebeloma crustuliniforme (Bull. ex Fries.) Quél., Amanita muscaria (L.) Pers., and Suillus variegatus (Sw.: Fr.) O.K., as demonstrated by using the elicitor-induced extracellular alkalinization in spruce cells as a test system. In addition, chitinases released monomeric products from the walls of these ectomycorrhizal fungi. The beta-1,3-glucanases (35 kD, pl 3.7 and 3.9), in contrast, had little influence on the activity of the fungal elicitors and released only from walls of A. muscaria some polymeric products. Furthermore, chitinases alone and in combination with beta-1,3-glucanases had no effect on the growth and morphology of the hyphae. Thus, it is suggested that apoplastic chitinases in the root cortex destroy elicitors from the ectomycorrhizal fungi without damaging the fungus. By this mechanism the host plant could attenuate the elicitor signal and adjust its own defense reactions to a level allowing symbiotic interaction.

Properties of a blue-light-absorbing photoreceptor kinase localized in the plasma membrane of the coleoptile tip region.

The blue-light-sensing apical part of coleoptiles of grasses is responsible for the first positive phototropic bending reaction of this organ. The photoreceptor responsible has been shown to be localized to the plasma membrane (PM) of this tip region. An approximately 100-kDa protein moiety of this receptor is rapidly phosphorylated upon irradiation. Properties of this protein kinase reaction were studied in vitro by using PMs from the maize (Zea mays L.) coleoptile tip region; (i) The substrate for the blue-light-triggered phosphorylation of the 100-kDa protein was found to be ATP as well as GTP. However, the affinity of the involved protein kinase for the substrate GTP was lower than for ATP. (ii) Experiments were undertaken to find out whether a photoreceptor moiety acts as an autophosphorylating protein kinase or whether the photoreceptor protein, when activated by light, becomes the target of an extrinsic protein kinase. Two studied extrinsic protein kinases (50 and 55 kDa) of the coleoptile tip were found not to be involved in the light-dependent protein phosphorylation. The degree of phosphorylation of the 100-kDa protein on isolated plasma membranes upon irradiation at 0 degrees C was scarcely different from a reaction at 30 degrees C, in contrast to the background protein phosphorylations which decreased with decreasing temperature. This result points to an autophosphorylation mechanism at the receptor. (iii) In mixing experiments, solubilized membranes from maize coleoptiles were irradiated and added to unirradiated membrane proteins from pea (Pisum sativum L.) epicotyls followed by addition of [gamma-32P]ATP. Unirradiated proteins from pea were not phosphorylated by light-activated (autophosphorylatable) maize protein kinases. (iv) It is suggested that the blue-light-sensitive photoreceptor localized to the PM of the phototropically active tip region of coleoptiles has an autophosphorylatable kinase domain which is able to use ATP or GTP as substrate.

A major isoform of the maize plasma membrane H(+)-ATPase: characterization and induction by auxin in coleoptiles.

The plasma membrane (PM) H(+)-ATPase has been proposed to play important transport and regulatory roles in plant physiology, including its participation in auxin-induced acidification in coleoptile segments. This enzyme is encoded by a family of genes differing in tissue distribution, regulation, and expression level. A major expressed isoform of the maize PM H(+)-ATPase (MHA2) has been characterized. RNA gel blot analysis indicated that MHA2 is expressed in all maize organs, with highest levels being in the roots. In situ hybridization of sections from maize seedlings indicated enriched expression of MHA2 in stomatal guard cells, phloem cells, and root epidermal cells. MHA2 mRNA was induced threefold when nonvascular parts of the coleoptile segments were treated with auxin. This induction correlates with auxin-triggered proton extrusion by the same part of the segments. The PM H(+)-ATPase in the vascular bundies does not contribute significantly to auxin-induced acidification, is not regulated by auxin, and masks the auxin effect in extracts of whole coleoptile segments. We conclude that auxin-induced acidification in coleoptile segments most often occurs in the nonvascular tissue and is mediated, at least in part, by increased levels of MHA2.

FAD is a further essential cofactor of the NAD(P)H and O2-dependent zeaxanthin-epoxidase.

In chloroplasts of plants the xanthophyll cycle is suggested to function as a protection mechanism against photodamage. Two enzymes catalyze this cycle. One of them, violaxanthin de-epoxidase, transforms violaxanthin (Vio) to zeaxanthin (Zea) via antheraxanthin (Anth) and is bound to the lumenal surface of the thylakoid vesicles, when being in its active state. The other enzyme, Zea-epoxidase, is responsible for the backward reaction (Zea-->Anth-->Vio) and is active at the stromal side of the thylakoid. For the epoxidation of Zea this enzyme requires NAD(P)H and O2 as cosubstrates. Using isolated thylakoid membranes we found that FAD enhances the epoxidase activity (decrease of apparent Km for NAD(P)H and two-fold increase of Vmax). The flavin functions as a third cofactor which is partially lost during the isolation procedure of thylakoids. Other flavins, such as FMN or riboflavin are without effect. The involvement of FAD in the enzymatic reaction is also demonstrated by the inhibitory action of diphenyleneiodoniumchloride (DPI) (IC50 = 2.3 microM), a compound that blocks the reoxidation of reduced flavins within enzymes. The Zea-epoxidase is a multi-component enzyme system which can be classified as FAD-containing, NAD(P)H- and O2-dependent monooxygenase that is able to epoxidize 3-hydroxy beta-ionone rings of xanthophylls in the 5,6 position.

In vitro allergy testing needs better standardization--test results from different laboratories lack comparability mostly due to missing effective standards.

Correct determination of specific IgE is important for the diagnosis of allergic diseases. Different detection systems are supposed to yield similar results of this standardized parameter. In order to investigate this assumption, 40 sera from allergic patients were sent in eight mailings to 23 laboratories over a 2-year period. Three IgE determinations in each serum had to be performed according to laboratory-specific methods. Whereas 'wrong' results were rare with any of the methods and the overall results to a large degree comparable, sensitivity and specificity seemed to vary considerably between antigens, methods, companies and laboratories. According to our observations, better standardization of test reagents and test methods by the suppliers is essential in order to harmonize the test results and to avoid wrong conclusions. At present, the diversity of techniques that are used for IgE determination leads to quite divergent results, and the allergist has to know that. And finally, the legislator is urged to better control these diagnostic tools, because wrong results may lead to life-threatening situations.

[50-year-old patient with dyspnea, dizziness, palpitations and brief sensory aphasia].

After a syncope, a 50 years old female patient was administered with an incomplete central facial paralysis on the left side combined with dyspnea, tachycardia, and hypotension as well as a systolic nurmur over the tricuspid valve. One week before, a vein stripping of the left long saphenous vein had been undertaken. By doppler-ultrasound, a thrombosis of the left tibial veins could be found as the cause of the pulmonary embolism. Additionally, the echocardiogram showed a relative tricuspid insufficiency and a floating atrial septum combined with an enlarged right atrium as an expression of the pulmonary hypertension. The incomplete central facial paralysis must be due to a paradox embolism in the cerebrovascular circulation as a result of an open foramen ovale. Under a therapy with high-dose heparin, all findings receded totally within one week.

[Whiplash trauma of the cervical spine from the neurosurgical, traumatologic and psychologic viewpoint].

Since the early 50's, whiplash injury has been a subject of intensive study in order to find out the genesis of this disease. Unfortunately, most of the studies were planned retrospectively and were based on inhomogeneous groups of patients. In our prospective study, we tried to keep the patient group homogeneous. 62 patients were examined neurologically and radiologically immediately after the accident. Furthermore, they underwent a psychological test, the so-called Freiburg personality profile. 6 months later the patients were checked again. On the basis on our findings, the suspected psychological influence has to be confirmed. Preexisting degenerative lesions of the cervical spine, also, are undoubtedly prognostic for longer-lasting symptoms.

Characterization of the H Translocating Adenosine Triphosphatase and Pyrophosphatase of Vacuolar Membranes Isolated by Means of a Perfusion Technique from Chara corallina.

Sealed tonoplast vesicles were isolated from single cells of Chara corallina with the aid of an intracellular perfusion technique in combination with a 3/10% Percoll two step gradient centrifugation. The isolated tonoplast fraction was free from plasmalemma and chloroplasts, and showed no activities of cytochrome c oxidase, and latent IDPase, but had about 10% of the NADH-cytochrome c reductase activity. The vesicles had both ATPase and PPase activities, which could be stimulated in the presence of 10 micromolar gramicidin by 170 and 130%, respectively, demonstrating the existence of sealed vesicles. Furthermore, ATP- and PPi-dependent H(+) pumping through the membrane into the vesicles was shown. Both ATPase and PPase had pH optima around pH 8.5. At the physiological pH, 7.3, they still had more than 80% of their maximal activities. Ammonium molybdate, azide, and vanadate had no or little effect on the activities of both enzymes or their associated H(+) pumping activities. N,N'-dicyclohexylcarbodiimide inhibited the ATPase strongly (I(50) = 20 micromolar) but the PPase only weakly. The ATPase was also more sensitive to N-ethylmaleimide than the PPase. 4,4'-Stilbenedisulfonic acid affected both enzyme activities and their associated H(+) pumping activities. This is in contrast to the H(+)-PPase of higher plants which is 4,4'-stilbenedisulfonic acid insensitive.
