[Accomplishments of tumor surgery in tumors of the stomach].

Accomplishments of gastric cancer surgery include (1) reduced operative risk, (2) surgical strategy based on histology and location of the carcinoma, (3) more operative radicalness (gastrectomy with extended lymph node dissection), (4) prolonged survival time and (5) adjuvant therapy (cytostasis, immunological treatment, intraoperative radiotherapy). In 1,070 patients the resection rate changed from 56% to 75%, the lethality rate of gastrectomy fell from 28% to 1.1% while the rate of gastrectomy increased from 21% to 68%. The stage of the tumor of operation influences the five-year-survival rate. As an additional treatment intraoperative radiotherapy has been introduced at our clinic.

Perioperative antibiotic prophylaxis in bile-duct interventions: results of two prospective randomized studies.

The success of perioperative prophylaxis in gallbladder surgery was examined in two prospective randomized studies with a total of 326 patients. Postoperative wound-healing impairments did not occur in any of the patients receiving a single preoperative application of an antibiotic with a high biliary elimination rate (ceftriaxone or apalcillin). 11% of a control group without prophylactic antibiotic application evidenced infectious wound-healing disturbances.

Antibiotic infection prophylaxis in gallbladder surgery--a prospective randomized study.

In the course of 1 year, 180 patients undergoing elective surgery for confirmed cholelithiasis were included in a prospective randomized study in which they either received 2 g of preoperatively applied ceftriaxon intravenously at the beginning of anesthesia or, as an alternative, no antibiotic at all. Infectious wound-healing disturbances occurred postoperatively in 11% in the control group and in no case in the prophylaxis group. The difference is statistically significant.

[Recurrent ulcer after stomach resection--its operative therapy with reference to etiology and pathogenesis].

Over a period of 12 years, 100 patients with a confirmed ulcer recurrence after prior gastric resection underwent a standardized surgical procedure. The mortality rate of the electively operated patients amounted to 1 per cent. The follow-up examination yielded satisfactory surgical results in over 80 per cent of the patients treated in this way. We consider the exclusively abdominal procedure suitable in cases of recurrent ulcer in order to ensure adequate diagnosis and therapy.

[Experimental liver arterialization following portacaval shunt - rehabilitation].

Comparative investigations were carried out on rats with healthy livers and rats with thioacetamide-cirrhotic livers on postoperative days 2 and 21 following end-to-side portacaval anastomosis alone and with additional (documented) flow-limited arterialization of the portal stump. After portacaval anastomosis alone, all the parameters investigated (biochemical, pharmacokinetic, and morphological) were impaired. By means of the additional flow-limited arterialization, the disadvantages can be greatly headed off, more clearly in healthy than in cirrhotic animals.

[Does flow-limited portal arterialization avoid the disadvantages of portacaval shunts in healthy and cirrhotic rats? (author's transl)].

The disadvantages of portacaval end-to-side shunt concerning hepatic function can be avoided by additional portal arterialization in healthy liver and cirrhotic rats. The maintenance of total liver perfusion is the cause of good RES clearance, nearly normal capacity of synthetic functions and normal biliary excretion. Flow and thereby pressure-adapted technique of arterialization is necessary.

[Swine liver ultrastructure following hypothermia and hyperbaric oxygenation preservation].

The livers of 20 pigs were preserved for up to 16 hours, applying hypothermia (4 degrees C) and hyperbaric oxygenation (3 ata). As a preparation solvent we used in one group the solution by Lie (17) which is rich in potassium and glucose, and in the other group the cryoprecipitated serum by Belzer (5). After 8 and 16 hours of preservation, the fine structure was examined light- and electronmicroscopically. After 8 hours of preservation, there were only little reversible changes in both groups. After 16 hours, both groups showed distinct changes of the endoplasmatic reticulum e. g. desorganisation, vesicular dilatation, and dissociation of the mural ribosomes. In addition, there were swellings of mitochondriae and Kupffer's cells. These changes were particularly distinct after preservation in cryoprecipitated serum. In this group a cellular depletion of glucogen was noticeable.
