Methylprednisolone intravenously 1 day after surgery has sustained analgesic and opioid-sparing effects.

BACKGROUND: In previous studies on glucocorticoids for postoperative pain, the test drug has been given perioperatively, usually before measurement of baseline pain. In order to evaluate the time course and magnitude of the analgesic effect of a glucocorticoid in well-established postoperative pain, we compared methylprednisolone with ketorolac and placebo, after assessment of baseline pain on the first postoperative day. METHODS: This was a double-blind, single dose, randomized, parallel comparison of intravenous (i.v.) methylprednisolone 125 mg, ketorolac 30 mg as an active control, and placebo in 75 patients with moderate to severe pain 1 day after orthopaedic surgery. Outcome variables were pain intensity (0-100 VAS), pain relief (0-4 PAR) and rescue opioid consumption. RESULTS: Methylprednisolone was not significantly different from ketorolac and gave significantly lower pain intensity from 1 h (0-6 h, P < 0.02), and more pain relief 2-6 h after test drugs (P < 0.05) compared with placebo. After 24 h, pain intensity was lower in both active drug groups compared with placebo (methylprednisolone, P < 0.0001; ketorolac, P < 0.007). Number needed to treat (NNT) calculated from patients having more than at least 50% of maximum obtainable total pain relief during the first 6 h (>50%maxTOTPAR(6 h)) was 3.6 for methylprednisolone and 3.1 for ketorolac. Number needed to treat calculated from the percentage reporting at least 50% pain relief for at least 4 h (>50%PAR(4 h)) was 2.8 for both groups. Opioid consumption was significantly reduced for 72 h after methylprednisolone compared with ketorolac (P < 0.02) and placebo (P < 0.003). CONCLUSION: Methylprednisolone 125 mg i.v. 1 day after surgery gave similar early reduction of pain as i.v. ketorolac 30 mg. Less pain than placebo 24 h after methylprednisolone, and lower opioid consumption for 72 h compared with ketorolac and placebo indicate sustained analgesic effects of methylprednisolone.

Quality monitoring, standardized documentation and management with a computerized system in oncology.

Within the last years the prerequisite was prepared to develop a computerized tumor--patient documentation system including quality monitoring and oncological therapy recommendations for every day use. In medicine today, there is an increasing need for quality oriented low cost and transparent management--what is especially true in the field of oncology. The German Federal Authority of Health demands the documentation of all tumor disorders for the establishment of an cancer registry. For these reasons our study group established the program "OncoDoc" in cooperation with the laboratory for Artificial Intelligence of the University Bremen.

Atherosclerosis precursors in Finnish children and adolescents. XIV. Zinc and copper concentrations in deciduous teeth.

Zinc and copper concentrations were analysed from cariesfree deciduous teeth of 125 Finnish children, 6-7- and 9-10-year-olds, from three cities. Enamel and dentine were separated and prepared into pills which were analysed individually with the proton induced x-ray emission spectrometry (PIXE) method. The total mean (+/- SD) zinc concentrations for enamel and dentine were 143 +/- 31 and 133 +/- 36 ppm, respectively, and are in agreement with the values reported recently. The median copper concentration for enamel was 0.54 ppm with a range from 0.0 to 4.29 and for dentine 0.27 ppm with a range from 0.0 to 3.60 being among the lowest reported. The low copper values are interesting as to the possible association with the development of atherosclerosis. Geographical differences were noted between the zinc concentrations of the three cities, the children of Helsinki having significantly higher zinc values than the children of Kuopio and Oulu. No linear correlation was found between zinc and copper concentrations at the individual level.

Effect of maternal fluorine intake on breast milk fluorine content.

1. Breast milk samples were collected from mothers living in areas having an average of 0.2 and 1.7 microgram fluorine/ml in drinking water. The samples were analysed for their ionic and total F concentrations. 2. Ionic F was determined after microdiffusion by the fluoride-selective electrode. For total F determinations, milk samples were ashed with magnesium succinate fixative, after which the F concentration was determined as mentioned previously. 3. The median totals for F content of breast milk in the low- and high-F areas were 7.0 ng/ml (range 4.3-14.0 ng/ml, fifteen mothers) and 10.9 ng/ml (range 4.5-50.7 ng/ml, nine mothers). The median ionic F concentrations were 5.0 ng/ml (range 3.0-6.3 ng/ml, fifteen mothers) and 8.9 ng/ml (range less than 2.0-39.9 ng/ml, nine mothers) respectively. Both the total F (0.01 greater than P greater than 0.001) and ionic F (0.05 greater than P greater than 0.01) in breast milk were significantly higher in the high-F area compared to the low-F area. 4. At the ages of 1,2 and 3 months, the calculated average total daily F intakes of exclusively breast-fed infants were 4.8 and 7.3 microgram; 5.0 and 7.8 micrograms; 5.4 and 8.5 micrograms in the low- and high-F areas respectively.

Lead concentrations of enamel and dentine of deciduous teeth of children from two Finnish towns.

The proton-induced x-ray emission method was used to analyze lead concentrations in enamel and dentine of deciduous teeth of 105 6- and 9-yr-old children living in two Finnish towns. Helsinki, the capital city, was assumed to represent high lead exposure and Kuopio, a country town in the middle of Finland, low or moderate lead exposure. In all but two teeth the enamel contained higher lead concentrations than dentine. The individual enamel-dentine lead concentration ratio was not stable but varied greatly. The median dentine lead concentration for 9-yr-old children in Helsinki and Kuopio was 2.0 ppm and 2.9 ppm, respectively, and for 6-yr-old children was 2.5 ppm and 2.7 ppm, respectively. The enamel lead concentrations found in 9-yr-olds in Helsinki and Kuopio were 4.2 and 4.5 ppm, and in 6-yr-olds were 9.1 and 4.4 ppm, respectively. The results reveal low or moderate lead concentrations. Nevertheless, a twofold and statistically significant (P less than .01) increase was found in the enamel of Helsinki children born in 1974 compared with those born in 1971.
