Role of TNF-alpha and its 55 and 75 kDa receptors in bronchial hyperreactivity.

The pathophysiological role of the tumour necrosis factor (TNF) system was studied in adults (n=37) and children (n=43) non asthmatic offspring of asthmatic parents with and without bronchial hyperreactivity proved by methacholine airway challenge test. SerumTNFalpha and its soluble receptors (sTNF-R1 and R2) were determined by enzyme-linked immunosorbent assay (ELISA). Significantly elevated TNFalpha (adults: mean +/- SD=5.18 +/- 0.87 pg ml(-1), children: 5.08 +/- 1.78) vs. non-hyperreactives (adults: 4.12 +/- 0.43, P < 0.0001, children: 3.75 +/- 0.68, P=0.0084), sTNF-R1 (adults: 144 +/- 0.31 ng ml(-1), children: 1.30 +/- 0 25 vs. adults: 1.21 +/- 0.14, P=0.0305, children: 1.13+/-0.11 ng ml(-1), P=0.0042) and sTNF-R2 (adults: 0.85 +/- 0.40ng ml(-1), children: 0.70 +/- 0.46 vs. adults: 0.56 +/- 0.56 P=0.0084, children: 0.33 +/- 0.17, P=0.0048) and decreased sTNF-R1/R2 ratio (adults: mean +/- SD=0.96 +/- 0.73, children: 2.85 +/- 2.06 vs. adults: 4.82+/-3.40, P=0.0272, children: 4 42 +/- 2 30, P=0.0167) were measured in patients with bronchial hyperreactivityThe provocation doses of methacholine causing a 20% reduction (PD20) in forced expiratory volume in 1 sec (FEV1) were found to be in a significant negative linear correlation with TNFalpha sTNF-R1 and R2 levels in hyperreactive adults and with TNFalpha, sTNF-R2 in hyperreactive children. TNFalpha correlated significantly with its receptors both in hyperreactive adults and children and with the body mass index (BMI) values of adults. The TNF system may contribute to the pathophysiology of bronchial hyperreactivity Altered shedding of sTNF-R1 seems to occur in hyperreactive patients.

[Effect of loratadine in children with allergic rhinitis].

The authors examined 25 children with allergic rhinitis, who were sensitive to grass, weed and tree pollens. During the allergic season nasal lavage was performed then repeated after 4 weeks of loratadine treatment. Tryptase, IL-5, ECP, TNF-alpha and RANTES levels were measured from the nasal lavage fluid. Tryptase, IL-5, and ECP levels were decreased significantly while the decrease of RANTES and TNF-alpha levels was not significant. The authors emphasize that loratadine is an effective anti-inflammatory drug which affects the early and late phase of immediate hypersensitivity. In the moderate cases loratadine is enough to relieve the symptoms, while in the severe forms anti-allergic eye drops and nasal antihistamine or steroid spray has to be added.

[Changes in the symptoms of bronchial asthma and associated allergic disorders from childhood to adult age].

The aim of the study was to determine the prognosis of bronchial asthma. 145 adults (96 men, 49 women) with bronchial asthma during childhood were examined above age of 28 years (mean age was 37.6, SD: 5.9 years). The patients filled out questionnaires about the asthmatic and accompanying allergic symptoms in their childhood, age at 18 years and at present. They all were physically examined and prick tested with 12 inhalant allergens. 43% of the patients became symptom-free but 57% still had intermittent or persisting asthmatic symptoms while growing up. More patients had intermittent asthmatic day (59%) and night (67%) symptoms than persisting ones (41 and 33%). The accompanying allergic diseases in childhood did not definitely affect the prognosis of bronchial asthma. These are more frequent in the females, regarding before all the atopic skin disorders. At the age of 18, the occurrence of allergic rhinitis was more frequent than in childhood. The frequency of other allergic disorders did not change significantly. Among patients with asthmatic symptoms, moulds and cat hair allergies were more frequent than in the symptom-free group. The long-term prognosis of childhood bronchial asthma is relatively good, however only less than 50% of the patients has become symptom-free. In most of the adult patients the complaints are relatively mild. The indoor allergens may contribute to the occurrence of asthmatic symptoms.

[Levels of interleukin-4, interleukin-5, tryptase and eosinophil cationic protein of nasal lavage fluid in pollen allergic rhinitis].

IL-4, IL-5, tryptase and eosinophil cationic protein levels were measured in nasal lavage fluid from 15 pollen allergic rhinitis beyond pollen season. Allergy was proved by prick test. There were 15 non allergic children in the control group. Specific nasal allergen provocation was performed on the rhinitic group. Nasal lavage were done before, 1 and 12 hours after the provocation. Before the nasal provocation the ECP and IL-4 levels were significantly higher in the allergic group compared to the non allergic group. The levels of tryptase, ECP and IL-4 rose significantly after the provocation. The results reflect to the possibility of an activated immune status in allergic rhinitis even without the presence of the triggering pollens. After the specific provocation elevated tryptase levels were measured, referring to the activity of the early phase of the I. type hypersensitivity reaction, while the ECP and IL-5 elevation to its late phase. According to our examinations it can be said, that tryptase, ECP and IL-5 might be used to detect the activation of the early and late phases of the IgE mediated hypersensitive reaction.

Microcephaly with normal intelligence, and chorioretinopathy.

An 18-year-old girl had microcephaly without mental or neurological disabilities. She had hypoplastic mandible, long cup-shaped ears, bilateral incurved little fingers, bilateral retinal pigmentation, and scattered areas of depigmentation as well as a history of osteochondroma. However, genetic test for mutation analysis of exon 15 of the APC gene showed negative results. To the best of our knowledge, this is the third case to be reported with microcephaly, normal intelligence, and bilateral congenital hypertrophy of the retinal pigment epithelium (CHRPE).

Construction of poly-met DNA yeast hybrids for increased methionine content: technofunctional properties of the hybrid yeasts.

Methionine is a limiting essential amino acid in human nutrition, to overcome the possible overdosage and improve bioavailability methionine supply should be in protein bound form. So insertion a poly-met encoding DNA sequence results a more efficient solution in increasing methionine content of yeast. Poly-met DNA yeast hybrids were constructed of Saccharomyces cerevisiae CB89, an auxotrophic mutant strain. Synthetic DNA sequence encoding methionine polypeptide was inserted into the polylinker region of pVT-U 100 vector with 2 mu plasmid replicon. After transformation of E. coli HB101 cells the efficiency of the ligation and transformation was checked by digesting the minipreps. S. cerevisiae CB89 was transformed with vector-poly-met insert and with the plasmid vector only as well. Hybrid yeasts were selected on uracilless medium. PVT-U 100 can be used as vector for the expression of DNA sequence in S. cerevisiae. The vector harbours the promoter of ADC1 gene immediately downstream from the promoter lies a polylinker sequence comprising unique restriction enzyme sites for BamHI, HindIII, PvuII, SacI, Xhol. The polylinker sequence is followed by the transcriptional stop site and polyadenylation signal of ADC1 gene. Plasmid pVT-U 100 has selection markers for S. cerevisiae (URA3) and for E. coli (amp, per F). Results show that the poly-met DNA hybrids methionine content is influenced by the length of the insert. Fusion hybrids containing 600 bp oligo insert showed the best values. Distribution of methionine content in the protein subfractions of polymet DNA hybrid and parent strain CB89 was determined in dependence of glucose concentration and aeration intensity. The increase in synthesized methionine appeared in fractions 1 + 2 and residue. Technofunctional properties of parent strains and hybrids were compared for whole cells and cell wall (residue). Results demonstrate that enrichment in methionine in the cell wall fraction resulted improvement of emulsifying ability. Bioavailability of methionine content was better in DNA hybrid yeast than in parent strain and was the best when propagated in whey medium.

[The Angelman or "happy puppet" syndrome].

A case of Angelman's or "happy puppet" syndrome is described and detailed analysed, first time in our country. The literature is reviewed. The diagnostic criteria are presented. Evoked potential study and more than two years long EEG follow-up results are discussed. The unusual clinical feature of the described case is that epilepsy presented itself very early, in age of two weeks, by infantile spasms. It changed later to focal secunder generalised epilepsy. The EEG has been always abnormal during the follow-up, and the pattern has been changing by the age of the patient. On the other hand, the evoked potentials were all normals (BAEPs, VEP, SSEP CCT) "O"n the basis of the electrophysiological dichotomy the authors suggest a predominantly gray matter's disorder in Angelman's syndrome. They emphasize, that Angelman's syndrome has to be keep in evidence in the differential diagnosis of early infantile epilepsies. In the presented case the familial accumulation of movement's dyscoordinations, the dysmorphic features and subnormal intelligence of the brother support the possibility of the role of an autosomal recessive gene with different penetrance in the pathogenesis of Angelman's syndrome.

Provocation of convulsive activity in the EEG by promazine-HCl-induced sleep in children.

The authors report on the convulsive activity provoking effect of Promazine as shown on EEG during sleep. It has successfully been used in infants and children who do not tolerate sleep deprivation. Promazine had smaller effect on the EEG-pattern than other related compounds or barbiturates. The greatest efficacy could be reached in generalised epilepsy, but the method was also helpful in other epilepsies. It is emphasized that in cases of acute neurological symptoms of not epileptical origin the compound is helpful to exclude epilepsy since Promazine induced sleep was never associated with convulsive activity on EEG in these cases.

[Provoking effect of promazine on EEG in childhood].

The authors report on the sleep activation of the EEG-recordings by Promazine a Phenothiazine derivative that can be used also in babies and in those elder children who does not tolerate sleep deprivation. The EEG-s are much less altered then by other related compounds or by Barbiturates. The greatest efficacy could be reached in generalized epilepsies, but the method was helpful in other epilepsy forms too. It is emphasized that in no case of acute neurological symptoms of not epileptic origin showed the Promazine induced sleep a convulsive activity.

Investigation of functional properties of partially hydrolyzed proteins.

Hydrolyzates were prepared from soy protein and milk protein by treatment with Alcalase(R) 0.6 L and Neutrase(R) 0.5 L (NOVO INDUSTRI A/S, Bagsvaerd, Denmark). The pH-stat is a good method to control the change of degree of hydrolysis (DH) during enzymatic reactions and gives as well a good possibility to produce partially hydrolyzed products. In case of both investigated substrates we found a good correlation between the DH-values measured by pH-stat and TNBS-method. Determining the functional properties, we found that the emulsifying activity of hydrolyzates with low DH (below 10%) is significantly improved. At the same time the enzymatic treatment decreases the water-binding considerably but increases the velocity of water binding.

Effect of gamma-irradiation on F-2 and T-2 toxin production in corn and rice.

Fusarium graminearum and F. tricinctum were grown on moistened corn and rice. After inoculation the substrates were exposed to gamma-irradiation and growth rate together with mycotoxin production were measured. A delay in mycelium growth and an increase in F-2 and T-2 toxin production occurred after irradiation with 1 and 3 kGy. The maximum F-2 production was 10.7 mg/kg on rice at 3 kGy, whereas T-2 was 735 micrograms/kg on rice at 3 kGy. At 9 kGy neither growth nor toxin production could be detected in any inoculated corn and rice substrate.

Immunoelectrophoretic characterization of cellulolytic enzymes of fungal origin.

A crude cellulase preparation from Gliocladium sp. was immunochemically characterized and the components of cellulolytic activity were identified. Nineteen different antigenic components could be detected by crossed-isoelectric focusing immunoelectrophoresis. The 28 components separated by preparative isoelectric focusing from the above preparation were characterized by fused-rocket immunoelectrophoresis using polyspecific antisera. The main protein components in the enzyme complex were found to be endoglucanases with pI values between 3.2 and 5.3. Most of the antigenically active components proved to be glycoproteins.

The significance of the arginine and arginase of tears in experimentally-induced herpes simplex corneae.

Experimental corneal herpes is always accompanied by the accumulation of arginine, the substrate of arginase, in tears, ensuring the multiplication of the herpes hominis virus. The main source of the large amount of arginine is the desquamating corneal epithelium, since after the epithelium of the cornea is abraded the arginine content of the tears again equals that of healthy tears. The low arginase content of rabbit tears can be supplemented by arginase applied as eyedrops, and this results in the cure of the herpetic process.

[Tuberous sclerosis--early diagnosis and its significance in juvenile epilepsy].

Twenty-three cases of tuberous sclerosis, found in the ten years' material of the authors among 2900 epileptics between o-16 years of age, are presented. 11 patients were treated before the age of 3 years, all showing infantile spasms. The earliest, in most cases congenital extraneural signs were depigmented areas on the skin. They were absent only in one case. They allowed, together with the Westsyndrome to establish the diagnosis if tuberous sclerosis before the appearance of other characteristic symptoms. Sebaceous adenoma, generally accepted as pathognostic for tuberous sclerosis, presented itself in all 12 early cases later then the leukoderma and the epileptic fits. Tuberous sclerosis is one of the important causes of epilepsy in childhood, primarily in early childhood. In this period of life the West-syndrome is the most frequent type of epileptic manifestations of tuberous sclerosis
