[Advanced carcinomas of the stomach. II. Histochemical properties of mucin].

Histochemical features of mucin were studied in a group of advanced cancers of the stomach. Study failed to find any essential difference in mucosubstances composition between diffuse type and intestinal type carcinomas. Proportion of single sulphated, non-sulphated and neutral components was always changing even in different parts of one tumour. An acid mucus with extracellular and intracellular admixture of O-acylsialomucin prevailed in 4 mucinous carcinomas similarly as in colorectal mucinous carcinomas. The study showed that single histologic types of carcinoma in different abdominal parts of digestive tract could not match in frequency but shared features of secretion without making any difference in localization. Occurrence of mixed tumours gives evidence of derivation of carcinomas from a common stem cell with various differentiang abilities.

[Advanced carcinoma of the stomach--I. Prognostically important signs].

Fifty one bioptic cases of resected advanced cancer of the stomach (34 males--mean age 67.8 years 17 females--mean age 75.9 years) were studied in a five-year period (1982-1986). Nine patients (17.6 per cent) has survived without any clinical or laboratory symptoms of disease on average 4 years and 8 months after operation, two patients less than 3 years, seven more than 3 years. Two of them had diffuse type carcinoma (DC), 7 had intestinal type carcinoma (IC) according to Lauren. The group was simultaneously evaluated according to Ming's classification as expansive (infiltrative types of carcinoma respectively. Both classification schemes gave similar results and combined with TNM system yielded sufficient prognostic information. According to the TNM system all cancers (excepting one DC) were stage II. pT2, pNO. IC had as much as 8 cm in diameter (T3), regional lymph nodes in 6 patients produced a favourable type of reaction with predominating lymphocytes of germinal centres mostly combined-with sinus histiocytosis. Absence of lymph node metastasis was more important for prognosis than the favourable type of lymph node reaction.

[Histochemical properties of mucus in epithelial tumors of the large intestine].

In a group of 9 polypous adenomas of the rectum and 27 adenocarcinomas of the large intestine histochemical properties of mucus were investigated. Malignization in the large intestine was associated with the disappearance of O-acyl esters of sialic acids, the presence of which was detected by the method PB KOH PAS only focally, mainly in an extracellular localization in 21.7% of all cases. In tubular adenocarcinomas (16 cases) mixed mucus with a different content of PAS+ mucosubstances predominated. With the PAS AM 2.5 method bluish violet to magenta colouration. Sulphomucin was not proved in these tumours, O-acyl esters of sialomucin only twice. Mucus with more acid histochemical properties (PAS AM 2.5 blue colouration) with sulphomucin (alcioanophilia at pH 1) was secreted by carcinomas of papillary structure (7 cases), the majority intensely mucus forming similarly as villous adenoma. We may therefore, consistent with data in the literature, assume their histogenetic relationship.
