Talocalcaneal coalition: diagnosis and surgical management.

The authors studied 22 patients who had 32 feet with symptomatic talocalcaneal coalition. All feet were treated with excision of the bar and interposition of an autogenous free fat graft. These cases were assessed clinically and radiographically before and after the operation. They conclude that this technique is a good surgery with gratifying results. In the final outcome, 78.1% of feet became completely painless and 21.8% achieved relief of pain. There was improvement of the deformity in 68.7% and of range of motion in 75%.

Diverticulitis of the midrectum.

Diverticulitis of the rectum is a rare condition. This report covers patient history, diagnosis, and treatment involved in such a case. The patient presented with a history of rectal pain and muscle spasm of six months' duration. After several available examinations had been completed, i.e., digital examination, sigmoidoscopy, and barium-enema examination, the diagnosis of a rectal diverticulum was made. Initially, conservative treatment, including high-fiber diet and sitz baths, proved effective. Approximately nine months later, the patient developed severe rectal pain, unrelieved by previously effective measures. After the above-described examinations had been repeated, the rectal wall was found to be ulcerated and inflamed, and a diagnosis of diverticulitis of the rectum was made. Antibiotic therapy and evacuation of the 3- to 4-cm mass under anesthesia resulted in subsidence of symptoms and resolution of the mass. Segmental resection will be considered if the diverticulum becomes infected again.

Antral gastrin concentration in patients with vagotomy and pyloroplasty.

Antral gastrin concentration was measured in endoscopic forceps biopsy specimens and prepyloric mucosa in patients with vagotomy and pyloroplasty, duodenal ulcer patients and nonulcer patients. Antral gastrin concentration in vagotomy and pyloroplasty patients, 35.5 +/- 4.1 ng of gastrin per mg of tissue (mean +/- SEM), was significantly greater (P less than 0.01) than antral gastrin concentration in duodenal ulcer patients, 13.0 +/- 2.4 ng per mg, and antral gastrin concentration in controls, 14.8 +/- 3.1 ng per mg. Basal and meal-stimulated serum gastrin levels in vagotomy and pyloroplasty patients were also significantly greater (P less than 0.05) than gastrin levels in either duodenal ulcer patients or controls. Acid secretion rates were not significantly correlated with serum or antral gastrin concentrations in the patients groups.

Universal coupled isometric guide for ligament reconstruction of the knee ("G.U.I.A.").

The author presents a new drill guide for reconstruction of the knee ligament called "Guia Universal Isométrico Acoplado--G.U.I.A." (Universal Coupled Isometric Guide). The guides uses the posterior region of the intercondylar notch to obtain the femoral tunnel for the anterior cruciate ligament (ACL). Based on biomechanics, literature and cadaver knee studies, the guide has proved to be practical, to facilitate the preparation of isometric tunnels and to avoid iatrogenic injuries.
