Inter-institutional information exchange in healthcare.

In this paper the results of the standardization efforts and an evaluation study concerning electronic data interchange (EDI) performed by the 31 (Inter Institutional Information exchange)-project are described. In an earlier paper the results of a preliminary study were reported. EDI concerns communication between autonomous information systems and therefore needs standardization of the messages to be exchanged. In the 31-project ten different types of standard messages have been defined. The use of these messages for exchanging information between hospitals, GPs and pharmacies is described. It is concluded that relatively large time savings can be obtained when communicating electronically. It is also concluded that the electronic data interchange between hospitals and GPs can be performed without additional costs. The electronic communication of prescriptions between GPs and pharmacies does raise the costs of communication at the moment. The impact of EDI on the functioning of people in the organizations involved varies widely.

A data storage and retrieval system for clinical research.

A data storage and retrieval system is presented that can be used for clinical research studies. The system is designed in such a way that the programs are independent of the application. It can be used by non-computer experts via a simple query language. Since it is bulit up in a modular way extensions can be easily made. The system is implemented on a PDP 11/70 under FORTRAN.

Dose response in radiotherapy for glottic carcinoma.

All consecutive patients with glottic squamous cell carcinoma treated primarily by radiotherapy from 1965 through 1974 have been analyzed to reveal response of the primary tumor in relation to the dose. This study appears to indicate presence of dose response for all stages of the primary lesions (T) of the glottic carcinoma except for the smallest ones namely TIS, T1a and T1b. The total number of advanced tumors is small and selected. But they tend to indicate presence of dose response. Out of 15 selected patients with T3 and T4 glottic tumors, 5 (33%) had attained local control by primary radiotherapy; two had distant metastasis at onset of therapy, while 5 (33%) needed salvage surgery for control of the residual or recurrent disease. The ultimate success rate is 66% (10 out of 15) for the patients with advanced tumors treated with a conservative approach. An ongoing study indicates similar, if not better preliminary results with high dose radiotherapy.

Quantification of EEG variability.

This paper presents a method of segmenting the EEG based on the well-known power spectrum analysis. This procedure is applied to the EEG recordings of two normal subjects in order to determine the temporal EEG variability. These results are compared with a more classical approach.

Demonstration of segmentation techniques for EEG records.

In this paper three different techniques for segmenting EEG's are presented. The principles of these techniques (the Kalman filter approach, the power spectrum analysis and the texture matrix approach) are explained and the results obtained summarised. The segmentation is used in an interactive EEG interpretation system.

Oncologic research and computerized data retrieval.

A computer-based data retrieval system for clinical research in oncology is described. The system has been designed to enable busy clinicians to retrieve and analyze data directly without major involvement of a computer specialist. The system uses texts similar to those in daily practice. In most situations no code is printed. The other important feature of the system is its ability to analyze clinical data in the form of tables, frequency distribution, etc. It may also furnish actuarial characteristics of the total population or subpopulations. Mathematical functions are calculated easily. Amendments and updating are simple enabling follow-up data to be incorporated. The system is under use for about 2 years and appears to be a versatile tool in the field of clinical research.

Female gonadal exposure dose in routine diagnostic radiology.

Measurements of gonad doses received by female patients during routine roentgenology are presented. LiF dosemeters were used to measure the exposure doses in the vagina. With the aid of phantom studies, the ovary doses were calculated from the doses determined in patients. Their relationships to skin doses are shown. The results are tabulated. A comparison with results by other authors is given.

CaSO4 (Dy) thermoluminescent dosimeters for the determination of gonadal doses.

For the measurement of gonadal doses sensitive dosimeters are necessary. Since CaSO4 (Dy) thermoluminescent dosimeters are very sensitive the energy dependence has been measured as compared to LiF in the tube potential region between 50 and 150 kV. Moreover the output of the LiF dosimeters was compared with the results of ionization chamber measurements. Although the energy dependence of CaSO4 (Dy) is well-known the actual dependence on the potential appears to be within +/- 10% for moderately filtered beams in the region from 50 to 150 kV.
