[A rare case of metastases in the diaphragm in a patient with gastric cancer].

In spite of the general affection of the greater curvature of the stomach, there were no dyspeptic manifestations, pains in the upper half of the stomach were not related to meals, 'and the stool remained unchanged. The main characteristic of the clinical picture was the distribution of the tumor process to the diaphragm, which was a reason explaining the complaints about acute pains in the right flank enhancing in the course of movements or deep breaths. No data about any metastatic affection of the skeleton bones, lungs or pleura were received. This case shows the need in the close examination of the diaphragm in the case of the pain syndrome in the right hypochondrium in patients with generalized tumor diseases.

[Treatment and diagnostic policy in gunshot wounds of the pancreas].

Results of treatment of 71 patients with gun-shot wounds of the pancreas were analyzed. Features and structure of gun-shot abdominal penetrating wounds with injury of the pancreas, potential of up-to-date diagnostic methods were studied. Surgical policy in combined injuries of hollow organs and pancreas was developed. Variants and sequence of surgeries were determined depending on location and types of pancreatic injuries. Proposed treatment and diagnostic policy permitted to reduce rate of purulent and septic complications by 11.1% and lethality by 5.3%.

[Dental status of patients with chronic diffuse diseases of the liver].

The intensity and prevalence of diseases of hard dental tissues and periodontium, correlation of serum, unstimulated mixed saliva, and gingival fluid biochemistry, and changes in the periodontal microcirculation were studied in 122 patients with chronic diffuse diseases of the liver. Specific changes in the biochemical parameters of unstimulated mixed saliva and gingival fluid in these patients were directly related to the biochemical parameters of the serum and the fact of dental treatment. Microcirculatory disorders in the distal portions of periodontal vessels (attached gingiva and gingival papilla) depended on local factors, while disorders in larger vessels of the buccal mucosa (transitional fold) were related to the severity of the underlying disease.

[Experience with spiral computed tomography used in the diagnosis of atherosclerotic calcification of coronary arteries].

Cardiovascular diseases, including coronary heart disease (CHD), are one of the main causes of death in persons aged above 40 years. The WHO report of human health in 2003 declares cardiovascular diseases as an occult epidemiology of not only the developed, but also developing countries. CHD prevention successfully tested in the USA and Europe is recognized to be the only method of its control. The standardized calculation of coronary calcification (Cardiac Scoring) is one of the most constituents of this prevention. Cardiac Scoring was first used during electronic radiation tomography in 1990. While algorithms of a study and clinical use of the results were produced, X-ray computed tomographs, both single-spiral (SSCT) and multispiral (MSCT) ones were perfected. The experience with Cardiac Scoring was successfully applied to the use of MSCT. Although routine spiral tomography ranks below MSCT in its capacity of visualizing the distal segments of coronary arteries and of synchronizing with the movement of the heart, SSCT has some advantages for its wide use, accessibility, and rather high efficiency. Demand for primary, ambulatory screening for coronary atherosclerosis, primarily asymptomatic one when the typical symptom complex of CHD has not been formed yet generates the necessity of using all possible methods for calculation of calcification, including spiral computed tomography. The determination of coronary calcification by SSCT and MSCT is under discussion, this problem has its supporters and opponents. At the same time it is accepted that it would near sightedly suppress innovation that may be finally advantageous.

[Spiral computed tomography with functional tests in diagnosis of chronic obstructive diseases of the lungs].

The potential of spiral computed tomography (SCT) and high resolution computed tomography (HRST) in diagnosis of chronic obstructive pulmonary diseases (COPD) is described. Semiotic sings of bronchial asthma (BA) and chronic obstructive bronchitis (COB) are specified. Informative value of SCT in differential diagnosis of BA and COB including early stage of the diseases is analysed. An updated technique of SCT and HRCT of the lungs in COPD patients is presented.

[Meta-analysis of the cumulative effects of piloting G-loads on the cervical vertebrae].

On the data of meta-analysis performed in 10 independent Russian and international studies of the cumulative effects of piloting g-loads on functioning of the vertebral column cervical segment, the conclusion has been made concerning a statistically significant linkage between the degenerative dystrophic diseases of the vertebral column and regular exposure to the +Gz loads.

[Ultrasonic investigations in diagnostics of prostate diseases in flying personnel].

Ultrasonic semiotics of the most common prostate problems was studied. Benefits from transabdominal and transrectal scanning to evaluation of the gland and surrounding tissues were highlighted. Prostate distresses were consistent with some unfavorable factors of air flight. The significance of incorporation of USI in the diagnostic algorithms of medical certification of flying personnel afflicted with prostate problems was stated.

[Modern diagnostic approaches to investigations of abdominal aorta changes in flying personnel].

Modalities of the modern radiation diagnostics (USI and computer tomography) in expert investigation of the ventral aortas in pilots of different aircraft were demonstrated. The most effective diagnostic algorithm of investigation and determination of hypertrophy of the ventral aorta, and detection of the atherosclerotic plaques calcinosis has been developed and its importance for medical certification demonstrated. Morphostructural changes in the ventral aorta were featured and correlated with the debilitating occupational conditions in different types of aviation.

[The role of radiation study methods in the expertise of flight personnel with kidney diseases].

Application of the most effective diagnostic algorithm in some renal diseases was investigated. There has been performed a combined clinico-radial examination of 291 pilots of various aviation categories who suffered from renal cysts, nephroangiosclerosis, chronic pyelonephritis and hydronephrosis. The radial semiotics of these diseases has been developed. The necessity of an integrated use of the methods of radial diagnostics is indicated, the criteria for taking an expert decision are elaborated.

[Radiation diagnosis of spleen changes in flying personnel of fighter and helicopter aviation].

Some specific alterations of the spleen in flying personnel of fighter and helicopter aircraft have been elucidated. Regular patterns of splenomegaly appear to be linked with the adverse effects of air flight. The ultrasonic investigation was proved to be one of the most significant elements of the diagnostic algorithm used to determine the size and structure of the glandular parenchyma. The X-ray computer tomography and methods of computational analysis are no more than ancillary tools to get better insight into syntopy of the organ and surrounding tissues. The radionuclide investigation provides tentative data concerning the functional state of the organ.

[Role of ultrasonic investigations in diagnosis of pancreatic diffuse pathology in flying personnel].

The article presents data of a complex clinical/radiation examination of flying personnel with diagnosed diffuse pathologies of pancreas. Consistent patterns of the fibrotic process in pancreatic parenchyma were related to professional specifics. Preponderance of ultrasonic investigations, mathematical analyses, and X-ray computer tomography for the enhanced diagnostic algorithm for the medical certification of flying personnel were demonstrated.

[Clinical and X-ray diagnosis of initial degenerative changes in the lumbar section of the spine in flight personnel].

The paper demonstrates the diagnostic validity of X-ray computerized and magnetic/resonance tomographies in detecting early degenerative/dystrophic processes in the lumbar compartment of vertebrae in flying personnel. Examination of 215 aviators with varying degrees of impairment of the vertebrae-pelvis-lower extremity biokinematic system allowed to relate protrusions of the intervertebral disks. Correlation between the neurologic pattern of disease and different forms of protrusions was studied. Pathogenesis of the radiculopathy syndrome was made more clear. Potentials of the X-ray computerized and magnetic/resonance tomographies were given a comparative analysis. As was established, the two methods are not mutually exclusive. MRI is more advantageous in the diagnosis of degenerative/dystrophic changes in soft a tissue structures. On the other hand, the X-ray computerized tomography is preferable for visualization of changes in highly dense tissues. Semiotics of processes articularis arthrosis was studied. Discovered were X-ray detectable morphological signs of arthrosis which had never been discussed in literature. Main strategies of medical treatment of patients with various forms of protrusions are proposed.

[The current aspects of diagnosis and expertise in flight personnel with gallstones].

The paper discusses the modern aspects of diagnosis and expertise of pilots suffering from gall-stones. The present-day methods were used to examine 196 pilots of different aviation categories (helicopter in the main). Semiotic signs of different variations of gall-stones are studied, comparative characteristics of diagnostic methods are demonstrated, more effective algorithms of examination are elaborated. It is shown that in the radio-clinical examination the ultrasonic investigation holds the lead in the diagnostic algorithm enabling in the shortest way early and correct detection of concretions in the biliary tracts. Viscous putty-like bile in the gall bladder found during preventive monitoring is an unfavourable sign calling for correction of diet, motor activity and medicamentous therapy.
