Callus distraction of the midface in the severely atrophied maxilla--a case report.

OBJECTIVE: This report introduces the possibilities of callus distraction in the extremely atrophied, edentulous midface in a cleft lip and palate patient. INTERVENTION: After a subtotal Le Fort II osteotomy, tension wires were fixed to the zygomatic buttresses and frontal sinus walls by way of titanium miniplates and mesh and connected to a rigid external distractor. Then distraction of the whole midface (1 mm/d) was performed. RESULTS: Even in severe atrophy a distraction of the maxilla of 20 mm was possible. Stability has been shown for more than 5 months. CONCLUSIONS: Rigid external midfacial distraction may be used in difficult cases for the correction of sagittal discrepancies where conventional orthognathic surgery is likely to be insufficient. Further investigations will concentrate on the long-term outcome.

Obstructive sleep apnoea syndrome: results and conclusions of a principal component analysis.

A cephalometric analysis according to Hasund, supplemented by special obstructive sleep apnoea syndrome (OSAS) parameters, was performed on 169 patients who had been referred from the sleep laboratory. Statistical analysis showed a correlation between specific cephalometric landmarks including posterior airway space (PAS), a soft palate length, hyoid position and posterior growth development of the mandible and OSAS severity. A principal component analysis differentiated between four subgroups of OSAS patients: (1) orthognathic obese subjects; (2) patients with a long soft palate and low-positioned hyoid; (3) retrognathic patients with narrow PAS; and (4) prognathic ones. Lateral cephalometry is an important contribution to OSAS diagnostics and oral and maxillofacial therapy procedures.

[Comparison of miniplate and lag screw osteosynthesis in treatment of collum fractures].

The question whether- and if by which means-operative reconstruction of condylar process fractures is appropriate is still unresolved. This study was initiated to compare osteosynthesis of low dislocated and/or luxated condylar fractures by means of miniplates or lag screws according to Eckelt. 22 patients participated, 16 being treated with miniplate osteosynthesis, 6 with lag screws. Investigation concentrated on the parameters TMJ-function (side shift in dynamic and static occlusion, maximum interincisal distance, impairment of pro- and laterotrusion), radiologic post-operative fragment position, intra- and postoperative complications, and evaluation by the patients. Miniplate osteosynthesis showed better results leading in 80% to an unimpaired TMJ function compared to 50% of the lag screw group. Miniplate osteosynthesis was performed faster. No intra- or postoperative problems occurred in contrary to the lag screw, where one fracture of the screw intra- and one post-operatively were noted. Even if the numbers are too small for statistical calculations, trends are shown favouring miniplate osteosynthesis for operative reconstruction of condylar fractures.

[Severity of obstructive sleep apnea syndrome--correlation with cephalometric parameters].

In 132 patients with polysomnographic records suffering from OSAS a cephalometric analysis according to Hasund supplemented by special OSAS-parameters was performed. The statistical analysis showed a correlation between severity of OSAS and width of posterior airway space, length of the soft palate, hyoid position and posterior growth development of the mandible. A principal component analysis showed four groups: 1) prognathic patients, 2) retrognathic patients with narrow PAS, 3) patients with long soft palate and low standing hyoid, and 4) orthognatic obese subjects. Lateral cephalometry is an important contribution to OSAS-diagnostics and should be performed on all OSAS patients in the course of interdisciplinary cooperation.

Secondary correction of traumatogenic enophthalmos with auto- and alloplastic implants.

In 23 patients with traumatogenic enophthalmos and diplopia autologous (rib cartilage, iliac crest bone), heterologous (lyophilized dura), and alloplastic (Silastic, P.D.S., HA-blocks) material was implanted into the orbit in a late secondary intervention. The results were encouraging as for the improvement of the diplopia as well as the enophthalmos. If rib cartilage should not or cannot be used a hydroxyapatite ceramic block is the best alloplastic implant material due to its stability, inertia, and availability.

[Clinical studies on the influence of Ornipressin (POR 8) on the cardiovascular system in general anesthesia].

A clinical prospective study of 231 patients was conducted to examine the influence of Ornipressin (POR 8) on the cardiovascular system. The standard concentration of the vasoconstrictor was 0.2 i.u./ml. A control group of 34 patients received 0.9% NaCl solution. In less than 10% of all cases blood pressure or pulse frequency changed up to 30 mmHg or 30 bp. m. Cardiac arrhythmias without clinical relevance could be observed sporadically. There were significant differences to the control group only in terms of pulse rate under volatile anaesthetics, and changes in blood pressure during the first 10 minutes under neurolept analgesia and under combined anaesthesia.

[Midfacial and mandibular body fractures in children and adolescents].

1423 patients with maxillo-facial trauma were admitted for hospital treatment from 1980-1986. Midfacial and/or mandibular corpus fractures were present in 87 juvenile cases. The follow-up study of 64 patients of this group revealed some interesting points: 1. Fractures of the juvenile skull must be reduced exactly. There will be no "smoothing of edges" during the later growth period. 2. Teeth which had been left in the fracture line showed no disturbance during their eruption. 3. An enlargement of the zygomatic-alveolar crest was a frequent observation in zygoma fractures. 4. There is no need for regular follow-up examinations if the primary treatment has been successfully done.

Neutron radiotherapy in adenoidcystic carcinoma: preliminary experience at the Münster neutron facility.

In adenoidcystic carcinoma macroscopic residual disease after surgery, inoperable and recurrent tumors are characterized by relapse rates up to 70% if treated according to conventional treatment strategies including photon radiotherapy. By neutron therapy a tumor control rate of 70% has been reported. At the d,T-generator (14 MeV) in Münster nine patients with adenoidcystic carcinoma of the salivary glands in locally advanced stage have been treated since 1/86. A combined modality schedule (30 Gy photon/5 to 10 Gy neutrons) used in the beginning has been replaced by a definitive neutron therapy (15 Gy) with single fractions of 1.0 to 1.3 Gy in five to six weeks. Treatment planning included CT/MRI as well as computer assisted dose calculations based on a specific software for neutron therapy. The volume reduction by neutron radiotherapy was 82% for advanced disease. Continuous remission was achieved in seven of nine patients in a short follow-up period of medium 15 months (from six to 28 months). The two recurrences occurred at the field edge. Based on this preliminary experience and on a survey of the literature the main issues for neutron therapy in salivary glands tumors in particular adenoidcystic carcinomas are discussed: histology, stage, tumor size, location, surgery, treatment planning and delivery, dose, and side effects. A long term evaluation of neutron therapy within a controlled clinical multicenter trial is proposed.

Long-term results in treating squamous cell carcinoma of the lip, oral cavity, and oropharynx.

At the department of Maxillofacial Surgery of the University of Münster, squamous cell carcinomas of the lip, oral cavity, and oropharynx are treated according to the principle of surgical resection of the tumour and, if indicated, radical neck dissection. In cases of lymph node infiltration, additional radiotherapy is given. The data of 352 patients, treated within 15 years, were evaluated with regard to 5-year minimal survival rate and to the incidence of recurrences in correlation to the tumour stages and form of treatment.

[Bone regeneration after filling of extensive cysts by means of bone collagen type 1].

It could be demonstrated by experimental studies on dogs that demineralized bone and bone products have a good osteogenetic potential which gives them a superior rating over artificial materials as replacement of autogenous bone transplants. The results of a prospective clinical study on 52 patients with 56 large mandibular or maxillary cysts are presented showing that bone collagen matrix type I is a good product for the filling of bone defects. Full reossification of a cyst was reaches as early as 8 months postoperatively in 20 cases. 32 defects had become substantially smaller. These results are discussed and compared to other methods mentioned in the literature.

[Filling cysts with type 1 bone collagen].

The good properties of bovine bone collagen type 1 for filling of bony defects was demonstrated in 56 cysts of the alveolar bone in 52 patients. 49 cysts (approx. 87%) healed without irritation. 8 months postop. complete reossification of the defects had been achieved in 20 (approx. 36%) of the cysts. Further 32 defects (approx. 57%) were markedly reduced. The discussion will compare the advantages of bone collagen and other materials.

[Long-term experiences with large bridge-work after partial resection of the lower jaw].

In a period of 12 years 121 bridging plates were implanted for the reconstruction of madibular defects following radical tumour surgery. 60 (49.6%) of these implants had to be removed prematurely due to: 1. postoperative perforation of the plate through either skin or oral mucosa, 2. implant fracture, 3. infection of the graft bed, 4. postoperative dehiscence and remaining fistula. Inadequate soft tissue coverage of the bridging plate is discussed as the main reason for the above mentioned complications.

The adenoid cystic carcinoma. Special aspects of its growth and therapy.

In spite of its rare incidence the adenoid cystic carcinoma must be treated with respect because of its hidden malignant character. In 5 out of 21 patients recurrences occurred much longer than 5 years after the first operation. The extension of this tumour along branches of the trigeminal nerve could be well demonstrated in 7 of our patients where we were able to produce histological proof or radiographic evidence. The theory of tumour growth along the "tissue of least resistance" is illustrated by a special case report. As for the therapy, ultra-radical surgery has proved to be the most effective so far, whereas radio- or chemotherapy were only useful as palliative measures.
