IgE responses to Dermatophagoides pteronyssinus native major allergens Der p 1 and Der p 2 during long-term specific immunotherapy.

We investigated by ELISA the IgE response to whole extract of the house-dust mite Dermatophagoides pteronyssinus (Dp) and to the native major allergens, Der p 1 and Der p 2, in sera from 18 adult patients (group A) with Dp-allergic asthma before (t0) and 1, 2, 3, and 4 (t1-t4) years after subcutaneous specific immunotherapy (SIT). A qualitative reduction (P = 0.05) of the IgE responses to Dp and Der p 2 was observed from t1 to t4, but a highly statistical significant decrease appeared at t3 (P < 0.01). With regard to Der p 1 IgE values, the immunotherapy induced a significant decrease (P < 0.01) at t3, but not before. In group A, the IgE responses to Der p 1 and Der p 2 were not correlated at t0 (rs = 0.31; P = 0.21) but were correlated at t3 (rs = 0.78; P = 0.001). We also examined sera from 14 adult patients (group B, same SIT schedule as group A) who were without respiratory symptoms at the end of the third year (t3) of Dp SIT. At this time (t3), there were no significant differences in Der p 1 and Der p 2 IgE levels between group A and group B.

Eosinophils in allergic diseases: immunopharmacological regulation.

Eosinophils (EOS) are cells that have excited great interest among researchers because of their involvement in many phenomena, and particularly those of defence against parasitic infestations and of modulation of allergic inflammatory reactions. The ontogenesis, morphology and functions of EOS are described, and especially the receptors and chemical mediators they synthesize. Particular attention is paid to the EOS role as a central element in allergic inflammation processes, able to maintain the inflammatory state by producing specific cytokines and to cause tissue damage by producing free radicals. The cell's role in the defence against parasitic infestations is also considered. Finally, emphasis is placed on EOS capacity for immunopharmacological regulation by means both of active secretion of regulating substances and of modulation of other substances, able to inhibit certain phases of allergic inflammation.

[New prospects in the therapeutic use of somatostatin and its derivatives].

The discovery and subsequent clinical application of somatostatine, a polypeptide neurohormone of 14 amino acids, and of its analogs, opens a novel chapter of neuroendocrinology that is still in full evolution and to a large extent unknown. The isolation of an octapeptide, a selective somatostatine analog, permits to prolong its action, in fact it has a halflife of about 140 min in old subjects and about 100 min. in the young. Thanks to its excellent tolerability, the synthetic hormone can be usefully applied in the treatment of acromegaly, gigantism and hypersomatotropinemic conditions in general, and even in other clinical branches, such as treatment of esophageal hemorrhage due to the rupture of varices in liver cirrhosis or to erosion of gastric blood vessels in patients suffering from peptic ulcer disease.

[Inhibition by ketotifen of specific skin wheal reaction in atopic subjects].

In order to reconsider the ability of ketotifen (K) to inhibit the specific (allergen-induced) skin wheal reaction (SWR) and the correlated problems, 32 patients, all suffering from seasonal allergic rhinitis due to grass pollens, have been studied. K has been compared with sodium cromoglycate (SCG), which is not metabolized in man. SPT with grass pollen extracts before and after treatment have been performed in all patients, divided into two numerically equal groups treated with either drug at usual dosages for eight weeks. The specific SWR has been significantly inhibited by K, but not by SCG. The theoretical and practical meaning of this finding is discussed.

[Endocrine aspects of puberty. Modifications of gonadotropins (LH, FSH), testosterone and cortisol circulating after ACTH and synthetic LH-RH].

The effect of synthetic LH-RH (Relisorm Serono: 25 microgram i.v.) on plasma LH, FSH and testosterone was examined in 19 normal pubescent and prepubescent boys. Changes in cortisol and testosterone after 0.25 mg/m2 body surface ACTH (Synacthen ciba) was assessed in a further 8 prepubescent subjects. Notable amounts of FSH, LH and testosterone were noted in prepubescent subjects, pointing to hormonal production prior to sexual maturation. There was a prompt and significant increase in LH in all cases. In prepubescent subjects, this was accompanied by marked increases in FSH on some occasions, though no appreciable change in circulating testosterone. This suggests that the hypophyseal reserve of LH is sufficient in puberty. Prepubescent subjects displayed a significant rise in cortisol 30' and 60' after ACTH, whereas testosterone fell with respect to the start values. The results indicate that acute stimulation of the adrenal cortex before puberty leads to testosterone suppression.
