[Letters from Sigmund Freud to Joan Riviere (1921-1939)].

A series of letters that Freud wrote to Joan Riviere in the period from 12-8-1921 to 22-1-1939 has been transcribed, translated into French and published here for the first time in the original text. They were given by Joan Riviere to the Sigmund Freud Archives and deposited at the Library of Congress in Washington where access to them had been restricted. Through the efforts of Dr. Harold Blum, the present Executive Director of the Freud Archives, the restriction was lifted. The letters concern arrangements that Freud made for his analysis of Joan Riviere; his (and her) relationship to Ernest Jones; the translation into English and the publication of his works in England. They also concern Freud's reaction to the reception in England of Anna Freud's book on The Technique of Child Analysis (1927) and his view of Melanie Klein's approach to the theory and practice of child analysis.

[Prognostical factors of breast neoplasms without metastasis. Synthesis by a multifactoral analysis].

Multifactorial analysis by dichotomous split was applied to a consecutive series of 541 female breast cancer patients without detectable metastasis at onset of treatment. These patients were entirely treated in the same institution (Fondation Bergonié), in the years 1960-1974. This multifactorial analysis sets a hierarchical order among the different prognostic factors and defines groups of patients homogeneous for metastatic risks. The significant prognostic factors in these series are : number of malignant axillary nodes; location and size of tumour; state of adjacent skin; relations with thoracic wall; pre or post-menopausal status; inflammatory signs; thermographic and mammographic data.

[Unusual isotope scanning aspects of hot thyroid nodules: diagnostic problems].

The classical picture of development of the "hot" nodule, which takes into account at the same time fixing or non-fixing nature of healthy perinodular tissue and the clinical condition of the patient fails to explain certain biological and scintigraphic findings. The authors discuss problems related to scintigraphy which they encounter in the course of the examination. In 174 stimulation tests carried out, 12 resulted in hypofixation of the isotope in the nodule, scintigraphy being carried out in three patients using 99-mTcO4 spontaneous reactivation of healthy tissue was seen in 5 patients. In the light of the cases reported, it would appear that the scintigraphic picture of the "hot" nodule is more in favor of a duality between the latter and healthy tissue with respect to iodine than of a hypersensitivity to TSH. In the 12 inhibition studies carried out, 3 showed the persistence of a contralateral shadow. Finally, on five occasions histological examination of the operative specimen showed the presence of neoplastic change, in two cases within the "hot/ nodule.
