[The analysis of adequacy of the treatment for acute otitis media and bacterial sinusitis in outpatient setting].

A retrospective study of adequacy of outpatient treatment for acute otitis media made analysis of 275 case histories of acute otitis media and 271 case histories of acute sinusitis. It was revealed that choice of antibacterial drugs was sometimes made without analysis of most possible pathogens, doses and regimens of pharmacotherapy are often not strictly adhered. The analysis of adequacy of the treatment of acute bacterial sinusitis and otitis in Moscow and 8 cities of Russia is also presented.

[Conditions of work and hearing in traffic policemen in Moscow].

Conditions of work, sound pressure levels on the city roads, direct and indirect hearing loss risk factors and hearing were studied in 200 traffic policemen working on the roads and 50 policemen working in offices. It was found that working conditions correlate with hearing loss in traffic policemen, so hearing loss in the policemen working on the roads is occupational and requires adequate prophylactic and therapeutic management.

[Medical standards of out- and inpatient care in otorhinolaryngology].

To improve the quality of out- and inpatient care in otorhinolaryngology, the authors propose standard minimum of medical measures (preventive, diagnostic and therapeutic) which should be taken in each nosological entity of ENT pathology. It is emphasized that medical standards should be adjusted to the disease peculiarities, possible atypical course, etc., but the scope of diagnostic and therapeutic care should not be less than the standard list of measures. Deviation from the standards is a criterion of medical service imperfection.

[Domestic 1.44 mcm Nd:YAG laser in combined treatment of dacryocystitis complicated by abscess formation].

We have treated 12 patients with acute dacryocystitis complicated by abscess formation. All the patients were examined by an ophthalmologist and otolaryngologist. The patients received intravenous antibiotics followed by Nd:YAG laser dacryocystorhinostomy via transcanalicular approach under the endonasal endoscopic control. The treatment resulted in resolution of symptoms and signs of acute dacryocystitis complicated by abscess formation in all the patients. No recurrence of acute dacryocystitis occurred during the median follow-up period of 11 months. Ostium patency defined as the absence of epiphora and observation of irrigated lacrimal fluorescein at the ostium was achieved in 9 patients. Epiphora recurred in 3 cases. Use of Nd:YAG laser reduces the duration of the operation and provides maximal functional and cosmetic results.

[Bacterial contamination of the maxilla after radical surgical treatment].

Qualitative and quantitative examinations of the microflora in a postoperative cavity of 62 patients early after radical surgery on the maxilla with opening of the ethmoidal labyrinth have found that in spite of a noticeable antibacterial effect of the operation pyoinflammatory process in the wound is possible. Treatment basing on bacteriological findings is recommended.

[Comparative efficacy and safety of amoxiclav (amoxicillin clavulanate) and ampicillin in the treatment of acute bacterial sinusitis].

Efficacy and safety of amoxiclav (amoxicillin clavulanate) and ampicillin were studied in antibacterial therapy of acute sinusitis. Antibacterial efficacy of amoxiclav in acute sinusitis reached 100%. Its safety was good. Antibacterial efficacy of ampicillin was 76%; 24% patients retained symptoms of the disease, x-ray picture of sinusitis. Its toxicity was higher.

[Effects of extrastimulation on morphofunctional state of cochlear stria vascularis].

An electron-microscopic study of the response of cochlear stria vascularis (CSV) cells to an electric impact of physiotherapeutic parameters was made in 26 guinea-pigs. Twelve guinea-pigs (group 1) have undergone 9 sessions of bilateral endaural electrostimulation with square bipolar impulses (4 ms, 0.1-0.4 mA, 50 Hz, 20 min). Fourteen guinea-pigs of group 2 have undergone a 12-18-day course of intramuscular ototoxic antibiotic canamycin in a dose 200-400 mg/day followed by the above electrostimulation. Nine anesthesized guinea-pigs of group 3 were inserted an electrode into the niche of the round window of the right ear. After a 10-day course of canamycin (200 mg/day) they were exposed to electrostimulation (8 sessions) with impulsed alternative current (100 Hz, 1 ms, 0.08 mA, 30 min). Five guinea-pigs served control. Morphofunctional state of CSV was assessed at electronograms by analyzing cell ultrastructure and counting the number of secretory vacuoles in the apical region of marginal cell cytoplasm. In group 1 there was intensification of biosynthetic processes in all CSV cells and a significant (p<0.01) increase of vacuole number in the marginal cells (53.9+/-3.8) vs normal value (33.9+/-1.5). In group 2 ultrastructure of all CSV cells was less affected and secretory activity was almost normal (31.4+/-2.9). CSV cell condition was similar to ultrastructure of the cells in group 2. The number of vacuoles in CSV marginal cells of the right (28.9+/-1.98) and left (29.1+/-5.4) cochleas was similar. Thus, electrostimulation activates biosynthetic processes in CSV structures and enhances secretory activity of marginal cells.
