[Initial clinical experience with the use of 31P MR spectroscopy in the examination of children with neuromuscular diseases].

31P MR spectroscopy (MRS) was used to investigate neuromuscular diseases of different origin in 35 patients, mostly children. After elaboration of the procedure of MR examination and evaluation of results the method of pattern recognition analysis was used. According to the results of 31P MR spectroscopy patients can be divided into three groups. Group A comprises patients with severe muscular lesions such as Duchenne dystrophy. The second group B is characterized by milder atrophic changes of muscle fibres. The third group comprises obscure cases. The 31P MRS method was also used for investigating the development of the disease in time--a drop of parameter PCR/Pi corresponds to a deterioration of the clinical condition of patients from group one. Maintenance of approximately the same values was observed in group B. An increase of PCR/Pi was also observed during administration of Prednisone in patients with child polymyositis.

Adolescent

[Techniques of endoscopic treatment of malignant colorectal adenomas with respect to the subsequent prognosis of the patients].

Since 1981 we detected in 45 patients (28 men, 17 women) 31 adenomas with severe dysplasia (ATD; formerly intramucous carcinoma) and 22 invasive carcinomas in the adenoma (IK penetrating beneath the muscularis mucosae). 95% of all findings were in the left colon before the lienal flexure. Patients with ATD were subjected only to endoscopic operation; of 22 of the subjects with IK 15 were recommended for surgery, and endoscopic treatment alone was provided to seven patients. Forty subjects were followed up on a long-term basis (on average for 34 months). Of 23 patients with ATD one female patient died from ischaemic heart disease, the remainder survive without signs of colorectal carcinoma. Of 17 subjects with IK followed up on a long-term basis five died (all after surgery as a result of generalization of the disease). All 7 patients with IK who had only endoscopic treatment survive without signs of relapse of the disease. In patients with ATD the authors consider EP as the definite therapeutic method, if it is feasible from the technical aspect. Patients with ATD and IK who were subjected to EP are checked after 6 weeks. A total coloscopic check-up is made subsequently in subjects with IK during the first year twice a year and in later years once a year, in subjects with ATD after annual intervals for a period of 5-6 years.

Adenoma

[Prognostic factors in non-Hodgkin's malignant lymphoma].

A group of 271 patients with non-Hodgkin's malignant lymphomas from years 1975-1989 was tested using multivariant statistical analysis, from the point of view of the prognostic value their basic clinical and laboratory data. In the group of low grade malignancy lymphomas, the following factors showed a prognostic validity: clinical stage, general status, centrocytic histological subtype, and anaemia. In the group of high grade malignant lymphomas following one were set as prognostically important: general status, clinical stage and age. In particular histological subtypes of low grade malignancy statistically valid difference of survival was not proved, the centrocytic type excluded. However, the difference was found between these types and particular subtypes of high malignancy grade. Based on these results, the Cox's risk model was made, enabling us to define 4 risk groups according to the histological subtype and clinical stage: group A (risk less than 0.5), group B (risk = 0.5-1.0), group C (risk = 1.0-2.0), group D (risk greater than 2.0).

[Diagnosis of G(M2) gangliosidosis in routine practice].

Diagnosis of GM2 gangliosidosis and other most frequent thesaurismoses of the central nervous system was exposed on a case of a 2-year-old boy. Diagnostic process which exploited routine methods commonly used in every department of pathology enabled to choose chemical and enzymological investigation of brain tissue.

[Atypical aneurysms of the abdominal aorta].

The authors submit a report on two patients with pseudoaneurysmatic bulging in the subrenal portion of the abdominal aorta. They present detailed histological findings in the two operated patients and discuss the aetiology of these formations. They draw attention on the failure of aortographic examinations in the diagnosis of these formations and the necessity of sonographic or CT examination. During surgery only partial resection of these formations is possible with subsequent reconstruction of the arterial circulation. The long-term results of these operations are favourable.

Aged

[Papovavirus papillomatosis in beagles].

Canine papillomatosis was diagnosed in 5 out of 12 dogs of the beagle race, aged three to four months, two weeks after arrival from a breeding station. The histological as well as electronmicroscopical findings were typical of the disease.

An unusual case of phospholipidosis.

We present the results of a structural, histochemical and lipid-chromatographic study of tissues obtained at postmortem from an unusual case of phospholipidosis. A previous biopsy of the appendix and liver (Elleder et al., 1975a) had revealed a predominance of phosphoglyceride storage, principally of lysobisphosphatidic acid (LBPA) postmortem material showed that this lipid was stored exclusively in central neurons. In the spleen and the lymph node, however, sphingomyelin (SP) was shown, histochemically and chromatographically, to be the main lipid stored. Total sphingomyelinase (SPase) activity in the appendix was reduced to about 50% of normal. Neuroaxonal dystrophy (NAD) and a conspicuous discrepancy between the degree of distension of some neurons and their lipid content deserve special mention. The case is contrasted with classical sphingomyelinosis; the complexity of the Niemann-Pick group of diseases is discussed as an indication of the difficulties of classification of any atypical case.

Appendix

[Toxicity of silica-gel polymers].

Mice of the ICR strain were tested for the effect of intraperitoneally applied polymers of silicagel. Silicagel applied in the form of spherical-shaped particles caused local irritation and resorptive reaction with granulocytes. There was also renal damage of the type of necronephrosis and signs of necronephrosis.

[Morphological findings in RES modified by methylpalmitate].

Intravenous injections of methylpalmitate (MP) suspension in extreme doses of 3 g/kg of the animal's weight were seen developing in rats after 24 hours manifestations of phagocytosis in the cells of the red pulp of spleen followed by red pulp necrosis. Smaller doses of 1-1.5 g MP/kg of b.w. caused no pronounced morphological changes in the animal's organs.
