[Tympanoplasty: problems and realization].

The authors describe how to provide safety and effectiveness of tympanoplasty, the principle of standard preparation and performance of the operation. They think it necessary to thoroughly assess stability of intralabyrinthine pressure and retrochochlear functions, to normalize humoral immunity and rheological blood properties, function of the acoustic tube, to choose optimal preserver and technique of reconstruction, how to evaluate postoperative outcome. Adequate use of the techniques widens the spectrum of reparative operations on the middle ear applied in practice.

[Vagotomy in bleeding ulcer of the duodenum].

The authors have analyzed immediate and late results of the surgical treatment of 162 patients. Organ-preserving operations are thought to be inferior to parcimonious resections of the stomach with vagotomy in reliability of arrest of bleeding and prevention of recurrences. Sphincter-destroying operations are believed to be expedient if the ulcer can be technically dissected both at the height of bleeding and in the intermediate (cold) period. Suturing of the ulcer is possible in patients with high operative risk.

[Reconstruction of the ossicle-tympanic system with a variable configuration prosthesis].

The author offers a prosthesis with variable internal configuration made of human fetus flat bone. The prosthesis is intended for ossicle tympanoplasty in chronic otitis media when total and subtotal destruction of the auditory ossicle takes place and in partial defect of the lateral attic wall. The design warrants adjustment to different area of the tympanic ring region. Morphological and functional outcomes of the reconstruction are satisfactory.

[A experimental device for ultrasonic control of ear ossicle prosthesis function].

A device designed by the author is intended for functional control over prosthetic auditory ossicle efficacy in experiment. The device employs Doppler effect and lies inside the frame adjusted for the experimental conditions. The device provides standard conditions for determining amplitude-frequency characteristics and signal transmission factor in the range 128-2000 Hz. As illustration, measurements of transmission from the T-shape prosthesis are described.

[Tympanoplasty in massive destruction of bony walls of the tympanic cavity].

New variants of occulotympanoplasty employing single and double--T, double--T--supporting prostheses in patients with chronic otitis media are provided. Surgical application of the flat bone and dura mater from human fetus facilitates the operative technique and reduces traumatism of the transplanted appliances. The results obtained confirm feasibility of adjusting the transformation system according to individual morphologic setting and suggest conclusions on superiority of the designs with little persistence, i.e. single and double--T prostheses.
