Vertical transmission of the virus of hepatitis B and neonatal hepatic cirrhosis in an Argentine family.

In a family in which the mother was a healthy HBsAg and anti HBc carrier, and the father had a chronic persistent hepatitis with anti HBs and anti HBc positive, the development of neonatal cirrhosis in two of the children was proved to be present. One of the girls was an anti HBs carrier (anti HBc was not determined) and the other had positive anti HBs and anti HBc. Both died and the diagnosis of cirrhosis was confirmed. The two healthy daughters were anti HBc carriers.

[Endoscopic treatment of ureteral lithiasis. Results of our second series of 100 cases].

The combined utilization of transurethral ureterorenoscopy (TUR) and extracorporeal shock wave lithotripsy (ESWL) have caused a revolution in the treatment of ureteral lithiasis. We present a total 113 TUR due to ureteral lithiasis performed in period of 20 months, from March 1988 to November 1989. In the last ten months of the study we used TUR an ESWL for the treatment of ureteral lithiasis. Results show success in 92.93% of the treatments; 8 cases of open surgery (7.07%), five of these were performed during the first 10 months of the study, when ESWL was not available, with only 2 serious complications (1.75%) and one death due to massive pulmonary embolism. TUR is an unsubstitutable technique for the treatment of ureteral lithiasis and when combined with ESWL obtains optimum results. It is our technique of choice in all cases non susceptible to treatment with in situ ESWL due to the impossibility of locating the lithiasis without auxiliary endourological manoeuvres.

[Ceroid-lipofuscinosis: ultrastructural study of 8 cases].

The authors studied the clinical and ultrastructural features of 8 cases of Neuronal Ceroid-Lipofuscinosis (NCL). Five cases of the late-infantile type and 3 cases of the juvenile type. The clinical diagnosis of all cases was confirmed by the electron microscopy analysis of the peripheral blood lymphocytes and biopsy material from the skin, palpebral conjunctiva, skeletal muscle and rectal mucosa. Our observations agree with previous reports concerning the predominance of curvilinear bodies in the cells of the late infantile type, and fingerprint-like structures in those of the juvenile type. The finding of parallel tubular arrays and NCL inclusions associated in the same lymphocyte (frequently in the same vacuole) suggests that those structures could be related to the metabolic error of NCL. Ultrastructural examination of microbuffycoats of peripheric blood lymphocytes is a simple, time-saving and reliable method that allows specific confirmation of a clinical diagnosis of Neuronal Ceroid-Lipofuscinosis.

[Our experience in the endoscopic treatment of ureteral lithiasis. 120 cases of transurethral ureterorenoscopy].

We present our experience over 3 years in the treatment of ureteral lithiasis by means of transurethral ureteroscopy. We analyze the results of our first 120 cases. The percentage of overall successes is 74%, which varies significantly according to whether the calculus is located in the upper or lower third of the ureter and according to the curve of apprenticeship. Iatrogenic problems drop considerably after 50 cases. Rigid transurethral ureteroscopy has become our preferential therapeutic procedure in practically every type of ureteral lithiasis and we have discarded flexible transurethral ureteroscopy for its useless results.

[How our pediatric services work].

The results of a questionnaire sent by the College of Pediatrics of the Portuguese Doctors Association (Ordem dos Médicos) to the pediatric departments all over the country are presented. From the answers of fifty-one services, physical conditions, equipment, human resources, assistancial and scientific activities developed are characterized.
