An assay procedure for solubilized thyroid hormone receptor: use of Lipidex.

A method using Lipidex-1000 is reported for the assay of thyroid hormone receptor activity. The receptor was extracted from rat liver nuclei, incubated with [125I]T3, and applied to 1-ml disposable pipet tip columns containing a small volume of Lipidex. The resin absorbed the free hormone, and the receptor-bound hormone was recovered in the eluate. The method allowed accurate assay of the receptor activity, satisfying a linear relationship between the activity and the receptor protein concentration. The usefulness of this method was demonstrated by determining the pH optimum and the Kd and Bmax of T3 binding by the receptor. The Lipidex column can be used for the preparation of an unoccupied receptor after dissociation of the endogenously bound hormone.

Complete sacral agenesis--a case report.

We report a case of a girl with complete sacral agenesis associated with arthrogrypotic-like deformity. This case demonstrates that a posterior knee release and braces following operation are effective treatment for knee-flexion contractures. Accordingly it is suggested that a supracondylar osteotomy is not required for mild or moderate recurrence of knee-flexion contractures.

[Comparison of two hours' biofiltration and four hours' bicarbonate hemodialysis by multiple clinical parameters].

Ten uremic patients maintained stable on regular dialysis treatment participated in a comparison study of 2 hours' biofiltration and 4 hours' bicarbonate hemodialysis with informed consents. In biofiltration, ultrafiltrate was replaced by a solution consisting of Na 145 mEq/l, HCO3- 100 and Cl 45 at the infusion rate 2.51/hour. Dialysate composition was Na 130-149 mEq/l, K 1.0, Cl 119, Ca 2.5, Mg 0.5, CH3COO- 15 and glucose 200 mg/dl. Hemodiafilter was F80, polysulphone, 1.9 m2, manufactured by Fresenius Co. Ltd. Blood flow rate was 5 ml/min/kg.body.weight to keep urea index (Kt/V) over 1.0. B-A-B' comparison was designed in which B and B' stand for 4 hours' bicarbonate hemodialysis while A for 2 hours' biofiltration, 3 times per week for 2 months, respectively. It was intended to find out if there are aggravations of clinical parameters in A after B and/or improvements in B' after A in view of evaluation of optimum for 2 hours' biofiltration. One patient was withdrawn from biofiltration at 15th treatment in A because of frequent muscular twitchings. Others finished the whole program, thus making drop-out rate 10%. No significant differences were observed in the following parameters between B and A and between A and B': cardiothoratic ratio, pre-treatment blood pressure, human atrial natriuretic hormone, cardiovascular dynamics, total protein, BUN, serum creatinine, uric acid, beta 2 microglobulin, blood counts, blood gas analysis, electrolytes, alkaline-phosphatase, PTH-C, protein catabolic rate (PCR), lipids and liver functions.(ABSTRACT TRUNCATED AT 250 WORDS)

[Successful treatment of "steroid-immunosuppressant resistant" polymyositis with immunoadsorption].

We report a case of "steroid and immunosuppressant-resistant" polymyositis that was successfully treated with immunoadsorption. A 46-year-old man was admitted to Shinshu University Hospital on June 2, 1989, with the chief complaint of muscle weakness and pain. He had a 1.5-year history of frequent episodes of Raynaud's phenomenon. Five months before presentation, he noticed muscle weakness and pain in the proximal muscles of his extremities. One month after the onset of these symptoms, his body temperature increased to 38 degrees C and remained elevated until the following month. He subsequently entered a local hospital. He was found to have diminished strength in all limbs that was more marked in the proximal areas. The muscle weakness and pain progressed until the patient was no longer able to walk. Muscle biopsy of the left quadriceps femoris showed mononuclear cell infiltration in the endomyosium that was accompanied by phagocytosis and interstitial fibrosis. Based on these findings, a diagnosis of polymyositis was made. During hospitalization, the weakness continued to worsen despite pulse therapy with methylprednisolone 1000 mg per day for 3 days and the administration of prednisolone 100 mg and azathioprine 100 mg per day. The patient was transferred to our hospital for further evaluation and treatment. On physical examination, he demonstrated hair loss, soft palate aphthae, tonsillar injection, and cervical lymphadenopathy. Neurological examination revealed generalized weakness; the patient was barely able to move all of his limbs. All of the muscles in his extremities were swollen and hard, and he felt pain when they were grasped. Deep tendon reflexes were diffusely hypoactive.(ABSTRACT TRUNCATED AT 250 WORDS)

Relationship of uptake of technetium-99m(Sn)-N-pyridoxyl-5-methyltryptophan by hepatocellular carcinoma to prognosis.

The relationship of technetium-99m(Sn)-N-pyridoxyl-5-methyltryptophan (99mTc-PMT) uptake by hepatic tumors to survival was studied in 162 cases of hepatocellular carcinoma (HCC). The median survival of 82 patients in whom hepatic tumors showed increased uptake in delayed 99mTc-PMT imaging was 1013 days, which was significantly longer than the survival time of 398.5 days of 80 patients in whom hepatic tumors did not show increased uptake of radioactivity (p less than 0.002). The relationship between the ability of hepatic tumors to take up 99mTc-PMT and survival was also analyzed in patients with HCC showing filling defects in 99mTc-colloid liver images and, in relation to the therapy, serum values of bilirubin and alpha-fetoprotein. Results indicated that the degree of 99mTc-PMT uptake by hepatic tumors is closely correlated with the prognosis of patients with HCC.

[Recent progress in the treatment of oral and pharyngeal cancer].

Today, in the treatment of oral cancer, combined resection and reconstructive surgery has shown marked progress. Especially, the surgical procedures of bone graft after mandibulectomy have been seen widespread application. In the treatment of nasopharyngeal cancer, adjuvant chemotherapy after therapeutic dose of irradiation indicates a better survival rate than radiation alone, and in advanced nasopharyngeal cancer, a wide resection is effective for cure. In the surgical treatment of hypopharyngeal cancer, by the immediate reconstruction of pharynx with jejunum, patients can eat orally in a short time. The most important problem in the treatment of head and neck cancer is development of effective chemotherapy against distant metastases.

Antineoplastic Combined Chemotherapy Protocols

Chromosomal assignments of the human endothelin family genes: the endothelin-1 gene (EDN1) to 6p23-p24, the endothelin-2 gene (EDN2) to 1p34, and the endothelin-3 gene (EDN3) to 20q13.2-q13.3.

Chromosomal assignments of the genes for the human endothelin family, the endothelin-1 gene (EDN1), the endothelin-2 gene (EDN2), and the endothelin-3 gene (EDN3), were accomplished by in situ hybridization to human metaphase chromosomes using a 3H-labeled human preproendothelin cDNA probe for each endothelin. The chromosomal assignment of the EDN2 was also performed by Southern blot analysis of somatic cell hybrid DNAs. EDN1, EDN2, and EDN3 were mapped to 6p23-p24, 1p34, and 20q13.2-q13.3, respectively.

Chromosome Banding

[Chemoembolization therapy with lipiodol, cisplatin and etoposide for hepatocellular carcinoma].

Chemoembolization using CDDP, VP-16 and lipiodol was carried out for 7 patients with hepatocellular carcinoma (HCC). CDDP/lipiodol, CDDP/VP-16, CDDP/lipiodol (lipiodol 2-10 ml, CDDP 1-2 mg/kg, VP-16 100 mg/body) and gelatine sponge were administered in that order through the catheter located in the proper, or right or left hepatic artery. Three patients underwent hepatic resection 38-50 days after this treatment. Complete necrosis of the tumor was recognized in the one case, although the portion of necrosis did not exceed 70% in large sized HCC as the diameter of more than 10 cm. In 4 unresectable cases the decreases in tumor size were observed by ultrasonography and computed tomography. The response was: 3 partial responses and 1 no change. One out of 4 cases could undergo hepatic resection 17 months after this treatment. Two patients are alive 20 months after this treatment, although one patient died of HCC after 25 months. Serious side effect was not observed.

[A case report of very huge dermatofibrosarcoma protuberans].

A 63-year-old male with dermatofibrosarcoma protuberans of the right hip is presented. The patient noticed a small nontender mass 40 years ago and then the tumor enlarged gradually. The patient was very emaciated and needed to receive blood transfusions. The massive multinodular tumor was pedunculated and on it's surface, there were areas of hemorrhage and secondary infection. The operative specimen weighted 2270 g and measured up to 25 by 20 by 10 cm. To our knowledge, this case represents the largest primary tumor of a dermatofibrosarcoma protuberans in review of English literature.

Immunohistochemical demonstration of nuclear S1 proteins in various cells.

S1 proteins are present in the nuclear structures sensitive to DNases and RNase. To examine localization of these proteins, an antibody was raised in a rabbit. Indirect immunofluorescence staining revealed that S1 proteins located in the extranucleolar nuclear regions of quiescent myocardial and cerebellar cells as well as actively duplicating mouse 3T3 fibroblasts. They located in euchromatin regions of thymus lymphocytes, with a characteristic aster-like immunofluorescence pattern, and on the border of condensed chromatin areas by deposition of immunogold particles in ultrathin sections of thymus. Thus, S1 proteins may be in a nuclear function assigned to the border of heterochromatin areas, and other than synthesis of DNA or of ribosomal RNA. Possible involvement of S1 proteins in the extranucleolar RNA synthesis is discussed.

Two splenic soluble tyrosine kinases from the rat.

Tyrosine kinase was extracted from rat spleen. Some 60% of the activity with angiotensin as the substrate was found in the cytoplasmic supernatant obtained at 100,000g. Two kinases in the supernatant, TKI and -II were purified 170- and 600-fold, respectively, by column chromatography. The molecular weights of TKI and -II were estimated to be 38,000 and 30,000 by Sephacryl S-200 gel filtration, and to be 42,000 and 36,000 by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. Experiments with various protease inhibitors suggested that these kinases were not the artifacts of proteolysis. Both kinases autophosphorylated their tyrosine residues. They had different Kms for angiotensin II, and for ATP, but similar dependencies on temperature and on divalent metal ions.

Development of a new type of influenza subunit vaccine made by muramyldipeptide-liposome: enhancement of humoral and cellular immune responses.

The muramyldipeptide (MDP), [6-O-(2-tetradecyl-hexa-decanoyl)-N-acetylmuramyl-L-isoglutamine] can be incorporated into liposomes with haemagglutinin and neuraminidase subunits were attached to the inner and outer surfaces of lamellar structures of the liposomes, probably through their hydrophobic ends. The addition of cholesterol resulted in much more stable liposomes, which were similar in size and shape to native influenza virus particles. These liposomes enhanced the immunogenicity of haemagglutinin in mice, such that the levels of antibody induced were about 16-fold higher than those of subunit haemagglutinin vaccine alone. Results of proliferation tests with spleen cells from mice and guinea-pigs were consistent with the immunopotentiation of haemagglutinin by liposomes. In addition, the higher antibody levels produced in mice, immunized with the haemagglutinin and MDP-containing liposomes (MDP-virosomes), were maintained for at least 6 months. Enhancement of the cellular immune response, measured by delayed type hypersensitivity reactions, was also observed in the guinea-pigs immunized with MDP-virosome vaccine. Preliminary tests with splenocytes from mice immunized with different vaccines also indicated that the MDP-virosome vaccine induced cytotoxic T-cell activity in these mice. This study revealed that the formation of liposomes with muramyldipeptide enhanced the level and persistence of circulating antibody, and enhanced cellular immunity in guinea-pigs and mice.

Acetylmuramyl-Alanyl-Isoglutamine

Growth inhibition and morphological changes of LLC-PK1 induced by ultimobranchial calcitonins.

Ultimobranchial calcitonins (CTs), known to stimulate cAMP production, inhibited the growth of a porcine kidney cell line LLC-PK1. This inhibition was accompanied by degenerative changes including vacuole formation and cell detachment. The electron microscopic study revealed marked swelling of rough endoplasmic reticulum (RER). Other cAMP-increasing agents such as human CT, arginine, vasopressin, and forskolin showed less growth inhibitory activities and no induction of the degenerative changes. These results indicate that the growth inhibition of LLC-PK1 by ultimobranchial CTs is mainly due to cellular death caused by the swelling of RER via a signalling pathway other than the cAMP-dependent event(s).

8-Bromo Cyclic Adenosine Monophosphate

Fatty acyl-CoAs are potent inhibitors of the nuclear thyroid hormone receptor in vitro.

We report that long-chain fatty acyl-CoAs are potent inhibitors of the thyroid hormone (T3) receptor isolated from rat liver nuclei. Both saturated and unsaturated fatty acyl-CoAs were similarly potent. Fifty per cent inhibition of T3 binding by the receptor was observed at an oleoyl-CoA concentration as low as 1.3 microM, and the affinity of oleoyl-CoA for the receptor (Ki) was estimated to be 0.45 microM. Fatty acyl-CoAs also promoted dissociation of the hormone bound to the receptor. The action of fatty acyl-CoAs was competitive for the hormone binding site, resulting in a reduction in the receptor's affinity for T3. These observations suggest that fatty acyl-CoAs modulate the binding of the thyroid hormone to its nuclear receptor, in vitro. Whether or not such events occur in vivo remains to be determined.

Increased focal uptake of Tc-99m stannous phytate in an irregular fatty liver demonstrated by SPECT imaging.

An area of focally increased uptake of Tc-99m colloid (hot spot) in the liver has been reported in superior vena cava syndrome, Budd-Chiari syndrome, and focal nodular hyperplasia. A case of irregular fatty infiltration in which SPECT imaging clearly demonstrated an area of focally increased uptake in the left lobe of the liver, which was not well visualized by planar imaging, is presented. There have been a few previous reports of this finding on SPECT imaging in patients with irregular fatty infiltration of the liver.

Fatty Liver

Clinical value of magnetic resonance imaging for cervical myelopathy.

The magnetic resonance imaging (MRI) findings in 115 cases of cervical myelopathy, 121 cases of cervical radiculopathy, and 64 cases of neck pain with no neurologic deficit were prospectively studied to investigate the clinical value of MRI for cervical myelopathy. The MRI findings in the T1-weighted sagittal projection were classified into four groups according to the degree of the compressed deformities of the cervical cord. The degree of compression of the cervical cord on MRI findings showed a significant correlation with the severity of myelopathy, the anteroposterior diameter of the spinal column, and the degree of compression of the dural tube in the myelograms (P less than 0.01). Fifty-one patients of cervical myelopathy had undergone both preoperative and postoperative MRI. Of these, the spinal canal of 47 patients that was well decompressed was recognized according to plain computed tomography (CT). However, 24 (51%) of these 47 patients showed on MRI a deformity in the spinal cord amounting to cord atrophy. The correlation between the clinical function of the spinal cord and the recovery of the cord deformity on MRI at the operative levels was accurately investigated in 34 patients who had no cord deformities in the adjacent intervertebral levels. Twenty patients with cord atrophy had slightly poor clinical results, although no significant difference was found between these 20 and 14 patients with recovery in the cord deformities. From these results, it was evident that T1-weighted MRI is useful in the accurate diagnosis of compression myelopathy, in accurately deciding the level of the disease focus, and in the accurate assessment of the surgical results.
