AIM: To evaluate frequency of clinical manifestations, biochemical and morphological alterations characteristic of liver affection in patients with glutenic enteropathy (GEP) in correlation with GE duration and severity of malabsorption syndrome (MAS). MATERIAL AND METHODS: 116 GEP patients have undergone clinico-laboratory examination, intestinoscopy with biopsy of jejunal mucosa, puncture biopsy of the liver with morphological examination of the specimens, ultrasonography of the liver. In addition, the immune status was assessed and tests for markers of hepatotropic viruses B and C were made. RESULTS: Clinical symptoms, typical for hepatic lesion in GEP patients, such as yellowness of the skin and scleras, palmar erythema, vascular stella and hepatomegaly were found in 7.2% cases. Hyperaminotransferasemia was detected in 48.4% of new cases of celiac disease, in 18.1% of patients on strict aglutenic diet. Aminotransferase hyperactivity detection was associated with severity of MAS. Morphological alterations in the liver in celiac disease are characterized by fat dystrophy, portal and periportal hepatitis, steatohepatitis or fibrosis. Combined therapy of celiac disease including strict aglutenic diet, correction of metabolic disturbances was not always effective in correction of metabolic processes in the liver. In new GEP cases early adequate therapy was most effective. CONCLUSION: The course of glutenic enteropathy is accompanied with clinicomorphological alterations in the liver. This necessitates use of drugs improving liver function.

The promoter of local immunity Heponum contributes to the restoration of eubiosis and normalization of showings of the immune status in patients with post-infection IBS. It is recommended to include Heponum in the complex therapy of chronic bowels diseases with the purpose of the restoration of normal microbiocenosis.
AIM: To study prevalence of celiac disease (CD) among patients with chronic diarrhea (ChD). MATERIAL AND METHODS: Serum levels of IgA-antibodies to gliadin, endomysium, reticulin and tissue transglutaminase were examined in 206 patients with CD. Biopsies were obtained from a distal portion of the duodenum or a proximal portion of the jejunum. RESULTS: CD was diagnosed in 35 (16.9%) of 206 patients with ChD. The disease was typical in 5 (2.4%) patients and was latent in 30 (14.5%) patients. Antibodies to gliadin, endomysium, reticulin and tissues transglutaminase in diagnostically significant titers were detected in the serum of all the examinees with CD. Formation of the antitissue antibodies occurred because of destructive-dystrophic alterations of the connective tissue of the lamina propria mucosae pointing to the autoimmune nature of a pathological process in celiac disease. CONCLUSION: To diagnose CD in ChD patients, it is necessary to supplement standard examination with duodenobiopsy, tests for antibodies to gliadin, tissue transglutaminase and to tissue structures--endomysium and reticulin.
This research is a continuation of the series of studies of the parietal microbiota of the bowel tissue samplings by the gas chromatography and mass spectrometry (GCMS) method [G.A. Ossipov et al.//Journal of the Society of Russian Gastroenterologists, 2001, 1:54-69]. The purpose was to study a number of new microorganisms in view of new data on the composition of their fatty acids (FA) and aldehydes; to confirm the presence of a number of bacteria, fungi and aerobic actinomycetes revealed earlier by FA markers in the composition of the bowel parietal microbiota by their isolation in a pure culture; to amend the estimation of the clinical value of changes in the composition of the human bowel parietal microflora in case of irritable bowel syndrome (IBS) and antibiotics-associated diarrhea (AAD). We examined 31 patients with IBS with predominating diarrhea, 18 patients with AAD and 3 volunteers (a control group). We studied the blood samples, tissue samplings of the mucous coat of the jejunum, ileum and colon and composition of healthy people's feces. The GCMS method was applied. Morphology of defined strains was controlled by methods of light and scanning electron microscopy. We found a substantial portion of eubacteria among the bowel microorganisms and specific changes of their species in case of IBS and AAD. Taking into account their physiological and biochemical activity, when regulating their concentration one can expect at least the same effect as when regulating the number of bifidobacteria and lactobacilli in treatment of intestinal pathologies and other diseases related to bowel dysbacteriosis. The analysis of the feces microbiota using the GCMS method by FA of parietal microorganisms provides reliable data on their number both in feces and in tissue samplings. We found a substantial portion of eubacteria among other bowel microorganisms (27% in the jejunum and 16% in the colon) and specific changes of their species in case of IBS and AAD. The concentration of streptomycetes, rhodococci and other members of the Actinomycetales order becomes dozens times more and/or reduces in pathological states. The tenfold concentration of markers of lactobacilli and bifidobacteria in some diseases stimulates the differentiated application of widespread probiotics based on these bacteria.

AIM: To develop screening diagnosis of gluten enteropathy (GEP), indications to administration of glucocorticoid hormones and objective criteria of effective treatment. MATERIAL AND METHODS: Clinical, immunological (antibodies-Abs to alpha-gliadin, reticulin and endomisium) examinations, enterobiopsy with morpho- and stereometry of small intestinal mucosa were made in 200 GEP patients. The examination was repeated 6 months to 5 years and later after the discharge from the hospital. RESULTS: Mean values of Abs to alpha-gliadin was 6 times higher than normal values. Positive titers of Abs to endomisium and reticulin were in 100 and 87.5% patients, respectively. Formed stool was registered 1.5 times more frequently, polyfecalia occurred 2.5 times less frequently, hypovitaminosis and trophic disorders were relieved 3 times more frequently, malabsorption syndrome reduced in patients given prednisolone vs those untreated with it. CONCLUSION: Screening diagnosis of GEP may be based on Abs tests to alpha-gliadin, reticulin and endomisium. Improvement of clinical condition of GEP patients can be stated by decreased diarrhea, polyfecalia and malabsorption symptoms. The treatment efficacy may be judged by clinical improvement, recovery of morphological structure of small intestinal mucosa, normalization of concentration of Abs to alpha-gliadin, reticulin, endomisium. Administration of prednisolone provides more complete and rapid rehabilitation of the patients.

In February 2002 the XXIX Scientific Session of the Central Research Institute on Gastroenterology was spent, where in the frames of the Round table some problems of the Syndrome of irritable intestine were discussed.
The purpose of the paper was studying the features of course and treatment of common variable hypogammaglobulinemia (CVHG), proceeding with intestinal function disorders functions.
AIM: To study the incidence, clinical manifestations, and diagnosis of chronic and lingering Yersinia ileitis. MATERIALS AND METHODS: 82 patients with pains in the right iliac area. The coagglutination test was used to reveal Y. pseudotuberculosis and Y. enterocolitica antigens as part of circulating immune complexes in the serum and coprofiltrates. RESULTS: Correct diagnosis was established 3 months to 2 years after the acute period of the disease. After etiotropic treatment, 2 patients continued to have the steady pain syndrome in the right iliac area, which is likely to be associated with periileitis and, 46 patients developed chronic Yersinia ileitis, as evidenced by their positive serological reactions to antigens and iliac morphological findings. Development of a residual phase may be presumed in 2 patients who continued to have stool disorders and abdominal pain, but no pseudotuberculous antigens in the stool and blood. CONCLUSION: Patients with lingering (up to 3 months) and chronic (over 3 months) Yersinia ileitis were 38.1% of the total number of those with ileocecal pain. The chronic nature of Yersinia ileitis is associated with late diagnosis, developed periileitis, and immunity disorders.
There are more than 500 species of microbes in the human bowels, and their total amount reaches 10(14), which is much higher than the total quantity of the cellular content in human organism. The history of teaching about microbiocenosis demonstrates various connections of microorganisms with the bowels and other organs and systems of human body. The available data about the significance of the bowels microbe flora for human organism give grounds for comparing it with the nervous, cardiovascular or any other organism system.
