[Substantiation of wound retractors choice for median approach in various abdominal trauma].

Clinical and morphological experimental examination for assessment of advisability of the use of wound retractor by Sigal-Kabanov in median celiotomy has been carried out. The results of anatomical experiments in 28 cadavers of adult persons and the application of wound retractors for surgical operations in 124 patients with injuries of abdominal organs have evidenced the advantages of proposed version for retraction of the wound over the other methods.

[Method for creating approach by Volkovich-D'iakonov in technically complicated appendectomies].

As a result of morphological experiments in 28 corpses, various methods for correction of the approach by Volkovich--Dyakonov regarding various topographo-anatomical location of the appendix were assessed. The design of Sigal--Kabanov retractors has been improved. The mode of making the approach by Volkovich--Dyakonov by apparatus correction was developed. It was used in clinical trial of 62 patients with acute appendicitis. Comparative clinical characteristics have shown some advantages of this original variant of correction of the surgical approach compared to traditional one.

[Middle weight molecules before and after detoxification in patients with peritonitis].

The dynamics of changes of the middle mass molecule (MMM) content in blood serum and peritoneal exudate was studied in 193 patients before and after different methods of detoxification: intraoperative cleansing of the abdominal cavity in closed treatment, planned stage cleansing in open treatment of peritonitis, hemosorption, therapeutic plasmapheresis, sorption through a pig spleen. The control group was made up of 88 donors and 16 patients with an urgent abdominal disease without signs of peritonitis. The diagnostic value of MMM and the expediency of combining surgical methods for cleansing of the abdominal cavity with methods of extracorporeal detoxification are shown.

[A method of peritoneostomy for treating diffuse purulent peritonitis].

For improvement of exudate outflow and prevention of the development of abscesses, a modified method of peritoneostomy has been suggested and successfully used in treatment of 59 patients. After elimination of a source of peritonitis and sanitation of the abdominal cavity, the greater omentum is covered with a hydrocellulose film of corresponding sizes, rolled up as a rouleau, and placed in the epigastrium. The intestinal loops are covered with a perforated polyethylene film sutured to the ends of a laparotomy wound.

[A variant of peritoneostomy as a means for intensive therapy and detoxication in acute diffuse suppurative peritonitis].

The authors have improved the method of open treatment of acute diffuse purulent peritonitis. Clinical test of the original peritoneostomy variant has shown it's perspectivity. The treating effect is explained by the possibility of performing of permanent controlled sanation of abdominal cavity, prophylaxis of lung postoperative complications and defence of abdominal wall tissues from the repeated infection. The method is recommended to be applied in clinic.
