We report a patient with herpes simplex virus encephalitis who presented with left hemiparesis and progressed to aphasia and generalized spasticity. Computerized axial brain tomography with and without infusion of contrast medium was normal, as were the cerebrospinal fluid findings. However, magnetic resonance imaging scan and brain biopsy, were diagnostic of herpes simplex encephalitis.

The detection of a left ventricular mural thrombus, a potentially fatal complication of various heart diseases, is often missed by current radiographic techniques. The findings on ECG synchronized radionuclide cineangiography in a patient with a large thrombus in the left ventricular cavity, confirmed by echocardiography and angiography are reported. This suggests a role for radionuclide cineangiography in the detection of left ventricular mural thrombus.
Renal cysts are common lesions especially in the older population. Although the occurrence of a tumor within the wall of an otherwise benign cyst is rare, its accurate diagnosis is difficult. We present such a case, diagnosed preoperatively by computed tomography.

Ring lesions are not infrequently noted at the operative site of intracranial tumors at varying intervals of time after surgery. We present a suture granuloma as an unusual cause of such a lesion. Two guidelines that help in the diagnosis and management of these lesions are discussed with emphasis on the value of the precontrast scan.
