An alpha-L-fucosidase from Thermus sp. with unusually broad specificity.

An alpha-L-fucosidase (E.C. 3.2.1.51) exhibiting a wide aglycon specificity expressed in ability of cleaving alpha1 --> 6-, alpha1 -->3-, alpha1 --> 4-, and alpha1 --> 2-O-fucosyl bonds in fucosylated oligosaccharides, has been isolated from culture filtrate of Thermus sp. strain Y5. The alpha-L-fucosidase hydrolyzes p-nitrophenyl alpha-L-fucopyranoside with V(max) of 12.0 +/- 0.1 microM/min/mg and K(m) = 0.20 +/- 0.05 mM and is able to cleave off about 90% of total L-fucose from pronase-treated fractions of fucosyl-containing glycoproteins and about 30% from the native glycoproteins. The purified enzyme is a tetramer with a molecular mass of 240 +/- 10 kDa consisting of four identical subunits with a molecular mass of 61.0 +/- 0.5 kDa. The N-terminal sequence showed homology to some alpha-L-fucosidases from microbial and plant sources. Hydrolysis of p-nitrophenyl alpha-L-fucopyranoside occurs with retention of the anomeric configuration. Transglycosylating activity of the alpha-L-fucosidase was demonstrated in reactions with such acceptors as alcohols, N-acetylglucosamine and N-acetylgalactosamine while no transglycosylation products were observed in the reaction with p-nitrophenyl alpha-L-fucopyranoside. The enzyme can be classified in glycosyl hydrolase family 29.

[25-year experience with surgical treatment of stomach cancer].

Based on an analysis of the surgical treatment of 4107 patients with gastric cancer the author discusses indications for different radical operations, specific features of the method of gastrectomy used in 1090 out of 1649 patients subjected to radical operations. Better immediate results of gastrectomies were obtained due to the improvement of the operation technique, preoperative and postoperative management of the patients. The average postoperative lethality was 11%, that in the recent years being not higher than 9,4%. The long-term results of the surgical treatment of gastric cancer still remain not satisfactory.

[Results of the surgical treatment of disseminated forms of stomach cancer].

The paper discusses both immediate and end results of 1,090 total gastrectomies for treatment of cancer carried out in 1958-1982. Postoperative complication incidence decreased twofold within 25 years. A considerable drop in postoperative lethality has been registered, its rate staying lately under 9.6%. Five-year survival rate is 38.0%; ten year-17.1%.

[Levels of growth hormone, prolactin, cortisone, aldosterone and insulin in the blood of patients with stomach cancer, before and after gastrectomy].

The basal blood levels of certain hormones were determined by radioimmunologic assay in 130 patients with stomach cancer before surgery and at different periods (10-30 days, up to 1 year, 1-5 years and over 5 years) after gastrectomy. Increased levels of growth hormone and prolactin were matched by a lowered concentration of insulin. Elevated levels of cortisol and aldosterone were observed in stage IV. Within the first 10-30 days after operation, hormonal profile (except insulin) showed a tendency to normalization. At later stages, hormone levels showed various changes.

[Radionuclide methods in evaluating the sequelae of gastrectomy for cancer].

The fat, protein and B12 absorption by the liver, kidney and small intestine was studied by radionuclide methods in gastric cancer patients during the preoperative period as well as within 15-30 days, 3-12 months and for more than a year following gastrectomy. It was observed that the function of the kidney, liver and B12 absorption was disturbed during the preoperative period. Gastrectomy caused a marked depression of the renal, hepatic and small intestinal function. During later periods following gastrectomy, these functions improved but were not restored to normal.

[Results of interilioabdominal amputations].

Twenty four interilioabdominal amputations for malignant tumors are described. Malignant tumors of the proximal portion of the femur or pelvis were taken as indications for surgery. The Salishchev incision was used. The main steps of the operation are given. Two patients died immediately after the operation. The fate of the other 22 patients was followed: 17 patients died within 3 years after the operation, 3 patients survived for more than 5 years. The worst results were obtained in osteogenic sarcomas and Ewing's sarcomas.

[Epidemiology of stomach cancer in the Kirghiz SSR].

The incidence of gastric cancer in the Republic is characterized by some features related to local natural conditions, ethnic and socio-economic factors. These features comprise variations in the gastric cancer incidence in separate climatic-geographical zones. A higher morbidity rate was noted in Chuisk valley in the north of the Republic. The lowest morbidity rate was recorded in Osha Province. The index of the gastric cancer morbidity among Russian population is higher than among Kirghiz and Uzbek nationalities. During recent years there is a tendency to the reduced morbidity and mortality due to cancer of the stomach in the Republic.

[Radioisotope and biochemical methods in the study of exocrine function of the pancreas and small intestine at early periods after gastrectomy for cancer].

77 patients were examined during a period from 1 to 30 days following gastrectomy for cancer. Depression of the exocrine function of the pancreas was observed. The concentration of amylase, lipase and trypsin was found to be reduced during the first 5 days after the operation. Following 19-30 days the amylase concentration is restored, the lipolytic activity is enhanced, whereas the tryptic one remained considerably impaired. The small intestine functioning is markedly disturbed, the processes of hydrolysis and fat absorption suffering most of all; the protein absorption is changed but to a less extent.

[Characteristics of cancer epidemiology in the Kirghiz SSR].

The oncological service in the Kirgiz republic is characterized, and some data on cancer morbidity there are reported. The oncological network of the republic covers 52 medical treatment establishments, including one oncological institute. The oncological beds provision makes 2.7 per 10 000 population. The regional features of the distribution of malignant diseases in that part of the country are conditioned by natural, ethnic and economic factors.

[The epidemiology of cancer in Central Asia].

The author reports the data characterizing the incidence of cancer, as a whole, and its main localizations in the Republics of Central Asia. According to these findings the cancer incidence indices the Central Asia are more low, as compared with the All-Union data and the data from other Socialist Republics. The main localizations of cancer, except esophageal cancer, are recorded more seldom than in other Republics. Especially low indices of the morbidity were noted in relation to cancer of the skin, lower lip, mammary gland, uterine cervix. In some Republics of Central Asia (Uzbek SSR, Turkmen SSR) the incidence of esophageal cancer was found to be high, exceeding several times the corresponding indices in the USSR and other Socialist Republics.
