[The characteristics of gastroduodenal pathology in the workers of a plant for plastic construction materials].

Out of 162 workers exposed to chemicals in the manufacture of construction plastics an outpatient and inpatient examination revealed gastrointestinal problems in 84.3%, vegetative nervous disorders in 62.6% of the examinees. In a random sample of 47 workers high acid production occurred in 63.3% and erosions, ulcers, cicatrices in the stomach and duodenum in 45.2% of them. No HLA genetic predisposition to duodenal ulcer was reported. Histological findings on gastroduodenal mucosa biopsies were indicative of the dystrophy in the chief and lining cells of the gastric glands, epithelium and stroma with formation of the dark cells. The authors insist on consideration of the above facts in examination and treatment of workers exposed to chemicals at construction plastics plants.

[Ultrasonic study of the bile ducts in patients after cholecystectomy].

Ultrasonic examination (USE) of the bile tract was conducted in 287 patients with removed gallbladder. The choleretic test based on comparative appraisal of the lumen of the bile ducts before and after medication with pharmacological agents stimulating bile secretion was performed in 167 cases to increase the USE efficacy. USE allowed choledocholithiasis to be diagnosed in 24 of 30 patients and obstruction of the bile ducts in 60 of 67 patients. The sensitivity of USE in pneumobilia detection was appraised highly. Preference was given to radiological methods of examination of patients with external biliary fistulae and drains in the common biliary duct in comparison with USE. The authors believe USE to be the optimal means for primary diagnosis of biliary pathology in persons with removed gallbladder, which can be undertaken in out-patient conditions and which reduces the need for conducting invasive radio-endoscopic examination (endoscopic retrograde cholangiography) in such patients.

Cholecystectomy↗

[Removal of foreign bodies from the upper segment of the digestive tract using an endoscope].

The work presents data of the endoscopic diagnostics and treatment of 73 patients with foreign bodies in the upper part of the digestive tract (93 objects). High efficiency and little traumatism of removals of the foreign bodies from the upper parts of the digestive tract was achieved due to using a rational tactic of curative endoscopy consisting in using different models of modern fibroendoscopes and instrumental devices.

[A complex x-ray endoscopic study in complicated cholelithiasis].

Recurrent or residual choledocholithiasis was diagnosed in 202 patients with a history of cholecystectomy, in 148 cases this condition was combined with papillostenosis. Transfistula extraction of the concrements and endoscopic papillosphyncterotomy helped achieve cure of 186 (92%) patients without relaparotomy. The authors discuss the role of roentgenology in such combined x-ray and endoscopic diapedetic examinations and emphasize its role in identification of the diagnostic details that are so important for the choice of the optimal treatment strategy. They come to a conclusion that the results of treatment of complicated cholelithiasis by 'minor surgery' means depend not only on the manipulations and skills of the surgeon endoscopist, but on the full-value employment of roentgenology as well.
