[Surgical outcome after severe craniocerebral trauma in childhood and adulthood. A comparative study].

During a period of 15 years 1123 patients were operated on for severe head injury in our Department of Neurosurgery. We evaluated 936 patients (83%) on the basis of the Glasgow coma scale and the Glasgow outcome scale and allocated them into four groups by diagnosis and also grouped them by age. The 170 patients in the groups of children and adolescents (15%) were compared with the adults, and the features characterizing the causes of the accidents and the prognosis were analysed. Young patients and adults with epidural haematomas or perforating head injuries had a better prognosis than patients with acute subdural haematomas or brain contusions. The postoperative results after severe head injuries in children and adults were the same as in the group with an initial rating of 3-5 points and 9-15 points on the Glasgow coma scale. Only the group of children with 6-8 points on the Glasgow coma scale on admission had better results than the adults. The reason for this might be the greater plasticity of the brain in childhood.

[Subdural empyema].

The subdural empyema is a severe inflammatory cerebral affection and shows some diagnostic and therapeutic problems. Extracerebral infections especially in the rhinogenous and otogenic region represent the most frequent cause of the subdural empyema. Seven cases, treated between 1982 and 1986 in our department, are the base to discuss the problems of symptomatology, differential diagnosis and therapy. The treatment of choice is a sudden craniotomy in combination with a through-drainage and administration of antibiotics.

[Typical CT findings with the intrathecal infusion test in post-traumatic disorders of cerebrospinal fluid dynamics].

Simultaneous computer tomographic and cerebrospinal fluid dynamics examinations in patients with high-pressure hydrocephalus, normal pressure hydrocephalus and cerebral atrophic processes are presented and discussed. Exclusive CT and MR examinations do not allow a differentiation of patients with normal pressure hydrocephalus and patients with cerebral atrophy. In the simultaneous examinations the morphological substrate during the intrathecal infusion test is represented.

[The individual variability of pyramidal neurons in the neocortex of the rat].

In Golgi-Kopsch impregnated pyramid neurons of the neocortex (regio precentralis agranularis and area striata) of adult rats the individual variability was investigated. As parameters served the dendritic lengths, spine numbers and spine densities along the dendritic orders (basal and apical), within single dendritic fields, the apical and basal parts of the dendritic tree and of the whole neuron. The statistical tests gave the following results: Pyramid neurons in the regio precentralis agranularis are more variable than pyramid neurons in the area striata. Within the area striata the lamina III-pyramid neurons are more invariable than the lamina V-pyramid neurons. The basal dendrites are statistically more invariable than the apical dendrites. The dendritic lengths are less variable than the spine values. In general, the variability is different in particular subdivisions of the neurons analyzed as well as for the parameter chosen. The variability of neuronal structures depends from genetical factors and from a functional training. The knowledges about this variability are still insufficient. They are of considerable importance for the planning of experiments followed by statistical analyses.

[Manifestation of sarcoidosis of the optic nerve].

Under the suspicion of an optic nerve glioma a resection of the optic nerve tumour was carried out in a 58-year-old female patient with an amaurosis on the right side. The diagnosis of the sarcoidosis of the Nervus opticus was only established on the basis of the histological findings obtained in the resected optic nerve. Even after a period of observation of five years no further general pathologic signs of the sarcoidosis appeared, so that it could be assumed that this was the case of an isolated sarcoid affection of the optic nerve.

[Individual variability of lamina V pyramidal neurons in the cingulate area of the rat].

Lamina V-pyramidal neurons of regio precentralis agranularis in Golgi-Kopsch impregnated frontal series of the brains of four adult male rats were analyzed quantitatively in order to estimate the degree of individual variability. As parameters served the dendritic lengths, spine numbers and spine densities along the dendritic orders, within single dendritic fields and within the apical and basal parts of the dendritic tree or of the whole neuron. The significance of individual differences in these parameters between the animals was proven by the tests of Mann and Whitney or of Kruskal and Wallis. The variability turned out to be different in particular subdivisions of the neuron analyzed as well as for the parameter chosen. The basal single dendritic field showed a relative high degree of constancy in its data and the dendritic lengths were statistically more invariable than the spine values. The knowledge about high or less variable parameters in strictly selected neurons is of considerable importance for the planning of experiments followed by quantitative analyses of neurons. The application of the most suitable statistical method is also discussed.
