Evaluation of interphase fluorescence in situ hybridization on direct hematological bone marrow smears.

Traditional cytogenetic analysis from bone marrow aspirates is time consuming and frequently suboptimal due to poor viability of cells in culture. Fluorescence in situ hybridization (FISH) with appropriate DNA probes is a potential alternative to routine cytogenetics. Our study examined the reliability of uptake of specific alpha satellite centromere probes from chromosome 18 (D18Z1) and X (DXZ1) and the Yq heterochromatin (pHY3.4) directly from routine hematological bone marrow smears. Altogether 34 separate hybridizations, performed on slides from 20 patients, were scored for fluorescence signals. Cells in interphase were examined with each probe using unstained slides. In addition Giemsa stained slides were destained and then used for interphase FISH. Between 412 and 631 cells were scored for the expected number of signals; 2 for the 18 centromere, 2 for the X centromere in females, one signal for the Yqh in males. The results showed the expected number of signals in 87-97% of cells with the 18 and X probes and 95-97% of cells with the Y probe. Interphase FISH is a reliable, reproducible technique for use on direct bone marrow smears.

[Cholecystitis in accident surgery].

Out of a large number of cholecystectomies four cases are reported. In some cases, the gallbladder was involved in the traumatic lesion itself but there are also cases, in which the cholecystitis developed independently of the primary lesion. Beside abdominal trauma which leads to a disruption of the organ wall we also see an inflammatory disease of the gallbladder after perforation during liver blind punction. In another case a patient with gall stone history developed acute cholecystitis after operative treatment of a collum femuris fracture. The fourth case shows acute cholecystitis which also arose after operative treatment of bone fracture in an acalculous bile vesicle. Acute cholecystitis after severe trauma not inflicting the organ itself is a severe, life threatening complication. One step to acute acalculous cholecystitis is the sludge formation in the gallbladder which can be recognized by ultrasonography. The pathogenetic mechanisms which lead to sludge phenomenon and later to acute cholecystitis are multifactorial. Some theories which try to explain this phenomenon are discussed. In each case the development of cholecystitis after trauma is a dangerous complication which urgently demands nonconservative treatment.

[The importance of mammography in the early diagnosis of breast cancer (author's transl)].

Mammography has been performed routinely preoperatively over the last 5 years on 278 patients with breast tumors. In 85 per cent the radiographic findings showed an exact correlation to histologic results. In 12 per cent there was only a partial correlation, in only 3 per cent the mammograpic findings could not be verified by the pathologist. The purpose of this paper is to emphasize that mammography should be considered as the prime diagnostic procedure in the diagnosis of breast disease. The examination of the breast should therefore always be followed by mammography, especially in women at risk and elderly patients. In this connection we would also like to stress the vital cooperation between the radiologist and surgeon both pre- and intraoperatively. and intraoperatively. In summary we would like to emphasize the importance of possibility to achieve a better survial rate in these patients.

[Complications of acupuncture (author's transl)].

During acupuncture in the chest region a right-sided pneumothorax developed in a 53-year-old woman which necessitated emergency admission to hospital. A reinflation of the lung could be achieved with two Bülau drains. The increasing frequency of acupuncture must remind one of the feasibility of lesions of internal organs including those of the abdomen. Complications due to insufficient disinfection of acupuncture needles may also arise.
